Summary: Early diagnosis of pregnancy- and lactation-associated osteoporosis (PLO) is crucial, and an individualized strategy is needed due to the lack of standardized care guidelines. This review summarizes the clinical characteristics and potential contributing factors of PLO based on recent research. The findings suggest that up to 50% of PLO patients have mutations in specific genes, indicating the potential role of gene testing in the future.

Summary: Pregnancy and lactation-associated osteoporosis (PLO) is a rare and special form of osteoporosis that can lead to vertebral compression fractures and height loss, potentially causing kyphosis of the spine. It is important to consider PLO in the differential diagnosis when a woman experiences severe back pain during or after pregnancy. Further research is needed to better understand the pathophysiology and treatment of PLO.

Summary: A woman's nutritional status during pregnancy and breastfeeding is crucial for both her own health and that of future generations. Different countries have varying recommendations for nutritional supplementation during pregnancy, and the nutritional requirements during pregnancy and lactation may differ due to maternal physiological adaptations.

Summary: The study evaluates mitochondrial dynamics and autophagy in the liver and skeletal muscle of rats in different reproductive stages. It found complex changes in these two tissues especially during lactation, with alterations in autophagy remodeling and mitochondrial fusion coinciding with liver mass size during lactation. The findings highlight the remarkable plasticity required in the liver and skeletal muscle during reproduction to meet metabolic demands.

Summary: The male offspring born and breastfed by mothers treated with dexamethasone during pregnancy displayed reduced adiposity, lower circulating triacylglycerol levels in the fed-state, and decreased expression of specific genes in the jejunum. These findings suggest that reduced fat absorption by the jejunum may contribute to the lower adiposity observed in the adult offspring.

Summary: Pregnant and lactating women lack adequate information on drug safety and efficacy, largely due to the pharmaceutical industry's lack of interest in conducting relevant studies, regulatory constraints, and medical-legal risks. Research in obstetrical and lactation pharmacology lags far behind that of pediatric pharmacology, but the government has made recommendations to address this issue.

Summary: In this study, temporary administration of the bisphosphonate alendronate to female mice during pregnancy or lactation was shown to be protective against bone loss without adverse effects on offspring, suggesting a potential strategy to prevent fragility fractures in mothers.

Summary: Studies on hypoparathyroidism during pregnancy and lactation are limited. In this retrospective study on 19 patients with hypoparathyroidism before pregnancy, different changes in calcium homeostasis and a high prevalence of adverse outcomes during pregnancy were observed. Close monitoring and optimal serum calcium level maintenance are crucial for female patients with hypoparathyroidism.

Summary: The breast undergoes significant changes in structure and function during pregnancy and lactation, affecting its imaging characteristics and potentially reducing the visibility of concurrent pathological processes. The high occurrence of benign gestational-related diseases further complicates the clinical and radiological evaluation of the breast during this period. This pictorial review highlights that, despite being currently underutilized, technical advancements and new clinical evidence support the use of unenhanced breast MRI during pregnancy and both unenhanced and dynamic-contrast enhanced imaging during lactation as effective supplementary modalities in the diagnosis of pregnancy-associated breast cancer (PABC).

Summary: This study aimed to test the diagnostic performance of breast dynamic contrast-enhanced (DCE) MRI during lactation. The results showed that the readers achieved a high diagnostic accuracy in the MIP-DCE images, but with some false-positive interpretations. In addition, the BPE grades were significantly higher in the healthy controls compared to the PABC cohort, while the CNR decreased gradually in the four post-contrast images.

Summary: Pregnancy-associated osteoporosis is a rare condition affecting women in pregnancy or the postpartum period, characterized by skeletal fragility and vertebral fractures. The etiology is poorly understood, and management is challenging due to limited evidence and potential spontaneous improvement in bone density post-pregnancy and lactation.

Summary: Pregnancy involves enhanced intestinal calcium absorption, while lactation involves additional calcium obtained through resorption from the maternal skeleton, leading to reversible bone loss. These maternal adaptations during pregnancy and lactation can affect or complicate the presentation, diagnosis, and management of parathyroid disorders.

Summary: Thyroid dysfunction is more common in women and can have significant impacts on fetal development and pregnancy outcomes. Untreated or poorly treated hyperthyroidism can lead to higher rates of pregnancy loss. Prompt treatment during pregnancy is crucial to ensure the health of the fetus.
