Imaging mass spectrometry (IMS) is still a relatively young imaging technique that allows molecular mapping of diverse biomolecules in their natural environment. Furthermore, IMS allows for the direct correlation of tissue histology and proteomic, metabolomic or lipidomic information. In recent years, increasing efforts have been made in the development and improvement of IMS, which aid its application in clinical research. In this article, current frontiers of clinical research applications of IMS are discussed in the context of recent developments of IMS technology. Critical stages in planning and realizing clinical studies are highlighted. Finally, a selection of recent prominent examples for successful clinical applications of IMS is presented.

Summary: Spatial metabolomics analysis showed potential for evaluating therapy response in neoadjuvant-treated esophageal adenocarcinoma patients, with the classifier based on tumor metabolite data outperforming histopathological assessment in predicting survival outcomes. This suggests that metabolic characteristics of the tumor may play a more significant role in patient prognosis than the quantity of residual tumor cells or stroma.

Summary: This study used high mass resolution imaging mass spectrometry to perform spatial metabolomics in 362 patients with gastric cancer. The study identified three tumor-specific subtypes and three stroma-specific subtypes, linked with clinicopathological characteristics and molecular features. The findings suggest that tissue-based spatial metabolomics could provide valuable additional information for existing gastric cancer molecular classification systems.

Summary: Matrix-assisted laser desorption ionization (MALDI) Fourier transform ion cyclotron resonance (FTICR) imaging mass spectrometry (MS) is a powerful technology for analyzing metabolites. This study proposes a simple and low-cost filter paper application method to improve MALDI-FTICR MS detection in lipid-rich white adipose tissue (WAT). The method removes excess liquid lipids and improves the detection of metabolites.

Summary: In this study, high mass resolution matrix-assisted laser desorption/ionization mass spectrometry imaging (MALDI-MSI) was used to classify and detect biomarkers for four common primary renal tumors. By analyzing tissue specimens, the metabolic pathways and profiles of different renal tumor subtypes were determined. This method has a high accuracy in tumor discrimination and can be a useful tool in differential diagnosis and biomarker detection.

Spatial metabolomics reveals upregulation of several pyrophosphate-producing pathways in cortical bone of Hyp mice

Summary: In this study, spatial metabolomics using matrix-assisted laser desorption/ionization Fourier-transform ion cyclotron resonance mass spectrometry imaging was employed to investigate the metabolic changes in bones of Hyp mice. The study found complex alterations in pentose phosphate, purine, pyrimidine, and phospholipid metabolism in Hyp mice. Additionally, an upregulation of biochemical pathways involved in the production of the mineralization inhibitor pyrophosphate was identified. The findings highlight the utility of MSI-based spatial metabolomics in bone research and provide insights into the changes in biochemical pathways associated with impaired bone mineralization caused by Phex deficiency.

Predicting the HER2 status in oesophageal cancer from tissue microarrays using convolutional neural networks

Summary: This study presents a deep-learning method based on convolutional neural networks (CNNs) for scoring microscopy images of gastroesophageal adenocarcinoma (GEA) patients. The method accurately detects HER2 overexpression and offers automated detection without the need for manual assessment and costly in situ hybridisation (ISH) tests. The approach shows high accuracy, precision, and recall, and exhibits superior performance in cases that require additional ISH testing.

In Situ Metabolomics of Cortisol-Producing Adenomas

Summary: Recent advances in omics techniques have allowed detailed genetic characterization of cortisol-producing adrenal adenoma (CPA). The current study conducted a comprehensive mass spectrometry imaging (MSI) map of CPAs in relation to clinical phenotypes and immunohistochemical profiles of steroidogenic enzymes. The results revealed novel metabolic links between endocrine function and tumorigenesis, which will further support the understanding of CPA pathophysiology.

Summary: COVID-19 presents with heterogeneous clinical manifestations, with severe cases leading to long-COVID. Analysis of lung pathology data and liquid biomarkers revealed that COVID-19 is mainly caused by microvascular occlusion and secondary lobular ischemia. Plasma fibrosis markers (TSP2, GDF15, IGFBP7, Pro-C3) can predict disease severity in COVID-19 infection.
