Case report: An occult hepatitis B virus infection reactivation in an HIV/HCV coinfected patient during an immune reconstitution inflammatory syndrome

Summary: We report a case of occult hepatitis B virus infection (OBI) reactivation in a HIV/HCV co-infected patient who was followed for 25 years but lost to follow-up after Direct-acting antivirals (DAAs) treatment. The patient showed severe immunosuppression and high replication of plasmatic HIV-RNA, followed by an immune reconstitution inflammatory syndrome (IRIS) with detectable HBV-DNA load and transaminase elevation after ART reintroduction. This case highlights the importance of closely monitoring HBV serological and virological markers in individuals with impaired immune systems, especially in patients on tenofovir or lamivudine-sparing regimens.

Summary: This study investigated the risk of hepatitis B virus (HBV) reactivation in rheumatoid arthritis (RA) patients who are seropositive for hepatitis B core antibody (HBcAb) and negative for hepatitis B surface antigen (HBsAg) during biologic or targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs) therapy. The results showed that RA patients with negative baseline anti-HBs and those receiving certain b/tsDMARDs, such as rituximab, abatacept, and adalimumab, have a high risk of HBV reactivation.

Summary: This systematic review and meta-analysis found that non-liver solid organ transplant recipients with resolved HBV infection have a high incidence and clinical risk of HBV reactivation, particularly in anti-HBs-negative recipients, those with acute rejection, ABO blood type-incompatible transplantation, and the use of specific medications. Therefore, monitoring and routine antiviral prophylaxis should be considered for non-liver SOT recipients at high risk of HBV reactivation.

Summary: The study compared the outcomes of haploidentical allogeneic hematopoietic-cell transplantation and matched unrelated donor transplantation for AML patients in different risk groups and remission statuses. The results showed that haploidentical transplantation increased nonrelapse mortality and decreased relapse incidence in high-risk AML in CR, leading to similar leukemia-free survival, overall survival, and graft-versus-host disease-free, relapse-free survival compared to matched unrelated donor transplantation. This suggests the potential benefits of using haploidentical family donors for high-risk AML patients without matched sibling donors.

Summary: In the setting of PTCy for patients with acute leukemia, haplo bone marrow transplantation resulted in a lower incidence of GvHD compared with UD mobilized peripheral blood stem cell transplantation. However, differences in GvHD did not affect overall survival outcomes, suggesting both donor sources are equally acceptable for allo-HCT.
