Changes in cytokine profile pre- and post-immunosuppression in acquired aplastic anemia

Cytokine expression assessed by flow cytometry in 53 acquired aplastic anemia patients before and after combined immunosuppression (EBMT WPSAA protocols) showed that CD3(+) marrow cells containing TNF-alpha, IFN-gamma and IL4 were similar in subjects with disease at onset (DO) and responsive to treatment who had more CD3(+)/TNF-alpha(+) and CD3(+)/IFN-gamma(+) cells than normal controls. In vitro block of TNF-alpha and/or IFN-gamma significantly increased BFU-e over baseline in 28 patients. In responsive to treatment patients only TNF-alpha block significantly incremented colonies over normal controls. Absolute marrow CD3(+)/TNF-alpha(+) and CD3(+)/IFN-gamma(+) cells prospectively tested in a group of 21 subjects declined significantly more in Responders than in Non Responders to immunosuppression at Response Evaluation Time respect to Diagnosis. Both in Responders and in Non Responders these cells remained higher than in normal controls. This study suggests that immunosuppression does not fully clear excess TNF-alpha and IFN-gamma from marrow of patients with good outcome and raises the hypothesis that additional cytokine blockade might be useful in immunosuppression for acquired aplastic anemia.

Progressive disease, advanced stage, and ≥5 involved sites were associated with significantly poorer outcomes.

Summary: This study found that survivors of Hodgkin lymphoma (HL) are at an increased risk of developing non-Hodgkin lymphoma (NHL) after treatment. The researchers also identified other types of childhood cancer survivors who are at risk of developing NHL, including survivors of Wilms tumor, leukemia, and bone sarcoma. Treatment with chemotherapy increased the risk of NHL, while treatment with radiotherapy did not.

Changes in the Use of Antibiotics for Methicillin-Resistant Staphylococcus aureus Bloodstream Infections in Children: A 5-Year Retrospective, Single Center Study

Summary: Monitoring antibiotic use in pediatric population is challenging, especially in determining the relationship between specific pathogens, infections, and antibiotic use. A retrospective analysis of the consumption of anti-MRSA drugs from 2017 to 2021 revealed an increase in the use of daptomycin and ceftaroline, accompanied by a decrease in vancomycin use. Additionally, an increase in MRSA bloodstream infections with a vancomycin MIC of 1, representing 100% of cases in 2021, was observed, along with difficulties in achieving effective vancomycin plasma concentrations in children.

[18F]FDG PET-MR in the Evaluation and Follow-Up of Incidental Bone Ischemic Lesions in a Mono-Center Cohort of Pediatric Patients Affected by Hodgkin's Lymphoma

Summary: Hodgkin's lymphoma (HL) has a good prognosis in children, adolescents, and young adults, but the development of adverse effects such as Bone Ischemic Lesions (BILs) is a concern. This study evaluated the role of [F-18]FDG-PET-MR in detecting BILs in pediatric HL patients. BILs were detected through PET-MR imaging as medullary lesions with a tortuous border. Among 53 HL patients, 18.9% developed BILs, with mostly multifocal lesions. PET-MR is a sensitive technique for detecting asymptomatic BILs, which can be monitored through clinical follow-up.

Summary: Radiation and exposure to alkylating agents increase the risk of bone cancer among survivors of childhood cancer. This study demonstrates, for the first time, that the risk of bone cancer is increased 5-10 fold after exposure of bone tissue to cumulative radiation doses of 1-9 Gy. Alkylating agents exceeding 10,000 mg/m(2), particularly procarbazine, ifosfamide, and cyclophosphamide, increase the risk 7-8 fold. These findings should be used to develop/update clinical follow-up guidelines and survivorship care plans.

Summary: The study conducted at IRCCS Istituto Giannina Gaslini in Genoa, Italy over a period of 32 years showed that allogeneic hematopoietic stem cell transplantation (allo-HSCT) is an effective treatment for inborn errors of immunity (IEI) in children. The overall survival rate at 5 years was 83.4% and recent years have seen significant improvements in outcomes and a reduction in severe acute graft-versus-host disease (aGvHD) incidence.
