Background and Aim: Environmental factors such as food, lifestyle and prevalence of Helicobacter pylori infection are widely different in Asian countries compared with the West, and physiological functions and genetic factors of Asians may also be different from those of Westerners. Establishing an Asian consensus for functional dyspepsia is crucial in order to attract attention to such data from Asian countries, to articulate the experience and views of Asian experts, and to provide a relevant guide on management of functional dyspepsia for primary care physicians working in Asia. Methods: Consensus team members were selected from Asian experts and consensus development was carried out by using a modified Delphi method. Consensus teams collected published papers on functional dyspepsia especially from Asia and developed candidate consensus statements based on the generated clinical questions. At the first face-to-face meeting, each statement was reviewed and e-mail voting was done twice. At the second face-to-face meeting, final voting on each statement was done using a keypad voting system. A grade of evidence and strength of recommendation were applied to each statement according to the method of the GRADE Working Group. Results: Twenty-nine consensus statements were finalized, including seven definition and diagnosis, five for epidemiology, nine for pathophysiology, and eight for management. Algorithms for diagnosis and management of functional dyspepsia were added. Conclusions: This consensus developed by Asian experts shows distinctive features of functional dyspepsia in Asia and will provide a guide to the diagnosis and management of functional dyspepsia for Asian primary care physicians.

The role of NADPH oxidase 1 in alcohol-induced oxidative stress injury of intestinal epithelial cells

Liuying Chen, Huikuan Chu, Lilin Hu, Zhonglin Li, Ling Yang, Xiaohua Hou

Summary: This study reveals that alcohol consumption can lead to intestinal barrier injury. NOX1, a protein, plays a role in accumulating reactive oxygen species (ROS) and causing cell apoptosis in intestinal epithelial cells. Propionate, an endogenous inhibitor of HDAC2, can alleviate oxidative stress injury and reduce cell apoptosis in the intestines.

A Matching Strategy To Guide Donor Selection for Ulcerative Colitis in Fecal Microbiota Transplantation: Meta-Analysis and Analytic Hierarchy Process

Summary: FMT, a treatment method for UC targeting gut microbiota, has shown varying effectiveness in clinical trials. Donor selection might be a contributing factor to this variability, as a patient's response to FMT could depend on the ability of the used donor's microbiota to restore the specific gut disturbances of the patient. This study developed a donor-recipient matching model to guide donor selection for UC in FMT, based on analysis of fecal bacterial diversity and composition.

Comparison of a Polysaccharide Hemostatic Powder and Conventional Therapy for Peptic Ulcer Bleeding

Da Hyun Jung, Chan Hyuk Park, Soo In Choi, Hye Rim Kim, Myeongjee Lee, Hee Seok Moon, Jun Chul Park

Summary: This study demonstrates that polysaccharide hemostatic powder (PHP) is an effective initial treatment for peptic ulcer bleeding (PUB) and is not inferior to conventional treatments. Further research is needed to confirm the re-bleeding rate of PHP.

Clinical outcome of disorders of gut-brain interaction in secondary care: A longitudinal study

Kee Huat Chuah, Wen Xuan Hian, Aik Tatt Teoh, Justin Kwan Yeu Ling, Sanjiv Mahadeva

Summary: This study investigated the outcome of routine treatment for disorders of gut-brain interaction (DGBI) in secondary care. It found that after 2 years of treatment, 66.1% of patients reported symptom improvement. Reassurance was associated with better symptom improvement, while patients with an overlap syndrome had a poorer outcome, leading to more healthcare consultations and work absenteeism.

Summary: This study aims to summarize changes in recommendations for injection treatments for knee osteoarthritis (OA) in clinical practice guidelines (CPGs) and assess their impact on public interest. The findings show that while CPGs have shifted their recommendations, public interest and YouTube videos have not responded accordingly.

Gastrointestinal Dysfunction in Parkinson's Disease: Neuro-Gastroenterology Perspectives on a Multifaceted Problem

Ai Huey Tan, Kee Huat Chuah, Yuan Ye Beh, Jie Ping Schee, Sanjiv Mahadeva, Shen-Yang Lim

Summary: Patients with Parkinson's disease often experience gastrointestinal symptoms like nausea, bloating, reduced bowel movements, and difficulties with defecation. These symptoms are common but difficult to manage, and objective testing does not always match up with symptoms. Treatment options are limited and evidence is scarce. Upper GI dysfunction can also complicate the pharmacological treatment of motor symptoms in PD. This review discusses the recognition, diagnosis, and management of common GI problems in PD, as well as potential future research.

Summary: The average age of first primary colorectal cancer (CRC) diagnosis in the USA has decreased significantly from 1992 to 2017, with a more pronounced decline after 2000. Proximal CRC cases have a higher age at diagnosis compared to distal CRC cases. Advanced stage CRC patients have a lower age at diagnosis than early stage patients.
