Inflammation in the pathophysiology of essential hypertension

In spite of the huge amount of research recently performed in this area, the pathogenesis of human hypertension remains elusive. Thus, hypertension has to be defined as essential for the majority of patients with high blood pressure. Given the lack of animal models useful to investigate essential hypertension, we analyze and discuss both clinical and basic research studies indicating that essential hypertension should be considered as a potential multifactorial inflammatory disease. The pathophysiology of essential hypertension might result from interactions between genetic and environmental factors. Morphological abnormalities in the renal parenchyma and arteries have also been shown to determine hypertension. Inflammatory processes might induce renal vasoconstriction, ischemia and injury that can sustain systemic hypertension. Arterial and tubulointerstitial infiltration of inflammatory cells in response to renal damage might further increase renal and vascular alterations through the production of oxidants and other soluble inflammatory mediators. The present review gives an update regarding the latest research on the possible direct role of inflammation in the pathophysiology of essential hypertension.

Summary: Inflammation may play a role in the development of coronary artery disease (CAD) in different cardiovascular risk factor categories. Osteopontin (OPN) levels were highest in the low risk factor group and associated with CAD. Other biomarkers, such as myeloperoxidase (MPO) and resistin, did not differ based on CAD presence.

Summary: This study found that res-like macrophages expressing LYVE-1 play a role in vascular calcification in atherosclerosis. These macrophages promote the transdifferentiation of VSMCs into osteogenic-like cells through the secretion of CCL24. The abundance of LYVE-1(+)CCL24(+) macrophages and elevated systemic levels of CCL24 are associated with vascular calcification and CAD events.

Summary: The study showed that SGLT-2 inhibition by empagliflozin does not impact experimental arterial thrombus formation, neither under baseline conditions nor during sustained low-grade inflammation. The results suggest that the beneficial effects of empagliflozin are likely independent of pathways mediating arterial thrombosis.

Summary: JCAD promotes arterial thrombosis by modulating coagulation and fibrinolysis. Knockout of JCAD reduces thrombogenicity in mice, and silencing of JCAD inhibits the expression of coagulation factors and fibrinolysis factors. These findings suggest JCAD as a potential therapeutic target for atherothrombosis.

Inflammatory biomarkers of ischemic stroke

Summary: Ischemic stroke is the second leading cause of death globally and a major cause of morbidity. Current therapeutic options are limited to early reperfusion strategies, while pharmacological neuroprotective strategies have failed to enter clinical practice despite promising results in experiments. Inflammation plays a significant role in the pathophysiology of ischemic stroke, and inflammatory mediators have been investigated as potential prognostic markers and therapeutic targets. This article summarizes the available evidence on the role of cytokines, soluble adhesion molecules, and adipokines in the pathophysiology, prognosis, and therapy of ischemic stroke.

Obesity phenotypes and cardiovascular risk: From pathophysiology to clinical management

Summary: The obesity epidemic is a global health crisis, affecting over one billion people worldwide. Obesity has a significant impact on cardiovascular health, causing various structural, functional, humoral, and hemodynamic changes. Accurate assessment of cardiovascular risk in individuals with obesity is crucial for reducing mortality rates and maintaining quality of life. Recent evidence suggests the existence of different phenotypes of obesity, each associated with varying degrees of cardiovascular risk. Diagnosis should not solely rely on anthropometric parameters, but also include a thorough evaluation of metabolic status. The World Heart Federation and World Obesity Federation have proposed an action plan for managing obesity-related cardiovascular risk and mortality, emphasizing the importance of comprehensive multidisciplinary programs. This review aims to provide an updated summary of the different obesity phenotypes, their specific effects on cardiovascular risk, and differences in clinical management.
