The pre-B-cell receptor associated protein VpreB3 is a useful diagnostic marker for identifying c-MYC translocated lymphomas

Background During B-cell development, precursor B cells transiently express the pre-B-cell receptor composed of mu heavy chain complexed with VpreB and lambda 5 surrogate light chain polypeptides. Recent profiling studies unexpectedly revealed abundant transcripts of one member of the VpreB family, VpreB3, in a subset of mature B cells and Burkitt lymphoma. Design and Methods Here we used a novel antibody to investigate the normal expression pattern of VpreB3 protein in human hematopoietic and lymphoid tissues, and to determine whether VpreB3 could serve as a useful diagnostic biomarker for select B-cell lymphomas. Results We found that VpreB3 protein is normally expressed by precursor B cells in bone marrow and by a subset of normal germinal center B cells in secondary lymphoid organs. Among lymphoid malignancies, we found an association between VpreB3 expression and B-cell tumors with c-MYC abnormalities. VpreB3 was highly expressed in all cases of Burkitt lymphoma, whether of endemic or sporadic origin (44/44 cases, 100%), all cases of B-cell lymphoma, unclassifiable, with features intermediate between diffuse large B-cell lymphoma and Burkitt lymphoma (5/5 cases, 100%), and the majority of diffuse large B-cell lymphomas harboring a c-MYC translocation (15/18 cases, 83%). The expression of VpreB3 in diffuse large B-cell lymphomas without a c-MYC translocation was associated with c-MYC polysomy in 25/75 cases (33%) but only rarely observed in diffuse large B-cell lymphomas lacking a c-MYC abnormality (9/98 cases, 9%). Conclusions We conclude that for B-cell tumors with features suggesting a possible c-MYC translocation, such as intermediate to large cell size and high proliferation rate, the presence of VpreB3 should prompt subsequent confirmatory genetic testing, whereas the absence of VpreB3 is virtually always associated with wild-type c-MYC alleles.

Summary: CD229 is a cell-surface molecule predominantly expressed on lymphocytes. Summary: By sequencing the whole genome of 230 Burkitt lymphoma (BL) and 295 diffuse large B-cell lymphoma (DLBCL) tumors, additional significantly mutated genes were identified, new subtypes of BL and DLBCL were discovered, and genetic features associated with patient outcomes were determined. These findings highlight the shared pathogenesis between adult BL and pediatric BL, and provide a new framework for epidemiology, diagnosis, and therapeutic strategies.

From Castleman disease histopathological features to idiopathic multicentric Castleman disease: a multiparametric approach to exclude potential iMCD histopathological mimickers

Summary: The aim of this study was to apply international consensus diagnostic criteria to classify 42 cases of Castleman disease (CD), and identify differences among unicentric CD, idiopathic multicentric Castleman disease (iMCD), HHV-8+ multicentric CD (HHV-8+MCD), and POEMS/plasma cell neoplasia (PCN)-associated CD. The results demonstrated that FDC proliferation was prominent in unicentric CD, while hypervascularity was increased in HHV-8+MCD, and germinal center hyperplasia was found only in iMCD cases and systemic lupus erythematosus (SLE). Monotypic plasma cells were readily identifiable in lymph node biopsies of PCN/POEMS-associated CD.

Summary: Follicular lymphoma (FL) accounts for approximately 20% of new lymphoma cases. In this study, two genetically distinct subgroups of FL, called DLBCL-like (dFL) and constrained FL (cFL), were identified and characterized based on whole-genome sequencing data. Using a machine learning approach, patients with FL could be stratified into cFL and dFL subgroups, and it was found that cFL status is associated with a reduced risk of lymphoma transformation.

Summary: Diffuse large B-cell lymphoma (DLBCL) patients with early disease progression or relapse have poor outcomes. This study found that patients with late relapse had better outcomes after second-line therapy and autologous stem-cell transplantation compared to those with primary refractory or early relapse. Despite concordance in cell-of-origin classification and genetics-based subgroup between diagnostic and relapse biopsies, late relapses showed significant genetic differences and demonstrated branching evolution patterns. In highly divergent tumors, the same genes acquired new mutations independently, suggesting that shared precursor cells constrain tumor evolution towards the same genetics-based subgroups at both diagnosis and relapse.

Summary: This study demonstrates that combinations of oncogenes in cellular subpopulations within a cancer can influence clinical outcomes. The percentage of cells with a unique combination of MYC, BCL2, and BCL6 predicts survival in diffuse large B-cell lymphoma (DLBCL), and can be derived mathematically from quantitative measurements of the individual oncogenes. Comparative analysis of DLBCL samples and transformed B cells identifies potential regulators of the unfavorable biology associated with this combination. These findings have implications for understanding cancer evolution and therapy resistance.

Invasive Cutaneous Candidiasis, Autoimmune Hemolytic Anemia and Pancytopenia: A Challenging Scenario for Waldenstrom Macroglobulinemia in an Elderly Patient

Summary: This article reports a case of a 77-year-old woman diagnosed with Waldenstrom macroglobulinemia (WM) following severe and sudden pancytopenia associated with cold agglutinin syndrome. Treatment with rituximab, corticosteroids, and cyclophosphamide was initiated to manage WM and underlying hemolysis. Despite improvements in hemolysis parameters, pancytopenia persisted and ibrutinib was started as a second-line treatment. The patient developed an uncommon invasive fungal infection (IFI) with bone marrow granulomatosis and myelofibrosis during treatment, highlighting a poor hematopoietic response and numerous complications.

Summary: This article presents a rare case of a 25-year-old patient diagnosed with human immunodeficiency virus (HIV) and the development of primary effusion lymphoma (PEL), Kaposi's sarcoma (KS), and multicentric Castleman's disease (MCD). Despite intensive treatment, the patient's condition did not improve. This case highlights the need for new therapies and research in this field.
