Summary: This study investigated the incidence of reactive thrombocytosis and its impact on survival in patients with advanced epithelial ovarian cancer (EOC) after maximal cytoreductive surgery. The results showed that thrombocytosis was observed in 34.8% of patients one week after surgery, and it had the most significant effects on overall survival when observed during adjuvant chemotherapy.

Summary: This study analyzed the association between 12 lifestyle and personal exposures and having residual disease after surgery in patients with high-grade serous ovarian cancer (HGSC). The use of menopausal estrogen-only therapy (ET) was associated with a lower likelihood of having macroscopic residual disease, while parous women who did not breastfeed also had a lower likelihood of residual disease. These factors could potentially be included in risk stratification models for HGSC patients.

Summary: This study aimed to determine whether the use of the PlasmaJet Surgical device during surgery has an effect on the quality-of-life of patients with advanced ovarian cancer. The results showed that using the PlasmaJet device during surgery leads to a higher quality-of-life compared to surgery with electrocoagulation alone. The differences in quality-of-life mainly relate to physical and role functioning, fatigue, and pain.

Summary: With the increasing use of neoadjuvant chemotherapy, it is apparent that some patients may benefit from less extensive interval cytoreductive surgery performed using minimally invasive techniques. Surgeons in various countries, including the United States, have started adopting these techniques based on their expertise in minimally invasive surgery for other indications in gynecologic oncology. Further research will determine the most suitable patients for this approach.

Summary: This study evaluated the clinical characteristics of 262 patients with recurrent ovarian cancer to identify factors associated with the selection of patients who would benefit from secondary cytoreductive surgery (SCS). The study found that patients who received SCS had significantly longer median survival compared to those who received chemotherapy alone. Limited regional recurrence emerged as a simplified factor for predicting no residual disease after SCS.

Summary: Ovarian cancer is the second most common and fatal gynecological malignancy worldwide, with most patients presenting in advanced stages. Cytoreductive surgery in combination with platinum-based chemotherapy is the standard treatment. Despite advancements, recurrence and death from the disease are still common. The role of secondary cytoreductive surgery in recurrent ovarian cancer is controversial and should be continuously reviewed.

Summary: The study found that patients undergoing primary debulking surgery had a higher rate of complete cytoreduction but also a higher rate of complications requiring re-intervention, resulting in a longer time to start adjuvant chemotherapy. It is important to balance the aggressiveness of surgery with the outcomes of severe complications to improve the quality of surgical outcomes.

Summary: This study aimed to evaluate the indications and management of grade III-IV postoperative complications in patients requiring vacuum-assisted open abdomen after debulking surgery for ovarian carcinomatosis. The results showed that 2% of patients required further management after laparotomy, and the indications included gastrointestinal perforation, necrotic enterocolitis, intestinal ischemia, anastomotic leakages, and intra-abdominal hemorrhages. The use of vacuum-assisted wound closure proved to be effective in reducing the risk of postoperative mortality.

Summary: This study aimed to investigate the quality of life (QoL) after surgical resection for advanced ovarian cancer and explore the association with surgical complexity and disease burden. The results showed that increasing surgical complexity did not lead to a decline in QoL. For patients with advanced ovarian cancer, higher surgical complexity was associated with greater improvement in postoperative QoL.

Summary: Cytoreductive surgery for advanced ovarian cancer commonly involves bowel resection. However, the degree to which UK gynaecological oncologists independently perform bowel surgery varies nationally. A survey of British Gynaecological Cancer Society members found that colorectal/general surgical teams are more commonly involved in large bowel procedures and left colon procedures.

Summary: This study demonstrates the use of low-coverage cell-free tumor DNA sequencing to detect chromosomal instability (CI) in ovarian cancer patients. The CI can serve as a reliable marker for ovarian cancer detection and is associated with prognostic and predictive clinical measures. The findings suggest that ctDNA analysis based on the CNI-Score can be an important tool for diagnostics and therapy monitoring in high-grade epithelial ovarian cancer.

Summary: In the management of advanced vulval cancer, there is a dilemma between primary closure and reconstructive surgery. This study compared the preoperative parameters and postoperative results between these two approaches. The findings suggest that reconstructive surgery may lead to better treatment outcomes by improving complete resection rates, anatomical restoration, and wound healing, consequently reducing recurrence rates.

Summary: Testing for HRD is crucial for recommending appropriate therapy in primary high-grade ovarian cancer patients. While patients with a BRCA mutation are defined as HRD deficient, approximately 20% of patients without BRCA mutation can still have HRD deficiency based on tumor genomic analysis. This study highlights the importance of obtaining sufficient tumor tissue and conducting additional BRCA germline testing to improve the accuracy of HRD testing and inform treatment decisions.
