Influence of Obesity on the Severity and Clinical Outcome of Acute Pancreatitis

Background/Aims: Obesity tends to be associated with increased mortality and morbidity in acute pancreatitis. However, in Asian populations, higher morbidity and mortality have been reported in patients with low body mass indexes (BMIs). This study was undertaken to evaluate the relation between obesity and outcome, and to investigate the occurrence of complications by overweightedness in acute pancreatitis. Methods: The medical records of 403 patients with acute pancreatitis were reviewed retrospectively, and Ranson's scores, modified Glasgow scores, Acute Physiology and Chronic Health Evaluation (APACHE) II scores and computed tomography severity indexes were calculated. Patients were categorized by BMI for the analysis. Results: When compared with normal patients (BMI 18.5 to 22.9), all categories with a BMI >= 23 had an increased risk of developing a severe form of acute pancreatitis (p=0.003) and all categories with a BMI >= 25 significantly predicted severity (p<0.001). Patients with class 1 obesity (BMI 25 to 29.9) developed significantly more systemic and metabolic complications. Conclusions: Overweightedness and obesity were found to be associated with a higher risk of developing severe pancreatitis. Further studies are needed to establish the precise prognostic value of obesity in members of the population with low BMIs. (Gut Liver 2011;5:335-339)

The results show a correlation between the proposed method and the radiographic assessment conducted by physicians.

Gastrointestinal bleeding risk of non-vitamin K antagonist oral anticoagulants versus warfarin in general and after polypectomy: a population-based study with propensity score matching analysis

Summary: This study compared the risk of gastrointestinal bleeding (GIB) for non-vitamin K antagonist oral anticoagulants (NOACs) and warfarin. The results showed that NOACs patients had a lower risk of overall GIB than warfarin patients, but a higher risk of post-polypectomy GIB. Among NOACs, apixaban had the lowest risk of GIB.

A Case of Hepatic Immunoglobulin G4-Related Disease Presenting as an Inflammatory Pseudotumor and Sclerosing Cholangitis

Summary: An inflammatory pseudotumor is a benign disease characterized by tumor-like lesions consisting of inflammatory cells including plasma cells and fibrous tissue. Some cases of inflammatory pseudotumor have been identified as Immunoglobulin G4-related disease (IgG4-RD), which is a fibroinflammatory condition characterized by lymphoplasmacytic infiltration, predominance of IgG4-positive plasma cells, storiform fibrosis, and elevated serum IgG4 concentrations. We report a case of IgG4-RD in the form of an inflammatory pseudotumor in the liver with combined sclerosing cholangitis. Adequate tissue samples and clinical follow-up are recommended for accurate diagnosis of IgG4-RD.

Summary: This study evaluated the clinical significance of hsa_circ_0003570 expression in hepatocellular carcinoma (HCC) and found that its expression was associated with tumor size, vessel invasion, tumor-node metastasis stage, Barcelona Clinic Liver Cancer stage, and AFP level. In addition, high hsa_circ_0003570 expression was correlated with better overall survival and progression-free survival in patients with HCC.

Comparison Study between ERCP and PTBD for Recurrent Choledocholithiasis in Patients Following Gastrectomy

Summary: This study aimed to evaluate the recurrence rate of choledocholithiasis and identify risk factors for recurrent choledocholithiasis in patients following gastrectomy. The study found that the recurrence rate of choledocholithiasis was 14.5%, and the stone removal modality, no use of balloon sphincteroplasty, and the presence of periampullary diverticulum were significant risk factors for recurrent choledocholithiasis. Therefore, the study recommends using ERCP with active use of balloon sphincteroplasty to decrease recurrent CBD stones following gastrectomy.

Relationship between gallstones and interstitial cells of Cajal in the gallbladder

Summary: This study investigated the relationship among ICC number, gallbladder contractibility, and gallstones in patients undergoing cholecystectomy. Results showed that male participants had a higher number of ICC in the mucosal layer compared to females, and larger gallbladder size was associated with decreased ICC in the muscular layer. The number of ICC in the muscular layer decreased with an increase in gallbladder stone number, but there was no significant correlation with other factors.
