The study of an Australasian MOGAD cohort revealed a subgroup possibly mediated by coexistent autoantibodies, presenting with myeloradiculitis, combined central and peripheral demyelination syndromes, and inflammatory neuropathies, being responsive to immunotherapy.

Summary: Pain is an under-recognized association of LGI1 and CASPR2 antibodies, with CASPR2 antibody patients experiencing pain at a higher rate (52%) compared to LGI1 antibody patients (19%). Pain in LGI1 antibody patients responded well to immunotherapy, while CASPR2 antibody patients showed greater residual impairment. Serum CASPR2 antibodies, but not LGI1 antibodies, bound to sensory neurons, indicating potential pathophysiological differences underlying clinical observations.

Genomic Insights into Myasthenia Gravis Identify Distinct Immunological Mechanisms in Early and Late Onset Disease

Summary: This study aimed to identify disease-relevant genes and explore immunological mechanisms contributing to early and late onset forms of myasthenia gravis. They found distinct immunological pathways linked to early and late onset of the disease, with early onset genes being enriched for regulators of toll-like receptor 4 signaling, and late onset genes being exclusively linked to T-cell regulators CD28 and CTLA4. Additionally, causal genetic variants near the CTLA4 gene were identified, resulting in low expression of CTLA4.

Summary: The study found that HLA alleles DRB1*07:01 and DQA1*02:01 are associated with anti-LGI1 encephalitis, independent of variation at DRB3 and DQB1 loci. Additionally, the DRB1*04:02 allele is also independently associated with the disease and lower age at onset. These findings provide evidence for secondary effects within the HLA locus that correlate with clinical phenotypes in anti-LGI1 encephalitis.

Summary: Loss of function of LGI1 gene is associated with epileptic disorders, and patients with LGI1-directed autoantibodies have frequent focal seizures. LGI1 consists of Leucine-Rich Repeat (LRR) and Epitempin (EPTP) domains, which interact with transsynaptic complex at excitatory synapses. Specific LGI1-autoantibodies against LRR domain can modulate neuronal excitability, potentially accounting for the observed epileptiform activity in patients.
