Impact of demographic factors, medication and symptoms on disease-specific quality of life in inflammatory bowel disease

To investigate the relation of demographic factors, medication and symptoms to health-related quality of life in patients with inflammatory bowel disease, and to identify patients in need of special support. A questionnaire packet comprising the Inflammatory Bowel Disease Questionnaire (IBDQ) and the European Federation of Crohn's and Colitis Associations' (EFCCA) Survey Questionnaire was sent to 3,852 adult members of the Crohn and Colitis Association of Finland. IBDQ was used for measuring health-related quality of life (HRQoL), and the EFCCA questionnaire gave information about demographics, symptoms and medication. Altogether 2,386 questionnaires (62%) were available for analysis. Patients reporting symptoms affecting leisure activities or work had significantly lower total IBDQ scores (indicating worse HRQoL) than patients with less disturbing symptoms. Satisfaction with current therapy, female gender and age affected the scores. Subjects who had undergone surgery scored lower than those who had not. Patients currently receiving corticosteroids and newly diagnosed patients had lower scores than other patients. In everyday practice, paying attention to and reducing patients' symptoms and their impact on daily life is important when aiming at improving HRQoL. Special attention should be given to patients who have undergone surgery, and to newly diagnosed patients.

Summary: The study aimed to establish robust DNA methylation biomarkers in bile for early and accurate diagnosis of CCA in PSC. Using ddPCR analysis, high accuracy was achieved in detecting CCA in PSC patients, even up to 12 months before official diagnosis, indicating the potential of these biomarkers to complement current detection methods.

Incidence and Mortality in Upper Gastrointestinal Cancer After Negative Endoscopy for Gastroesophageal Reflux Disease

Summary: This population-based cohort study conducted in Denmark, Finland, Norway, and Sweden from 1979 to 2018 found that negative upper endoscopy in patients with GERD is associated with a significant and long-lasting decrease in both incidence and mortality of upper gastrointestinal cancer. The risk reduction persisted for at least 5 years in incidence and 10 years in mortality after a negative upper endoscopy.

Summary: This study aims to evaluate the burden of hepatitis C virus (HCV) in 2020 and forecast the burden by 2030. Using literature review, Delphi process, and mathematical modelling, it estimated the global prevalence of viraemic HCV infection to be 0.7% in 2020, corresponding to 56.8 million infections. The findings suggest that the current efforts are not on track to achieve the global elimination targets by 2030.

Novel histological scoring for predicting disease outcome in primary sclerosing cholangitis

Summary: This study aimed to develop a histological classification system, the PSC histoscore, for PSC. The results showed that the PSC histoscore better correlated with clinical endpoints, and an elevated total PSC histoscore increased the risk for disease progression.

Automated image analysis of keratin 7 staining can predict disease outcome in primary sclerosing cholangitis

Summary: A study found that measuring the cytokeratin 7 (K7) load in liver samples using an artificial intelligence (AI) model can serve as an efficient surrogate marker to evaluate disease progression in primary sclerosing cholangitis (PSC). The K7-AI model 2.0 was more accurate in predicting the compound endpoint compared to other serum markers and pathological evaluations, and it also provided additional variables that cannot be measured by human pathologists.

Abdominal obesity and alcohol use modify the impact of genetic risk for incident advanced liver disease in the general population

Summary: This study investigates the association between genetic variants, abdominal obesity, alcohol use and incident liver disease (ILD). Variants in genes such as PNPLA3, IFNL4, and TM6SF2 are found to be associated with ILD. Genetic risk scores (GRSs) calculated from these variants are exponentially associated with ILD, while not associated with waist-hip ratio (WHR) or alcohol use. The risk of ILD in individuals with high GRS and high WHR or alcohol use is increased by up to 90-fold.

Combined use of the ELF test and CLivD score improves prediction of liver-related outcomes in the general population

Summary: This study evaluated the predictive performance of a combination of liver fibrosis assessment, phenotype profile, and genetic risk in predicting liver-related outcomes. The combination of the chronic liver disease risk score and the enhanced liver fibrosis test significantly improved the predictive performance compared to the enhanced liver fibrosis test alone. The addition of the polygenic risk score did not significantly increase the predictive performance.
