Cigarette smoking and liver diseases

David Marti-Aguado, Ana Clemente-Sanchez, Ramon Bataller

Summary: Smoking is a preventable risk factor for premature morbidity and mortality, with clinical evidence showing negative impacts on fatty liver, fibrosis, hepatocellular carcinoma, and liver transplant outcomes. Promoting smoking cessation is crucial for improving treatment response and reducing the risk of liver diseases.

Seitz, Michael Soyka, Felix Stickel, Andrew Thompson, Mark R. Thursz, Eric Trepo, Timothy R. Morgan, Devanshi Seth

Summary: This study developed a genetic risk score that can identify individuals at high risk of developing cirrhosis. It was found that the risk of cirrhosis is increased over 10-fold in patients with diabetes and a high genetic risk score. This risk assessment could lead to early and personalized management of the disease in high-risk patients.

Management of alcohol use disorder in patients with cirrhosis in the setting of liver transplantation

Juan Pablo Arab, Manhal Izzy, Lorenzo Leggio, Ramon Bataller, Vijay H. Shah

Summary: This review discusses the management of alcohol use disorder in patients with alcohol-associated liver disease, particularly in the setting of liver transplantation. It emphasizes the importance of a multidisciplinary approach, including diagnosis, predictors of relapse, behavioral and pharmacological therapies. The review also proposes a comprehensive definition of relapse using contemporary biomarkers to guide future clinical trials for optimizing treatment, transplant selection, and post-transplant care of patients with AUD and cirrhosis.

Subclinical versus advanced forms of alcohol-related liver disease: Need for early detection

Concepcion Gomez-Medina, Luma Melo, David Marti-Aguado, Ramon Bataller

Summary: Alcohol-related liver disease (ALD) encompasses a wide range of clinical presentations, from asymptomatic patients to severe complications like cirrhosis and cancer. Late-stage identification of ALD limits effective treatment options, highlighting the need for early detection, which is lacking in many countries. Alcohol underreporting by patients hampers the identification of alcohol misuse, but there are biomarkers and non-invasive techniques available to detect recent alcohol use and assess fibrosis severity. This article provides a comprehensive overview of the differences between early stages of ALD and alcohol-induced hepatitis (AH) and suggests campaigns targeting early identification of subclinical forms to prevent life-threatening complications. A Quantitative Systems Pharmacology Platform Reveals NAFLD Pathophysiological States and Targeting Strategies

Daniel E. Lefever, Mark T. Miedel, Fen Pei, Johanna K. DiStefano, Richard Debiasio, Tong Ying Shun, Manush Saydmohammed, Maria Chikina, Lawrence A. Vernetti, Alejandro Soto-Gutierrez, Satdarshan P. Monga, Ramon Bataller, Jaideep Behari, Vijay K. Yechoor, Ivet Bahar, Albert Gough, Andrew M. Stern, D. Lansing Taylor

Summary: This study describes an integrated quantitative systems pharmacology (QSP) platform that comprehensively defines disease states that promote non-alcoholic fatty liver disease (NAFLD) progression and predicts drugs to normalize these states. Experimental validation demonstrated several drugs and drug combinations that can mitigate NAFLD pathology. Additionally, the study identified the involvement of the pregnane X receptor in the transcriptional dysregulation of NAFLD.

Healthcare burden and outcomes of hepatorenal syndrome among cirrhosis-related hospitalisations in the US

Ashwani K. Singal, Yong-Fang Kuo, K. Rajender Reddy, Ramon Bataller, Paul Kwo

Summary: The study found that in 2019, there were 27,180 hospitalisations caused by Hepatorenal syndrome (HRS), requiring an estimated $4.2 billion USD in healthcare expenditures. Despite a decrease in in-hospital mortality over time, HRS remains a significant cause of death in patients hospitalized with cirrhosis.

Summary: Liver inflammation inhibits side chain shortening and conjugation of bile acids, but other mechanisms involved in bile acid homeostasis counterbalance the effects on serum bile acid profile.

Genome-Scale Metabolic Modeling Reveals Sequential Dysregulation of Glutathione Metabolism in Livers from Patients with Alcoholic Hepatitis

Alexandra Manchel, Radhakrishnan Mahadevan, Ramon Bataller, Jan B. Hoek, Rajanikanth Vadigepalli

Summary: Metabolic pathways in the liver of patients with alcoholic hepatitis vary in activity, with increasing disease severity resulting in sequential dysregulation of solute transport mechanisms underlying the glutathione metabolic pathway. Targeting solute transporters in the glutathione pathway to mimic healthy liver state flux activity may be a potential therapeutic intervention for alcoholic hepatitis.

Bariatric Surgery Is Associated with Alcohol-Related Liver Disease and Psychiatric Disorders Associated with AUD

Edilmar Alvarado-Tapias, David Marti-Aguado, Kevin Kennedy, Carlos Fernandez-Carrillo, Meritxell Ventura-Cots, Dalia Morales-Arraez, Stephen R. Atkinson, Ana Clemente-Sanchez, Josepmaria Argemi, Ramon Bataller

Summary: This study evaluated the association between bariatric surgery and alcohol use disorder (AUD), alcohol-related liver disease (ALD), and psychiatric disorders associated with AUD. The results showed that bariatric surgery is associated with an increased prevalence of AUD, ALD, and psychiatric disorders associated with AUD, independent of vitamin D deficiency.

New insights into the molecular basis of alcohol abstinence and relapse in alcohol-associated liver disease

Luis Antonio Diaz, Gerald Scott Winder, Lorenzo Leggio, Jasmohan S. Bajaj, Ramon Bataller, Juan Pablo Arab

Summary: Alcohol use disorder is a significant public health concern affecting approximately 5% of adults worldwide. Recent research has identified novel pathways of damage, including changes in gut microbiota composition and function, disruption of the gut-liver-brain axis, and activation of the immune system. Therapeutic targets for AUD may include interventions to restore gut microbiota balance, regulate neuroinflammatory pathways, and target neuroendocrine pathways. Psychological and behavioral treatments are also essential for comprehensive care. A personalized approach considering these novel targets has the potential to significantly reduce the burden of alcohol-related diseases.
