Background: Genetic association studies have revealed numerous polymorphisms conferring susceptibility to melanoma. We aimed to replicate previously discovered melanoma-associated single-nucleotide polymorphisms (SNPs) in a Greek case-control population, and examine their predictive value. Methods: Based on a field synopsis of genetic variants of melanoma (MelGene), we genotyped 284 patients and 284 controls at 34 melanoma-associated SNPs of which 19 derived from GWAS. We tested each one of the 33 SNPs passing quality control for association with melanoma both with and without accounting for the presence of well-established phenotypic risk factors. We compared the risk allele frequencies between the Greek population and the HapMap CEU sample. Finally, we evaluated the predictive ability of the replicated SNPs. Results: Risk allele frequencies were significantly lower compared to the HapMap CEU for eight SNPs (rs16891982 - SLC45A2, rs12203592 - IRF4, rs258322 - CDK10, rs1805007 - MC1R, rs1805008 - MC1R, rs910873 - PIGU, rs17305573 - PIGU, and rs1885120 - MTAP) and higher for one SNP (rs6001027 - PLA2G6) indicating a different profile of genetic susceptibility in the studied population. Previously identified effect estimates modestly correlated with those found in our population (r = 0.72, P<0.0001). The strongest associations were observed for rs401681-T in CLPTM1L (odds ratio [OR] 1.60, 95% CI 1.22-2.10; P = 0.001), rs16891982-C in SCL45A2 (OR 0.51, 95% CI 0.34-0.76; P = 0.001), and rs1805007-T in MC1R (OR 4.38, 95% CI 2.03-9.43; P = 2610 25). Nominally statistically significant associations were seen also for another 5 variants (rs258322-T in CDK10, rs1805005-T in MC1R, rs1885120-C in MYH7B, rs2218220-T in MTAP and rs4911442-G in the ASIP region). The addition of all SNPs with nominal significance to a clinical non-genetic model did not substantially improve melanoma risk prediction (AUC for clinical model 83.3% versus 83.9%, p = 0.66). Conclusion: Overall, our study has validated genetic variants that are likely to contribute to melanoma susceptibility in the Greek population.

Prognostic factors for adverse outcomes in patients with COVID-19: a field-wide systematic review and meta-analysis

Vanesa Bellou, Ioanna Tzoulaki, Maarten van Smeden, Karel G. M. Moons, Evangelos Evangelou, Lazaros Belbasis

Summary: This study conducted a systematic review and meta-analysis to investigate the prognostic factors for COVID-19 patients. The results indicated that factors such as angiotensin-converting enzyme inhibitors, obstructive sleep apnoea, and pharyngalgia were associated with at least one adverse outcome. However, there were some issues identified in the risk of bias assessment.

Summary: The introduction of ICIs revolutionized oncologic therapy but also brought the risk of irAEs, with dermatologic irAEs being common. The EADV task force provided treatment recommendations for skin toxicities, focusing on balancing symptom relief with ongoing oncologic treatment.

Summary: Pythia is a literature identification system that combines query formulation and citation screening. Across seven systematic reviews, Pythia's sensitivity ranges from 0.09 to 0.58. The number of relevant abstracts reviewed per needed abstract number varies across the reviews. Overall, reviews with higher sensitivity retrieve more relevant citations in early batches, but other factors generally do not impact retrieval.

Comparison of Low-Value Care Among Commercial and Medicaid Enrollees

Summary: Differences in the provision of low-value care varied across measures, with commercial enrollees receiving slightly higher rates of low-value services than Medicaid enrollees. The difference was more pronounced for less expensive services, suggesting that financial incentives may not be the sole driver of low-value care.

A retrospective multicentric cohort study of checkpoint inhibitors-induced pruritus with focus on management

Chryssoula Papageorgiou, Elizabeth Lazaridou, Konstantinos Lallas, Kyparissos Papaioannou, Vasiliki Nikolaou, Valeria Mateeva, Konstantinos Efthymiadis, Chrysanthi Koukoutzeli, Konstantia Loga, Eleni Sogka, Evangelos Karamitrousis, George Lazaridis, Dimitrios Dionysopoulos, Aimilios Lallas, Christina Kemanetzi, Christina Fotiadou, Eleni Timotheadou, Zoe Apalla

Summary: This study retrospectively analyzed the characteristics of pruritus induced by immune checkpoint inhibitors (ICIs) and evaluated the effectiveness of different therapeutic modalities. The results showed that pruritus per se occurred in 22.0% of ICI-induced pruritus patients, with the majority also presenting with additional cutaneous toxicity. Antihistamines and topical regimens were the first-line treatment, while narrow-band UVB, oral steroids, and GABA analogs were used as second-line interventions in resistant cases. The study confirmed the efficacy of current treatment strategies and suggested NBUVB as a potential steroid-sparing alternative.

Cutaneous Adverse Reactions of Immunotherapy in Patients with Advanced Melanoma

Vasiliki Nikolaou, Antonis Tsimpidakis, Alexander Stratigos

Summary: Immune checkpoint blockers (ICBs) have been widely used for the treatment of melanoma, but they can also cause autoimmune toxicities, especially in the skin. The role of dermatologists is important in detecting and treating these adverse events. Common cutaneous toxicities include rash, psoriasis-like and lichenoid dermatoses, and bullous pemphigoid. Rare toxicities such as sarcoidosis, dermatomyositis, and subacute lupus have also been reported. This review provides a summary of ICB-induced cutaneous toxicities in melanoma patients and discusses management and treatment options.

Summary: This study suggests that alterations in lymphocyte subpopulations are associated with COVID-19 severity markers. Increased levels of CD16CD56 cells are associated with higher risk of lung injury. An increase in CD3CD4 and CD4RO cell count results in a decrease in CRP levels. However, further evaluation in larger scale trials is needed to validate these findings.

Summary: Treatment with CDK4/6 inhibitor in combination with endocrine therapies can improve progression-free survival in patients with hormone receptor-positive, HER2-negative, advanced breast cancer. However, limited data exists on its cutaneous adverse events. This retrospective study investigated the prevalence, types, and management of cutaneous adverse events during CDK4/6 inhibitor treatment.

Summary: This study aimed to quantify the types of health outcomes assessed in randomized controlled trials (RCTs) in primary care settings over the past 20 years and identify potential gaps in specific types of health care and interventions. The results showed research gaps in preventive and palliative care, behavioral interventions, as well as safety and patient-centeredness outcomes as primary outcomes in primary care RCTs.
