Critical roles for thrombin in acute and chronic inflammation

Thrombin can amplify inflammation induced by other stimuli, either through ischemia (consequent upon thrombosis), indirectly through generation of downstream mediators such as activated protein C, or directly via signals through protease activated receptors (PAR). This paper will summarize recent data from our laboratory indicating that thrombin is required to initiate CCR2-dependent leukocyte recruitment and that it is the principal determinant of the outcome after vascular injury, via PAR-1 activation of a distinct subset of smooth muscle cell progenitors. In both, tissue factor (TF) initiates thrombin generation and the thrombin acts locally, exemplifying that the initiation phase can generate autocrine or paracrine signalling molecules. Thrombin is an important constituent of innate immunity, able to amplify and modify responses to invading pathogens or tissue damage. With novel anti-thrombin therapeutics and agents to target PAR, a new understanding of the importance of thrombin may allow the development of innovative anti-inflammatory strategies.

Summary: B cells play a crucial role in transplant rejection by controlling indirect T cell responses, either through antibody-mediated mechanisms or by presenting donor antigens to T cells. Depletion of B cells prolongs skin graft survival, highlighting their contribution to indirect pathway sensitization. Additionally, B cells may present alloantigens via acquisition of MHC-peptide complexes, and combining B cell depletion with regulatory T cell transfer further enhances skin graft survival.

Regulation of T- and B-cell interactions determines the clinical phenotype associated with donor-specific antibodies

Summary: The cellular mechanisms underlying donor-specific antibody formation and antibody-mediated rejection are still unknown. This study reveals that specific subsets of T-regulatory cells and B-regulatory transitional cells are simultaneously reduced in patients with donor-specific antibody and subsequent antibody-mediated rejection. Furthermore, alterations in specific cytokines and costimulatory molecules are identified as important mechanisms by which these cells suppress donor-specific antibody formation and independently contribute to the progression of antibody-mediated rejection.

A transitional B-cell cytokine biomarker for risk stratifying renal transplant patients with borderline rejection

Summary: Borderline allograft rejection can lead to acute rejection and graft loss in certain patients. Cherukuri et al. have developed a novel test based on transitional T1 B cells that produce interleukin-10 and tumor necrosis factor-a, which can identify high-risk patients for poor outcomes. Further research is needed to understand the potential mechanisms of how transitional T1 B cells modulate alloreactivity, but this biomarker has the potential to stratify patients and guide early intervention.

Summary: This study aims to suppress memory T and B cell responses against specific HLA antigens in highly sensitised kidney transplant patients by introducing autologous polyclonally expanded regulatory T cells. It is a two-part prospective clinical trial, with the first part being an observational phase and the second part being an interventional phase. The results will help determine the feasibility of using Treg therapy to modulate memory immune responses.

Summary: Solid phase immunoassays have improved the detection of donor-specific antibodies (DSA) to human leukocyte antigens (HLA) in kidney transplantation, but the monitoring of DSA in recipients with stable graft function is not universally accepted as standard care. This consensus report aims to assess the clinical utility of DSA monitoring in recipients without signs of graft dysfunction using the Wilson & Junger criteria and presents the findings and recommendations of a dedicated workgroup on subclinical DSA monitoring.
