The present study investigated myocardial T2* heterogeneity in thalassaemia major (TM) patients by cardiac magnetic resonance (CMR), to determine whether is related to inhomogeneous iron overload distribution. A total of 230 TM patients consecutively referred to our laboratory were studied retrospectively. Three short-axis views (basal, medium and apical) of the left ventricle (LV) were obtained by multislice multiecho T2* CMR. T2* segmental distribution was mapped on a 16-segment LV model. The level of heterogeneity of the T2* segmental distribution, evaluated by the coefficient of variation (CoV), was compared with that of a surrogate data set, to determine whether the inhomogeneous segmental distribution of T2* could be generated by susceptibility artefacts. Susceptibility artefacts offer an explanation for the T2* heterogeneity observed in patients without iron overload. In subjects with global T2* below the lower limit of the normal, T2* heterogeneity increased abruptly which could not be explained by artefactual effects. Some segmental T2* values were below and others above the limit of normal threshold (20 ms) in 104 (45%) TM patients. Among these patients, 74% showed a normal T2* global value. In conclusion, a true heterogeneity in the iron overload distribution may be present in TM patients. Heterogeneity seemingly appears in the borderline myocardial iron and stabilizes at moderate to severe iron burden. Copyright (C) 2009 John Wiley & Sons, Ltd.

Summary: The aim of this multicenter study was to assess the predictive value of multiparametric cardiac magnetic resonance (CMR) for cardiovascular complications in sickle cell disease (SCD) patients. Reduced left ventricular ejection fraction and increased right ventricular mass index showed a significant prognostic value in patients with SCD.

Summary: This study evaluated the prevalence of myocardial involvement in SSc patients using native T1 and T2 mapping, as well as the diagnostic performance of mapping in addition to conventional LLC. The results showed that myocardial T1 and T2 values were significantly higher in SSc patients. Native T1 and/or T2 increases were present in 62.1% of patients with normal conventional CMR techniques.

Gender Differences and Cardiometabolic Risk: The Importance of the Risk Factors

Summary: Metabolic syndrome is a clinical condition characterized by the aggregation of major risk factors for cardiovascular disease and type 2 diabetes, including dyslipidemia, high blood pressure, abnormal blood sugar, and abdominal obesity. These risk factors have an independent effect but become synergistic when combined, leading to a doubled risk of cardiovascular diseases and a 1.5-fold increase in all-cause mortality. This article explores gender differences in the epidemiology, etiology, pathophysiology, and clinical expression of metabolic syndrome components, as well as gender differences in new biochemical markers of metabolic syndrome and cardiovascular risk.

Summary: COVID-19 is a highly variable disease with different symptoms and signs. Men are more susceptible to the infection, with a higher rate of hospitalization and mortality compared to women. The impact of cardiovascular diseases on COVID-19 outcomes differs between men and women, potentially due to sex-related biological factors and behavioral traits. The renin-angiotensin system and the angiotensin-converting enzyme 2 play key roles in disease pathogenesis, showing hormone-dependent actions. Men also have a higher incidence of myocardial injury associated with inflammation and coagulation disorders. The relationship between cardiovascular diseases and COVID-19 during pregnancy is also discussed.

Summary: This study investigated the ability of radiomic features extracted by two different radiomic platforms to predict clinical outcomes in patients treated with radiosurgery for brain metastases from non-small cell lung cancer. The results showed differences in the quality and number of features extracted using the two platforms, leading to mismatches in terms of performance, statistical significance, and clinical variables found in combined models. This study sheds new light on the discrepancies that can arise from extracting radiomic features using different platforms.

Bone status and HCV infection in thalassemia major patients

Summary: The purpose of this study was to investigate the relationship between hepatitis C virus (HCV) infection and bone mineral density (BMD), vitamin D, and bone turnover biomarkers in multi-transfused patients with thalassemia major (TM). The results showed that HCV infection was positively associated with the prevalence of low bone mass, suggesting that HCV infection is an additive risk factor for low bone mass in TM patients. Osteocalcin may play a role in this association.

Summary: This paper focuses on using the Visual Transformer (ViT) neural network architecture to automatically detect the left ventricle (LV) from cardiac magnetic resonance (CMR) relaxometry sequences. Performance evaluation showed variations in performance based on slice location, but overall performance was comparable to other state-of-the-art methods. The study confirms the feasibility of using ViT architectures for LV detection and establishes a benchmark for relaxometry imaging.

Summary: In this study, myocardial T-2 values were measured by a segmental approach in patients with thalassaemia major (TM), comparing them with T-2* values for the detection of myocardial iron overload (MIO), evaluating their potential in detecting subclinical inflammation, and correlating with clinical status. The study found that T-2 values were significantly higher in TM patients than in a cohort of healthy subjects, and there was a significant correlation between T-2 and T-2* values. Patients with decreased T-2* values also had reduced T-2 values, and these patients had higher hepatic and pancreatic iron deposition.
