Effects of Varicocelectomy on Sperm DNA Fragmentation, Mitochondrial Function, Chromatin Condensation, and Apoptosis

The aim of this study was to evaluate conventional semen parameters (density, morphology, and progressive motility) and the flow-cytometric parameters of DNA fragmentation, mitochondrial membrane potential, phosphatidylserine externalization, and chromatin compactness in patients with varicocele before and after varicocelectomy. Thirty men (26.5 +/- 3.2 years old, range 20-32 years) with oligoasthenoteratozoospermia and grade 3 left varicocele were selected (without other causes of male infertility). Each of them underwent sperm analysis and flow cytometric evaluation before and 4 months after subinguinal microsurgical varicocelectomy (SMV). After varicocelectomy, men had significantly higher sperm density, progressive motility, and normal forms compared with baseline. They also had a significantly lower percentage of spermatozoa with low mitochondrial membrane potential. After SMV, they showed a significantly lower percentage of spermatozoa with phosphatidylserine externalization, an early sign of apoptosis. Significantly decreased percentages of spermatozoa with abnormal chromatin compactness and spermatozoa with DNA fragmentation were found after SMV compared with baseline. Subinguinal microsurgical varicocelectomy improves sperm function in oligoasthenoteratozoospermia secondary to grade 3 left varicocele. Improvements are seen in conventional parameters and biofunctional parameters not routinely evaluated.

Sexual and Reproductive Outcomes in Obese Fertile Men with Functional Hypogonadism after Treatment with Liraglutide: Preliminary Results

Summary: This study prospectively investigates the effects of liraglutide, a GLP1 analog, on reproductive and sexual function in men with metabolic hypogonadism. This suggests that liraglutide is a useful pharmacological treatment for male patients with metabolic hypogonadism.

Assisted Reproductive Technology without Embryo Discarding or Freezing in Women ≥40 Years: A 5-Year Retrospective Study at a Single Center in Italy

Summary: The commonly used protocols in assisted reproductive technology (ART) involve long-term embryo culture up to the blastocyst stage, freezing of all embryos, and subsequent transfer one by one. These practices have the potential risks of obstetric and perinatal complications, adverse epigenetic consequences, and ethical-legal concerns regarding frozen embryo storage. To minimize risks, the study suggests selecting a limited number of oocytes for insemination and transferring embryos at the cleaving stage. The results showed a comparable live birth rate to the commonly used protocols in older women with reduced ovarian reserve.

Red Wine and Sexual Function in Men: An Original Point of View

Summary: Red wine is rich in nutrients and has been the focus of numerous scientific studies. It has been reported that moderate consumption of red wine, with its phenolic and antioxidant properties, can have positive effects on various diseases. Additionally, the ethanol content in red wine has shown various biological properties. However, little is known about the correlation between red wine consumption and male sexual function. This review aimed to evaluate the effects of moderate red wine consumption on erectile function and found potential benefits in patients with erectile dysfunction as well as positive influences on reproductive function through vasorelaxant and antioxidant mechanisms.

Mechanisms Suggesting a Relationship between Vitamin D and Erectile Dysfunction: An Overview

Summary: Vitamin D deficiency and erectile dysfunction are common in males, especially in the elderly, suggesting a possible relationship between the two conditions. Studies indicate that vitamin D affects erectile function by regulating endothelial function and interfering with comorbidities associated with erectile dysfunction. Low levels of vitamin D may worsen erectile function through various mechanisms. However, the quality and heterogeneity of clinical trials evaluating the effects of vitamin D on erectile function and related comorbidities require further investigation.

The Impact of a Very Short Abstinence Period on Assisted Reproductive Technique Outcomes: A Systematic Review and Meta-Analysis

Summary: This systematic review and meta-analysis found that a very short abstinence period (within 4 h) has a positive effect on assisted reproductive technique (ART) outcomes, significantly increasing the implantation rate, clinical pregnancy rate, and live birth rate. However, it does not have a significant impact on fertilization rate and miscarriage rate. It suggests that a second ejaculation shortly after the first one can be a simple strategy to improve ART outcomes, particularly in couples involving patients with abnormal sperm parameters.

Is It Time for Andrology and Endocrinology Professionals in Assisted Reproduction Centers?

Summary: Epidemiologists emphasize that male factors account for about half of couple infertility cases, yet the importance of andrologists or endocrinologists in ART centers is undervalued. A lack of comprehensive clinical evaluation for male partners, often limited to semen analysis, performed by laboratory technicians, leads to missed opportunities for identifying treatable causes, recognizing diseases requiring immediate treatment, and detecting genetic diseases in offspring. Integrating medical personnel with expertise in andrology and endocrinology in ART centers is the first step towards precision medicine. It can enhance the success rate of ART, reducing psychological and financial burdens on public health resources and infertile couples. Guideline of scientific societies should acknowledge this possibility.
