This Guideline is an official statement of the European Society of Gastrointestinal Endoscopy (ESGE). This Guideline was also reviewed and endorsed by the Governing Board of the American Society for Gastrointestinal Endoscopy (ASGE). The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system was adopted to define the strength of recommendations and the quality of evidence. ESGE guidelines represent a consensus of best practice based on the available evidence at the time of preparation. They may not apply in all situations and should be interpreted in the light of specific clinical situations and resource availability. Further controlled clinical studies may be needed to clarify aspects of these statements, and revision may be necessary as new data appear. Clinical consideration may justify a course of action at variance to these recommendations. ESGE guidelines are intended to be an educational device to provide information that may assist endoscopists in providing care to patients. They are not rules and should not be construed as establishing a legal standard of care or as encouraging, advocating, requiring, or discouraging any particular treatment.

Variability in adenoma detection rate in control groups of randomized colonoscopy trials: a systematic review and meta-analysis

Summary: This study conducted a systematic review and meta-analysis of the control groups in randomized controlled trials (RCTs) using high-definition white-light endoscopy to investigate the variability of adenoma detection rate (ADR). The results showed a high level of variability in ADR among different studies. Fecal immunochemical testing-based colonoscopy studies were found to have an impact on ADR, while primary colonoscopy screening was similar to other indications. Standardization of variables related to clinical, methodologic, and technical parameters is necessary to achieve generalizability and reproducibility.

Summary: Even with advanced imaging techniques, the presence of common bile duct sludge or stones can be missed during ERCP. Pre-procedural EUS or MRCP can reduce the proportion of unnecessary ERCPs. A shorter time interval between EUS or MRCP and ERCP increases the detection rate of suspected CBD stones during ERCP.

Summary: Pancreatic ductal adenocarcinoma (PDAC) patients with a CDKN2A/p16 pathogenic variant diagnosed under surveillance have better outcomes, including increased resectability and improved survival, compared to patients diagnosed outside surveillance. This study analyzed data from the Netherlands Cancer Registry and found that PDAC patients under surveillance had higher rates of early stage cancer, underwent more surgical resections, and had longer survival times.

Summary: Endoscopic Ultrasound (EUS) is widely used for diagnosing bilio-pancreatic and gastrointestinal (GI) tract diseases, as well as evaluating subepithelial lesions and sampling lymph nodes and solid masses. Artificial Intelligence (AI) algorithms can aid in lesion detection and characterization in EUS by analyzing images and identifying suspicious areas, leading to faster and more accurate diagnoses. Deep learning techniques, such as convolutional neural networks (CNNs), have shown potential in tumor identification and subepithelial lesion evaluation by extracting important features from EUS images. AI integration in EUS has the potential to improve diagnostic accuracy, patient outcomes, and reduce non-diagnostic biopsies.

Summary: In this study, a 3D-printed anatomically correct phantom of a female peritoneum was used to validate the thermal module of a treatment planning software. The software, based on OpenFoam, helps understand and map thermal heterogeneities during Hyperthermic IntraPeritoneal Chemotherapy (HIPEC) treatments. The experimental data compared well with simulated temperature distributions, indicating the accuracy of the software.
