Summary: Anovulation is a common condition characterized by amenorrhea, oligomenorrhea, and abnormal uterine bleeding. The most common chronic disorder associated with anovulation is polycystic ovary syndrome (PCOS), which affects 6% to 10% of the global population. Ovulation induction (OI) techniques are used in women with PCOS who are unable to conceive naturally. This narrative review provides an overview of the current evidence and available techniques for OI in women with PCOS, discussing their performance and applicability.

Summary: For infertile women with anovulatory PCOS, letrozole appears to improve live birth rates and pregnancy rates compared to SERMs when used for ovulation induction followed by intercourse. There is high-certainty evidence that OHSS rates are similar between letrozole and SERMs, with no difference in miscarriage rate and multiple pregnancy rate. It is uncertain if letrozole increases live birth rates compared to LOD. Updated with high-quality trials and removal of those with data validity concerns, the evidence base has been strengthened.

Summary: The objective of this study was to identify clinical, biological, and ultrasound factors that may predict resistance to 50 mg/day of CC in the first cycle of treatment for women with anovulatory PCOS. The study found that higher levels of AMH and lower levels of SHBG were significantly associated with resistance to 50 mg/day of CC. However, no satisfactory thresholds have been established for predicting resistance to 50 mg CC.

Summary: This study compared endometrial receptivity during implantation windows among letrozole, clomiphene citrate, and natural cycle in polycystic ovary syndrome (PCOS) patients. The results showed that letrozole had superior endometrial receptivity compared to clomiphene citrate, leading to higher clinical pregnancy and ongoing pregnancy rates. Endometrial flow index during the implantation window was found to have the highest predictive value for pregnancy.

Summary: The clinical efficacy of Fuke Qianjin tablets combined with clomiphene citrate in treating infertility patients with polycystic ovary syndrome (PCOS) was retrospectively analyzed in this study. One hundred PCOS infertility patients were divided into the observation and control groups. The results showed that Fuke Qianjin tablets combined with clomiphene citrate improved uterine receptivity and ovarian status, sex hormone levels, inflammation and oxidative stress, and pregnancy outcomes in PCOS infertility patients.
