Unraveling the two entities of endometrioid ovarian cancer: A single center clinical experience

Objective. Due to the increasing prevalence of the benign condition, ovarian carcinoma arising from endometriosis is emerging as a relevant clinical entity with an unclear biological signature. We have investigated clinical and histologic features of endometriosis-associated endometrioid ovarian cancer using an institutional retrospective database. Methods. Patients diagnosed with endometrioid ovarian cancer at our institution were divided into two groups according to the fulfillment or not of Sampson's and Scott's criteria for the detection of endometriosis-associated ovarian cancer. Clinical and histological data were reported and compared. Survival analysis was obtained using the log-rank test in an unadjusted Kaplan-Meier method. Multivariate analysis was performed using the Cox proportional hazards regression model to establish independent factors associated with endometriosis-associated endometrioid ovarian cancer and to identify predictors of survival. The degree of concordance was evaluated by Cohen's Kappa measures. Results. Patients with endometriosis-associated endometrioid ovarian cancer were significantly younger, had a lower disease stage (62% vs 23%; p = 0.003), a less prevalent high grade tumor (38% vs 82%; p = 0.002) and a higher prevalence of squamous and mucinous metaplasia. The rate of endometrial cancer diagnosis was significantly higher in women with endometriosis-associated endometrioid ovarian cancer (33%) than in other patients (11%) (p = 0.04) with a 92% concordance between ovarian and endometrial histologic tumor grade. A significant difference in survival rate could not be demonstrated between patients with or without endometriosis. Conclusions. The analysis of a retrospective endometrioid ovarian cancer database may allow to suggest a 40 molecular, morphological and clinical parallelism between endometrial and endometrioid ovarian cancers. Conventional IVF performs similarly in women with and without endometriosis

Summary: This study aimed to compare the factors underlying infertility in women with endometriosis. The results showed that there were no significant differences in fertilization rate, embryo quality, and clinical pregnancy rate between women with and without endometriosis. Therefore, conventional IVF can still be used in endometriosis patients unless there is a male factor involved.

A Case-Control Study on the Effects of Plasticizers Exposure on Male Fertility

Summary: Male infertility is a significant public health concern, and the potential link between exposure to plasticizers like phthalates and bisphenol A and this condition is a topic of debate. A study was conducted with 155 infertility cases and 211 controls to investigate this issue. The results showed higher urinary concentrations of phthalates in cases compared to controls, but no significant difference in BPA levels. However, no clear quantitative correlation between specific plasticizers and sperm parameters was identified, calling for further research on the risks associated with exposure to these chemicals.

Association of endometriosis and adenomyosis with pregnancy and infertility

Summary: This review synthesized systematic reviews and meta-analyses published in the last 5 years to explore the association between endometriosis and adenomyosis with reproductive and obstetric outcomes. The findings suggest that women with endometriosis and adenomyosis have reduced pregnancy and live birth rates, increased miscarriage rates, and higher risks of various adverse obstetric outcomes. Women with deep infiltrating endometriosis and severe adenomyosis should be considered at high obstetric risk and referred to tertiary care centers for safe management during pregnancy and delivery. It is uncertain whether the same applies to pregnant women with minimal endometriosis and adenomyosis forms. Emerging evidence suggests that some adverse outcomes observed in women with endometriosis are associated with coexisting adenomyosis.

ART and the forgotten siblings: a call for research

Endometriosis increases the risk of gestational diabetes: a meta-analysis stratified by mode of conception, disease localization and severity

Is There a Correlation between the Second-to-Four Digit Ratio (2D:4D) and Endometriosis? Results of a Case-Control Study

The kisspeptin system in and beyond reproduction: exploring intricate pathways and potential links between endometriosis and polycystic ovary syndrome

Recent discovery of kisspeptin as a pivotal mediator in human reproduction and its involvement in metabolism, inflammation, and pain sensitivity provides new insights into understanding these conditions. This review discusses the clinical relevance and potential translational applications of these discoveries in developing novel therapies for endometriosis and PCOS.

Summary: According to epidemiological data, the incidence of endometriosis increases rapidly around the age of 25. The delay in diagnosis is usually between 5 and 8 years in adult women and over 10 years in adolescents. Ovulation and menstruation, which are inflammatory events, may contribute to the early development of endometriosis and adenomyosis. Therefore, secondary prevention targeting severely symptomatic adolescents is crucial for reducing suffering, limiting lesion progression, and preserving future reproductive potential.
