Summary: In this study, an automated preoperative artificial intelligence algorithm was developed and validated for the segmentation of tumor and lymph nodes in patients with pancreatic ductal adenocarcinoma (PDAC) using CT imaging. The performance of the algorithm was compared to radiologists and clinical models, and it was found that the AI model outperformed the other models in predicting lymph node metastasis. The AI model-predicted positive LN metastasis was also associated with worse survival.

Summary: This study aimed to determine whether the treatment site of neoadjuvant therapy relative to surgery location impacts surgical timing and long-term outcomes. The results showed that the time interval between the location of neoadjuvant chemotherapy and surgery did not significantly affect surgical timing and long-term outcomes. Although patients who received neoadjuvant therapy at local centers may have higher postoperative readmission rates, perioperative mortality and long-term survival were not adversely affected by the location of chemotherapy.

Summary: Background: The study explores the advantages and efficacy of the no-touch isolation principle in pancreaticoduodenectomy (PD). Materials and methods: Among 370 PD patients, a no-touch PD group (n = 70) and a conventional PD group (n = 300) were compared using propensity score matching. Results: The no-touch PD group showed better overall survival and disease-free survival, lower disease-related mortality rates, and lower recurrence and liver metastasis rates compared to the conventional PD group. Conclusions: The no-touch isolation principle could improve long-term survival for pancreatic cancer patients.

Proximity of mesenteric vessels increases the risk of vein involvement, leading to the need for concomitant venous resection to reduce the risk of positive margins. Surgeons must balance the technical difficulty of venous resection with the hope for absence of perivascular tumor infiltration to achieve optimal surgical outcomes in pancreatic cancer cases.

Summary: The study aimed to determine if classifying hypoechogenicity into three degrees could improve the distinction between benign and malignant nodules and influence the Thyroid Imaging Reporting and Data System (TI-RADS) Category 4. A total of 2,574 nodules were retrospectively assessed, and a subanalysis was performed on solid nodules to evaluate mainly TI-RADS 4 nodules. The results showed that mild hypoechogenicity had a lower association with malignancy compared to moderate and marked hypoechogenicity, and there was no significant association between mildly hypoechoic solid nodules and cancer.
