Frequent intravenous pulses of growth hormone together with alanylglutamine supplementation in prolonged critical illness after multiple trauma: Effects on glucose control, plasma IGF-I and glutamine

Objective: We aim to demonstrate that low dose growth hormone (GH) administered in i.v. pulses every 3 It is able to normalize IGF-I levels in subjects with prolonged critical illness, after multiple trauma. We also ask whether it is possible to control glycaemia during such a treatment and how alanylglutamine (AG) supplementation influences plasma glutamine concentration. Methods: We used a prospective double-blind (group I vs. 2), randomized trial with an open-label control arm (group 3). Thirty multiple trauma patients (median age: 36, 42, 46 years) were randomized on day 4 after trauma to receive (group 1, n = 10) i.v. AG supplementation (0.3 g/kg day from day 4 till 17) and i.v. GH (0.05 mg/kg day divided into 8 boluses, maximum dose at 3 AM, administered on days 7-17) or AG and placebo (group 2, n = 10). Group 3 (n = 10) received isocaloric isonitrogenous (proteins 1.5 g/kg day) nutrition without AG. Glycaemia was controlled by i.v. insulin infusion according to a routine protocol. Results: GH treatment caused an increase of IGF-I (from median 169 on day 4 to 493 ng/ml on day 17), IGFBP-3 (from 2.4 to 3.2 mu g/ml) and a fall in IGFBP-1 (from 11.5 to 3.1 mu g/ml), whilst in both groups 2 and 3 these indices remained unchanged. At the end of the study (day 17) IGF-I and IGFBP-1 differed significantly among groups (p = 0.008 resp. p = 0.010, Kruskal-Wallis). Plasma glutamine remained below the normal range through the study in all groups (median: 0.18-0.30 mM), but had a tendency to rise in group 2 in contrast with a fall in groups I and 3 (NS). Group I required more insulin (p < 0.01) than did the control group but median glycaemia was only 0.4-0.5 mM higher in group 1 (6.5 mM) than in groups 2 and 3 (6.1 resp. 6.0 mM). Conclusions: GH (0.05 g/kg day) administered in i.v. pulses is able to normalize IGF-I levels in subjects with prolonged critical illness after trauma. During this treatment, the standard dose of AG prevents worsening of plasma glutamine deficiency and glucose control is possible using routine algorithms, but it requires higher insulin doses. Impact of admitting department on the management of acute coronary syndrome after an out of hospital cardiac arrest

Summary: This study analyzed the influence of the hospital admitting department on adherence to the European Society of Cardiology guidelines for management of acute coronary syndromes (ACS) in patients after out-of-hospital cardiac arrest (OHCA) of coronary etiology. The results showed that the choice of admission department significantly affected the extent to which recommended treatments were used, and examination by a cardiologist upon admission increased the likelihood of early diagnosis of ACS as the cause of OHCA.

Progress in on-line, at-line, and in-line coupling of sample treatment with capillary and microchip electrophoresis over the past 10 years: A review

Summary: This review evaluates the progress made in the past decade in on-line, at-line, and in-line sample treatment coupled with capillary and microchip electrophoresis. Previous antithrombotic therapy does not have an impact on the in-hospital mortality of patients with upper gastrointestinal bleeding

Summary: This study aimed to assess the impact of antithrombotic therapy on in-hospital and 6-month outcomes and determine the re-initiation rate of antithrombotics after a bleeding event. The results showed no association between antithrombotic treatment and in-hospital outcomes, but the development of hemorrhagic shock predicted poor prognosis. Furthermore, higher 6-month mortality was observed in older patients, patients with more comorbidities, with liver cirrhosis and cancer.

A randomized comparison of His bundle pacing versus right ventricular pacing: Effect on left ventricular function and biomarkers of collagen metabolism

Summary: This study compared the effects of His bundle pacing (HBP) and right ventricular pacing (RVP) on left ventricular ejection fraction (LVEF) and studied their impact on serum markers of collagen metabolism. The results showed that in high-risk pacing-induced cardiomyopathy (PICM) patients, HBP was superior to RVP in providing better ventricular function. In RVP patients, those with higher baseline levels of Gal-3 and ST2-IL experienced a greater decline in LVEF.

Plasma levels of creatine, 2-aminobutyric acid, acetyl-carnitine and amino acids during fasting measured by counter-current electrophoresis in PAMAPTAC capillary

Summary: In this study, capillary electrophoresis was used to determine minor nitrogen metabolites, including creatine, 2-aminobutyric acid (2-AB), and acetyl-carnitine (Ac-carn), in human plasma. Baseline separation of these metabolites was achieved using a 6% PAMAPTAC coated fused silica capillary and a counter-current mode. Plasma levels of these metabolites were monitored during a 60-hour fasting period, and significant differences were observed compared to baseline levels.

Immune cell infiltration, tumour budding, and the p53 expression pattern are important predictors in penile squamous cell carcinoma: a retrospective study of 152 cases

Summary: Penile squamous cell carcinoma (pSCC) is a rare malignancy with a slowly increasing incidence and variable prognosis. Regional lymph node involvement signifies poor prognosis but represents a late sign, and more prognostic markers for effective patient risk stratification are urgently needed. In this study, traditional pathological variables, tumour budding, p53, p16, and mismatch repair proteins (MMR) immunohistochemistry were analyzed in 152 tumor samples. The results showed that the density of tumour lymphocytic infiltrate and the presence of tumor budding were significant predictors of overall survival and cancer-specific survival.

A new electromembrane extraction probe for on-line connection with capillary electrophoresis for determination of substances in biological matrices

Summary: A miniature probe for electromembrane extraction was developed. It was made of polypropylene hollow fiber with a supported liquid membrane. The probe parameters were tested for extraction of creatinine and basic amino acids with promising results in increased sensitivity. The probe is simple to prepare and can be used for all-day measurements.

Mitragyna speciosa (Kratom) poisoning: Findings from ten cases

Summary: This case series discusses the signs, symptoms, and management of kratom overdose in the prehospital setting and intensive care. A retrospective analysis of 10 cases in the Czech Republic revealed neurological symptoms and vegetative instability. Naloxone was effective in preventing intubation in some cases. All patients survived and recovered within two days.

Microdialysis as a tool for antibiotic assessment in patients with diabetic foot: a review

Summary: Diabetic foot is a serious complication caused by infection and ischaemia, requiring prompt and aggressive treatment to avoid amputation. Peripheral arterial disease therapy can be easily verified using ultrasound or other examinations, but infection treatment success is difficult to establish. Intravenous antibiotics are recommended for moderate or serious infections, and microdialysis techniques show promise in determining antibiotic levels in diabetic foot lesions.

Electromembrane extraction of anesthetic ketamine on-line coupled to capillary electrophoresis

Summary: This article introduces a method for extracting the anesthetic ketamine using electromembrane extraction coupled with capillary electrophoresis. The extraction occurs in a flow-through extraction mini probe, consisting of a porous hollow fiber filled with nitrophenyloctyl ether as extraction liquid. The pH of the sample solution does not affect the extraction efficiency, and the calibration dependence is linear in both model samples and urine. The LOD values are around 1 µM.

Spinal fusion for single-level SPECT/CT positive lumbar degenerative disc disease: the SPINUS I study

Summary: The study investigated the effectiveness of posterior interbody fusion on patients with SPECT/CT positive lumbar degenerative disc disease. The results showed that fusion surgery led to significant clinical improvement and high satisfaction with the treatment. SPECT/CT imaging could be a useful tool for assessing patients with chronic low back pain, especially when MRI findings are inconclusive.
