Impact of Anti-CD20 therapies on the immune homeostasis of gastrointestinal mucosa and their relationship with de novo intestinal bowel disease in multiple sclerosis: a review

Summary: Multiple sclerosis (MS) and inflammatory bowel disease (IBD) are autoimmune disorders that cause inflammation in the brain and gastrointestinal (GI) tract, respectively. While they share common pathogenic mechanisms, their immune responses to treatment differ. Anti-CD20 therapies effectively control inflammation in MS but may cause bowel inflammation in susceptible individuals. This review explores the association between immunity in MS and IBD, the impact of anti-CD20 therapies on the gut microenvironment, and recommendations for early detection and management of GI toxicities in MS patients undergoing B-cell depletion.

Identifying neutrophil-associated subtypes in ulcerative colitis and confirming neutrophils promote colitis-associated colorectal cancer

Summary: This study investigated the pathogenesis of ulcerative colitis (UC) and found that neutrophils may promote the conversion of UC into colitis-associated colorectal cancer (CAC). By analyzing transcriptome data, 65 differentially expressed genes related to UC were identified, as well as immune-related biological pathways. The association between neutrophils and UC was confirmed using CIBERSORT and WGCNA analysis. These findings were validated in a mouse model, and 5 genes were identified as potential biomarkers. The results contribute to a better understanding of the pathogenesis of CAC and provide new insights for its prevention and treatment.

Dysregulation of miR-1-3p: An Early Event in Colitis-Associated Dysplasia

Summary: This study aimed to identify risk factors associated with severe infections in patients with antineutrophil cytoplasm antibody-associated vasculitis (AAV). The use of low-dose TMP/SMX was found to be associated with a reduced risk of severe infections in patients treated with either rituximab (RTX) or cyclophosphamide (CYC)/azathioprine (AZA). Reduced B cell subpopulations at the start of treatment may be a useful indicator of reduced immunocompetence.

Roles of macrophages on ulcerative colitis and colitis-associated colorectal cancer

Summary: Colitis-associated colorectal cancer (CAC) is a severe complication of ulcerative colitis (UC), characterized by multiple lesions, worse pathological type, and poor prognosis. Macrophages, as innate immune cells, play a crucial role in both inflammatory response and tumor immunity. Enhanced macrophage infiltration in UC leads to the production of inflammatory cytokines, promoting UC tumorigenesis. Macrophage polarization into M1 has an anti-tumor effect, while M2 polarization promotes tumor growth. Targeting macrophages has shown effectiveness in preventing and treating CAC.

Rituximab Associated Hypogammaglobulinemia in Autoimmune Disease

Summary: Using global surveys, this study investigated the incidence, predictors, and patterns of flares following COVID-19 vaccination in individuals with autoimmune rheumatic diseases (AIRDs). The results showed that flares occurred in nearly 1 in 10 AIRDs patients after vaccination, with higher vulnerability in patients with comorbidities, mental health disorders, and those receiving the Moderna vaccine. The presence of multiple autoimmune diseases and mental health disorders, as well as receiving the Moderna vaccine, were predictive of flares, while the use of MMF and glucocorticoids were protective. Further research and optimization of vaccine strategies are needed.

Summary: This study investigated the determinants of recovery from dyspnoea in adults with COVID-19 and compared them to non-COVID-19 patients. The results showed that the factors associated with dyspnoea recovery in COVID-19 patients were similar to those in non-COVID-19 patients. These factors included age, gender, obesity status, mental health, cardiovascular disease, and length of hospital admission.

The results showed that DM-type rashes were frequent in ASyS patients, but there were distinct clinical differences between the two groups. Skin involvement in ASyS did not require increased surveillance for malignancy.

Summary: This study investigated the safety of COVID-19 vaccines in pregnant and breastfeeding women with autoimmune diseases. The results showed that adverse events were more frequent in pregnant women compared to non-pregnant patients, but there was no significant difference compared to pregnant healthy controls. There was no difference in adverse events between breastfeeding women and healthy controls. This study provides reassuring insights into the safety of COVID-19 vaccines for women with autoimmune diseases during pregnancy and postpartum periods.

Summary: This study analyzed delayed adverse events (ADEs) related to COVID-19 vaccination in patients with systemic sclerosis (SSc), other systemic autoimmune and inflammatory disorders (SAIDs), and healthy controls using data from a global e-survey. The study found that patients with SSc did not have a higher risk of delayed ADEs compared to other SAIDs and healthy controls. However, the diffuse cutaneous phenotype of SSc and coexisting autoimmune conditions such as myositis and thyroid disease may increase the risk of minor ADEs.

Summary: This study aimed to assess the occurrence, predictors, and outcomes of breakthrough infection (BI) following COVID-19 vaccination in patients with systemic sclerosis (SSc). The study found that SSc patients had a lower risk of BI compared to healthy controls and that advancing age was associated with an increased risk of BI. Ethnicity and use of immunosuppressive drugs did not affect the occurrence of BI. SSc patients were more likely to have asymptomatic COVID-19, while symptomatic patients reported more breathlessness. The study highlighted the importance of understanding the infection situation in SSc patients after vaccination.
