Transplantation Ethics: Are We Approaching the Crossroads?

Ethical issues of organ transplantation are of extreme importance to involved physicians and to society. The basic principle is that the donor cannot be considered as a commodity; financial incentives must not occur. Regarding deceased-donor organ transplantations, ethical issues are related to the consent for donation, determination of death, and principles of organ allocation. Living donors should be healthy, giving free consent after being fully informed about the risks of the procedure. Transplant professionals have a double responsibility because they must remember about the rights of the organ recipient as well as of the donor. Because of the organ shortage we commonly use organ recovery from donors after cardiac arrest and extend the living-donor pool, practices that may influence some important ethical principles. A proper detailed determination of donor death is of utmost importance. The dead donor rule must be preserved. However category III non heart-beating donors (so-called controlled cardiac arrest) raise doubts as to the time of the decision to transfer the dying (not yet dead) patient to the operating room to withhold supportive treatment. In certain centers, not quite healthy living donors are being used; they are called extended-criteria or complex donors. Although organ trade is condemned, some workers agree to use as complete strangers donors, obviously believing in pure altruistic motivation of such donors without the additional incentives. Finally, is the trend to consider utility in organ allocation justified? It seems that quite soon we may need a new transplantation ethics code that is not totally directed by transplant professionals exclusively to the needs of potential recipients.

Summary: This study provides new information on the efficacy of PD-L1-targeted CAR against PD-L1(low) targets. The results show that PD-L1-CAR cells have strong reactivity and cytotoxicity against both PD-L1(high) and PD-L1(low) target cells. Additionally, PD-L1-CAR cells also exhibit potent cytotoxic effects against non-malignant cells.

Ibuprofen in Therapeutic Concentrations Affects the Secretion of Human Bone Marrow Mesenchymal Stromal Cells, but Not Their Proliferative and Migratory Capacity

Summary: This study investigated the effects of nonselective COX inhibitor ibuprofen on human bone marrow MSCs (hBM-MSCs). The results showed that ibuprofen reduced PGE(2) secretion, but did not significantly affect proliferation and migration of MSCs. However, it significantly decreased the secretion of certain pro-regenerative factors.

Fluorine-Containing Drug Administration in Rats Results in Fluorination of Selected Proteins in Liver and Brain Tissue

Summary: Administration of the fluorinated drug cinacalcet in rats resulted in tissue-specific fluorination of proteins, with three fluorinated proteins identified in the liver and four in the brain tissue after 21 days of treatment. The fate of fluorine released from fluorine-containing drugs and its impact on protein fluorination warrant further investigation.

Costs of Treatment of Acute Antibody-Mediated Rejection in Kidney Transplant Recipients

Summary: This study assessed the costs of treating acute AMR in the Polish settings, finding that the treatment costs are considerably high and nearly double that of hemodialysis. Intravenous immunoglobulin was the main contributing factor to these costs.

Summary: Cross-kingdom gene expression regulation by xenomiRs, particularly from plants, has been found to have potential scientific significance. In this study, miR172a derived from Brassica oleracea was shown to decrease the levels of FAN protein in human and mouse cells. Furthermore, treatment with miR172a was found to mitigate inflammation in a mouse model of rheumatoid arthritis.

Comparison of Post-Transplantation Lymphoproliferative Disorder Risk and Prognostic Factors between Kidney and Liver Transplant Recipients

Summary: The study compares the risk and prognostic factors of PTLD between adult kidney and liver transplant recipients. The results showed that PTLD develops earlier in liver transplant recipients, especially in patients over 45 years old, those receiving tacrolimus or not receiving cyclosporin. In kidney transplant recipients, PTLD onset is significantly affected by tacrolimus use, male sex, and age over 45 years old. Survival is longer in liver transplant recipients under 45 years old and they are more likely to achieve complete remission. The factors affecting PTLD development and outcome differ between kidney and liver transplant recipients.

Osteopontin-A Potential Biomarker for IgA Nephropathy: Machine Learning Application

Summary: This study investigates osteopontin (OPN) as a potential biomarker in nephrology. Using machine learning, urinary OPN levels were found to accurately differentiate between immunoglobulin A nephropathy (IgAN) and other glomerulopathies (GNs). However, the discrimination of membranous nephropathy (MN) and lupus nephritis (LN) was less effective. OPN appears to be a valuable biomarker for identifying IgAN patients and should be further validated in future studies.

The Role of COX-2 and PGE2 in the Regulation of Immunomodulation and Other Functions of Mesenchymal Stromal Cells

Summary: The molecular mechanisms underlying the immunomodulatory properties of MSCs and the role of PGE2 and COX-2 in these properties are not fully understood. This article summarizes previous observations regarding the role of PGE2 and COX-2 in the immunomodulatory properties and other vital functions of MSCs. The most consistent results suggest that MSC-derived PGE2 inhibits the early maturation of dendritic cells, suppresses the proliferation of activated lymphocytes, and stimulates the differentiation of macrophages into M2 phenotype. COX-2/PGE2 also plays an important role in maintaining the basic life functions of MSCs and affects the formation of niches conducive to hematopoiesis and carcinogenesis.

Summary: Genome-wide association analyses have identified 30 risk loci for IgA nephropathy, a progressive form of kidney disease. Functional annotations of potential causal genes suggest inflammatory signaling pathways and cytokine ligand-receptor pairs as potential new drug targets. These findings provide valuable insights into the genetic basis of IgA nephropathy and offer potential avenues for developing therapeutic interventions.

Summary: Chronic kidney disease (CKD) is diagnosed using serum creatinine or cystatin C levels, but more sensitive biomarkers are needed. This study aims to verify the correlation between saliva and serum levels of trimethylamine N-oxide (TMAO) in CKD patients. The results show a good correlation between TMAO levels in saliva and serum, suggesting that saliva can be used for non-invasive monitoring of renal failure in CKD.

Hybrid Immunity Provides the Best COVID-19 Humoral Response in Immunocompromised Patients with or without SARS-CoV-2 Infection History

Summary: The study evaluated the role of SARS-CoV-2 infection history in the vaccination schedules of kidney and liver transplant recipients and patients with chronic kidney disease (CKD). The results showed that patients with a COVID-19 history had higher antibody levels after two doses compared to three doses. Two doses also resulted in a longer median time to infection. The study suggests that vaccination schedules should take into account vaccine response rate and COVID-19 history.

Summary: Hypertension is a risk factor for graft failure and mortality among kidney transplant recipients. Monitoring blood pressure and adequate treatment contribute to longer survival of the graft and recipient.
