Objective: To investigate whether cigarette smoking affects the blood lead levels (BLL) and whether exposure to lead introduces sperm chromatin DNA damage in factory workers. Design: Cross-sectional study. Setting: A battery plant in Taiwan. Patient(s): Eighty male workers employed within a battery plant. Intervention(s): Standard semen analysis was performed according to the World Health Organization guidelines. Main Outcome Measure(s): Assessment of BLL, sperm chromatin DNA structure, reactive oxygen species generation and other conventional parameters of semen quality. Result(s): As compared with nonsmoking workers, the BLL were found to be considerably higher among smokers. Statistically significant differences were found in the sperm DNA denaturation (alpha T) induction and the percentage of sperm with increased DNA denaturation (COMP alpha T) in workers with moderate BILL (>= 25 mu g/dL). After adjustment for smoking propensity, a positive correlation was discernible between BLL and alpha T, COMP alpha T, and morphologic abnormality. Furthermore, alpha T and COMP alpha T were also found to have positive correlations with sperm superoxide anion production. Conclusion(s): Workers with higher BLL were found to be at a higher risk of sperm morphologic abnormality and chromatin DNA integrity. These data are significant because they can facilitate the estimation of lead exposure in reproductive toxicology. (Fertil Steril (R) 2009;91:1096-103. Summary: This study examined the impact of PM2.5 components on mortality and found associations between several components and relative risk. Increasing the proportion of ammonium increased the risk, while increasing the proportion of nitrate reduced the risk. The findings contribute to the identification of more hazardous emission sources.

Summary: The study found that countries that adopted the NSPA had lower NSI incidence rates among healthcare workers, with a more significant reduction seen in nurses than physicians. Further research is needed to develop preventive strategies and incorporate them into national regulatory agency standards of care for protecting against NSIs.

The effect of cumulative noise exposure on distortion product otoacoustic emissions

I-Fan Lin, Perng-Jy Tsai, Jiunn-Liang Wu, Wei-Shan Chin, Cheng-Yu Lin, Yue Leon Guo

Summary: This study aimed to differentiate noise-induced hearing loss (NIHL) from age-related hearing loss by examining the characteristics of distortion product otoacoustic emissions (DPOAEs). The results showed that DPOAEs and DPOAE signal-to-noise ratios (SNRs) were correlated with age at all frequencies, and were correlated with both age and noise exposure at 2 kHz, 3 kHz, 4 kHz, and 6 kHz. The difference between DPOAE SNRs at 1 kHz and 3 kHz showed significant correlation with noise exposure but not with age.

Incomplete off-duty work hours and sleep quality among firefighters: a cross-sectional study, Taiwan

Rou-Shiuan Huang, Yi-Chuan Chen, Shang-Yin Tsai, Yan-Siang Eason Huang, Yue-Liang Leon Guo

Summary: This study examined the sleep problems among firefighters in Taiwan and identified the factors associated with poor sleep quality. The results showed that more than three-quarters of firefighters had poor sleep quality and it was influenced by demographic characteristics such as age, working tenure, having children, and chronotype, as well as work-related characteristics including shift schedule and incomplete off-duty hours. The study suggests that firefighters should have complete off-duty days to avoid poor sleep quality and recommends implementing appropriate shift schedules to ensure adequate rest time.

Air quality health index (AQHI) based on multiple air pollutants and mortality risks in Taiwan: Construction and validation

Summary: The currently used air quality index (AQI) is criticized for its inability to capture the additive effects of air pollution on health risks and reflect non-threshold concentration-response relationships. In this study, we propose the air quality health index (AQHI) based on daily air pollution-mortality associations and compare its validity in predicting daily mortality and morbidity risks with the existing AQI.

Summary: Current research focuses on the impact of rising temperatures on heat-related mortality, but it is unclear how climate change will exacerbate the effects of extreme summer seasons on human health. This study combines empirical heat-mortality relationships with climate model data to predict future highly impactful summer seasons. The findings emphasize the urgent need for mitigation and adaptation measures to reduce the impact on human lives.

Summary: This study investigated the interactive effects of fine particulate matter (PM2.5) and ozone (O3) on daily mortality at a global level. The results showed a synergistic effect of PM2.5 and O3 on total, cardiovascular, and respiratory mortality, particularly in high latitude regions and during cold seasons. This highlights the importance of coordinated control strategies for both pollutants.

Pretherapy F-18-FDG PET/CT in Predicting Disease Relapse in Patients With Immunoglobulin G4-Related Disease: A Prospective Study

Mei-Fang Cheng, Yue Leon Guo, Ruoh-Fang Yen, Yen-Wen Wu, Hsiu-Po Wang

Summary: This study aimed to determine whether inflammation levels detected by 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) could predict disease relapse in immunoglobulin G4-related disease (IgG4-RD) patients receiving standard induction steroid therapy. The results showed that whole-body total lesion glycolysis (WTLG) > 600 on FDG-PET was an independent risk factor for disease relapse, and could be used for guiding the treatment and management of IgG-RD patients.
