Epidemiology of aplastic anemia: a prospective multicenter study

Background Aplastic anemia is a rare and severe disease. Its incidence varies considerably worldwide. We aimed at describing the epidemiology of this disease, including the incidence, mortality and survival trends, in a well-defined population. Design and Methods Since 1980, a case-control surveillance study of aplastic anemia has been carried out by a cooperative group, in the metropolitan area of Barcelona. Inclusion is dependent on the patient having at least two of the following features: white blood cell count <= 3.5x10(9)/L, platelet count <= 50x10(9)/L, hemoglobin <10 g/L or hematocrit of <30%; when only one of these last two criteria is fulfilled, a reticulocyte count of <= 30x10(9)/L is also required. The bone marrow biopsy has to be compatible with the diagnosis of aplastic anemia. Results Between 1980 and 2003, a total of 235 cases of aplastic anemia were identified. The overall incidence was 2.34 per million inhabitants per year and the incidence increased with age. Most of the cases were classified as severe or very severe aplastic anemia. Survival rates at 3 months, and at 2 and 15 years after the diagnosis were 73%, 57%, and 51%, respectively. Advanced age and more severe disese at the time of diagnosis were associated with a lower survival rate.There was a trend to a better 2-year survival rate among patients treated with bone marrow transplantation. Forty-nine cases (20.8%) were exposed to drugs reported to be associated with aplastic anemia, and 21 (8.9%) to toxic agents. Conclusions The incidence of aplastic anemia in Barcelona is low but the case fatality rate is high. Advanced age and severe disease at the time of diagnosis were associated with decreased survival.

The results showed a significant decrease in the proportion of patients with two or more STOPP criteria and START criteria at the end of the intervention and at 6 months. Patients who experienced a decrease in STOPP criteria also showed a reduction in polypharmacy and falls at 6 months.

Summary: This study investigated the effect of individual genetic variations on the risk of upper gastrointestinal haemorrhage (UGIH) induced by non-steroidal anti-inflammatory drugs (NSAIDs). The study found that certain specific gene polymorphisms interacted with NSAIDs, increasing the risk of UGIH. Additionally, the study identified larger risk increases associated with specific gene polymorphisms. These findings suggest that individual genetic characteristics can impact the efficacy and side effects of medications.

Humoral and cellular response in convalescent COVID-19 lupus patients

Summary: In patients with systemic lupus erythematosus (SLE), the immune dysregulation and immunosuppression may increase susceptibility to and severity of COVID-19. This study aimed to characterize COVID-19 infection in SLE patients, identify risk factors for susceptibility and severity, assess the presence of SARS-CoV-2 IgG antibodies, and analyze the cellular response. The findings showed that SLE patients were at higher risk of COVID-19 infection, and new risk factors for susceptibility and severity were identified. Despite immunosuppressive therapy, SLE patients were able to mount humoral and cellular responses to COVID-19.

Characteristics of drug safety alerts issued by the Spanish Medicines Agency

Summary: This study aimed to evaluate the incidence and severity of lymphopenia following siponimod treatment. Medical Students' Opinion of Their Learning Process

Summary: The aim of this study was to analyze the opinion of medical students about their learning process and the influence of their experience according to their year of medical degree. The study found that student motivation and teaching quality were generally high, and student-teacher interaction and new learning technological tools were important for student motivation. Late-year students showed a higher interest in becoming part of a medical team and in learning to write scientific articles. The study concluded that the year of career had little influence on the medical students' opinion on their learning process, and the use of new technologies and student-teacher interaction is essential for student motivation.
