The accuracy and reproducibility of the endometrial receptivity array is superior to histology as a diagnostic method for endometrial receptivity

Objective: To compare the accuracy and reproducibility of the endometrial receptivity array (ERA) versus standard histologic methods. Design: A comparative prospective study (May 2008-May 2012). Setting: University-affiliated infertility clinic. Patient(s): Eighty-six healthy oocyte donors, regularly cycling, aged 20-34 years with a body mass index (BMI) of 19-25 kg/m(2). Intervention(s): Endometrial biopsies were collected throughout the menstrual cycle. For the accuracy study, 79 samples were grouped into two cohorts: the training set (n = 79) for ERA machine-learning training and dating, and a dating subset (n 49) for comparison between histologic and ERA dating. For the reproducibility study, seven women underwent ERA testing and it was repeated in the same patients on the same day of their cycle 29-40 months later. Main Outcome Measure(s): Concordance of histologic and ERA dating related to LH as a reference, and interobserver variability between pathologists were statistically analyzed by the quadratic weighted Kappa index. The ERA reproducibility was tested and its gene expression visualized by principal component analysis. Result(s): For each pathologist, concordance against LH peak yielded values of 0.618 (0.446-0.791) and 0.685 (0.545-0.824). Interobserver variability between pathologists yielded a Kappa index of 0.622 (0.435-0.839). Concordance for ERA dating against LH peak showed a value of 0.922 (0.815-1.000). Reproducibility of the ERA test was 100% consistent. Conclusion(s): The ERA is more accurate than histologic dating and is a completely reproducible method for the diagnosis of endometrial dating and receptivity status. Integrative Genomic and Transcriptomic Profiling Reveals a Differential Molecular Signature in Uterine Leiomyoma versus Leiomyosarcoma

Summary: The absence of standardized molecular profiling to differentiate uterine leiomyosarcomas from leiomyomas is a current diagnostic challenge. In this study, artificial intelligence was used to search for a differential molecular signature for these myometrial tumors. Differential exome and transcriptome-wide research revealed higher tumor mutation burden in leiomyosarcomas compared to leiomyomas, as well as alterations in specific copy number variant regions and differentially expressed genes. A machine learning approach based on differentially expressed genes was developed to differentiate both tumor types with high sensitivity and specificity. These findings provide a novel molecular signature for the diagnosis of leiomyoma and leiomyosarcoma.

While most of our understanding of preimplantation human development comes from studying mouse embryos, this article discusses the potential differences in humans and highlights new approaches for studying this process using noninvasive imaging and genetic technologies.

Failure to achieve these milestones negatively impacts the developmental potential of embryos and may affect the success of assisted reproductive cycles.

Human embryo live imaging reveals nuclear DNA shedding during blastocyst expansion and biopsy

Summary: Combining fluorescent dyes with live imaging, this study reveals the dynamics of early development in human embryos, including chromosome segregation, compaction, polarization, blastocyst formation, and hatching. The study also shows that blastocyst expansion mechanically affects trophectoderm cells, causing nuclear budding and DNA shedding into the cytoplasm. Moreover, the study suggests that aneuploidies in human embryos may not only result from chromosome segregation errors during mitosis, but also from nuclear DNA shedding.

Association of blood cadmium and lead levels with self-reported reproductive lifespan and pregnancy loss: The national health and nutrition examination survey 1999-2018

Summary: Cadmium and lead exposure can affect hormonally regulated processes and increase reproductive lifespan and prevalence of pregnancy loss in post-menopausal women. This study evaluated the association between blood cadmium and lead levels with reproductive lifespan and personal history of pregnancy loss in a sample of 5317 post-menopausal women. The results showed that higher blood cadmium and lead levels were associated with longer reproductive lifespan and increased prevalence of pregnancy loss, especially in smokers.

To improve male reproductive health, it is essential to establish centers for data and sample collection and study the effects of genetics, epigenetics, and environment on fertility and offspring health. Personalized treatment protocols should be developed and education about infertility should be increased.

This review discusses the role of maternal decidua, the maternal-embryonic interface, the endometrial microbiome, and the myometrium in pregnancy health during the periconceptional period.
