A consensus on the diagnosis and treatment of acromegaly complications

In March 2011, the Acromegaly Consensus Group met to revise and update the guidelines on the diagnosis and treatment of acromegaly complications. The meeting was sponsored by the Pituitary Society and the European Neuroendocrinology Association and included experts skilled in the management of acromegaly. Complications considered included cardiovascular, endocrine and metabolic, sleep apnea, bone diseases, and mortality. Outcomes in selected, related clinical conditions were also considered, and included pregnancy, familial acromegaly and invasive macroadenomas. The need for a new disease staging model was considered, and design of such a tool was proposed.

Clinical aspects of multiple endocrine neoplasia type 1

Summary: Multiple endocrine neoplasia type 1 (MEN1) is a rare syndrome characterized by the co-occurrence of primary hyperparathyroidism, duodenopancreatic neuroendocrine tumours (NETs) and/or pituitary adenomas. Patients with MEN1 should undergo tailored biochemical and imaging screening. Knowledge about the clinical phenotype of these tumours has progressed over the past 10 years, particularly in France and the Netherlands.

International Multicenter Validation Study of the SAGIT® Instrument in Acromegaly

Diabetes Increases Severe COVID-19 Outcomes Primarily in Younger Adults

Summary: The study suggests that diabetes is an independent risk factor for the severity of COVID-19, particularly in young adults more so than in older adults, especially for individuals younger than 70 years. Patients with diabetes had higher main outcomes, which were inversely proportional to age, and higher mortality rates, especially significant in patients under 50 years old.

Treatment of acromegaly has substantial effects on body composition: a long-term follow-up study

Summary: The treatment of acromegaly strongly affects body composition until biochemical disease remission, characterized by an increase in fat mass and a decrease in lean mass. These changes are closely associated with the normalization of IGF-I and are independent of individual therapy.

IGF-I Variability Over Repeated Measures in Patients With Acromegaly Under Long-Acting Somatostatin Receptor Ligands

Summary: This study compared the serum IGF-I dynamics and variability in patients treated with SRLs, surgically cured patients, and healthy controls. The results showed that IGF-I decreased significantly at Day 7 and Day 14 after injection in SRL-treated patients, but remained stable in cured patients and healthy controls. In addition, SRL-treated patients had higher IGF-I variability, especially in nonoptimally controlled patients. The measurement at the farthest distance from SRL injection was the best predictor of optimal disease control.

Prolactin immunoassay: does the high-dose hook effect still exist?

Summary: The high-dose hook effect still exists in a very small minority of prolactin assays, as demonstrated in a study evaluating different assays in France in 2020. One assay tested was sensitive to the effect, leading to falsely lower prolactin concentration when measuring native serum compared to after dilution. The study suggests that the hook effect should still be considered in prolactin assay recommendations.

Impairment in insulin secretion without changes in insulin resistance explains hyperglycemia in patients with acromegaly treated with pasireotide LAR

Summary: Pasireotide, a second-generation somatostatin receptor ligand (SRL) used for treating acromegaly, may be associated with glucose metabolism impairment. In a retrospective study of 33 patients, we observed that treatment with pasireotide was associated with normalization of serum IGF-1 in almost 60% of patients, but one-third of patients developed diabetes. In the patients who stopped pasireotide because of hyperglycemia, HbA1c promptly decreased. Longitudinal data in 14 patients show that diabetes is mediated by impaired insulin secretion, which occurred shortly and then remained stable on long term, while no significant changes in insulin sensitivity were observed, despite a marked reduction of GH/IGF-1 concentrations. Older age and a worse glycemic control at baseline were the strongest predictors for hyperglycemia.

Despite normalized urinary free cortisol levels, a subset of patients still displayed altered circadian rhythm of cortisol secretion.

Summary: Cushing's syndrome (CS) affects the gonadotrope axis and testicular functions differently depending on the intensity of hypercortisolism and underlying etiology. Excess cortisol caused by CS leads to varying degrees of hypogonadotropic hypogonadism (HH), with more severe impairment in men with intense hypercortisolism associated with paraneoplastic/ectopic adrenocorticotrophic hormone (ACTH) secretion (EAS).
