Rifaximin is a semisynthetic, rifamycin-based non-systemic antibiotic, with a low gastrointestinal absorption and a good antibacterial activity. The antibacterial action covers Gram-positive and Gram-negative organisms, both aerobes and anaerobes. Its antimicrobial action is based on its property to bind to the P-subunit of bacterial DNA-dependent RNA polymerase inhibiting, thereby, the bacterial RNA synthesis. Rifaximin contributes to restore gut microflora imbalance, becoming an important therapeutic agent in several organic and functional gastrointestinal diseases such as hepatic encephalopathy, small intestine bacterial overgrowth, inflammatory bowel disease and colonic diverticular disease. This antibiotic has the advantage of low microbial resistance and few systemic adverse events and is safe in all patient populations, including young children.

Summary: A correct differentiation between hepatocellular carcinoma (HCC) and intracellular cholangiocarcinoma (ICC) is essential but challenging. Dynamic contrast-enhanced ultrasound (D-CEUS) and shear wave elastography (SWE) can provide valuable information for accurate diagnosis. A multiparametric ultrasound (MP-US) score based on D-CEUS and SWE parameters showed high accuracy in distinguishing ICC from HCC, potentially reducing the need for liver biopsy.

Safety and efficacy of endoscopic ultrasound-guided radiofrequency ablation for pancreatic insulinoma: A single-center experience

Summary: The gut microbiota plays a critical role in host metabolism and immune response regulation, and its impairment is associated with various gastrointestinal and extraintestinal diseases. A. muciniphila is proven to be important in maintaining intestinal barrier integrity, modulating the host immune response, and improving metabolic pathways, making it a key factor in the pathogenesis of several human diseases. Further research is needed to explore its mechanisms of action and properties in order to develop a more integrated and personalized therapeutic approach based on the knowledge of gut microbiota.

Effect of COVID-19 Vaccination on the In-Hospital Prognosis of Patients Admitted during Delta and Omicron Waves in Italy

Summary: The impact of SARS-CoV-2 vaccination on the prognosis of infected patients was evaluated in this study. Results showed that vaccination with at least two doses reduced the need for ICU admission, and even one shot of the vaccine significantly reduced 30-day mortality. All-cause mortality related to SARS-CoV-2 infection has declined from the first wave to subsequent waves.

Correlation between a New Point-Shear Wave Elastography Device (X+pSWE) with Liver Histology and 2D-SWE (SSI) for Liver Stiffness Quantification in Chronic Liver Disease

Summary: This study aimed to investigate the feasibility, correlation, and accuracy of a new point shear-wave elastography device (X+pSWE) in patients with chronic liver disease. The results showed a good correlation between X+pSWE and 2D-SWE with SSI, with X+pSWE values being slightly lower. X+pSWE had high AUROC values for staging fibrosis, and correctly identified most patients according to histologic classification.

A literature search identified several studies that demonstrated an increase in thrombus stiffness with DVT age, despite significant differences in study design, thrombus age definitions, and patient characteristics.

Gut Microbiota and Infectious Complications in Advanced Chronic Liver Disease: Focus on Spontaneous Bacterial Peritonitis

Summary: Liver cirrhosis is a chronic disease that often leads to complications such as variceal bleeding, hepatic encephalopathy, ascites, and jaundice, with increased mortality. Infections, particularly spontaneous bacterial peritonitis (SBP), are common in cirrhotic patients due to impaired immunity and altered intestinal microbiota. SBP is primarily caused by Gram-negative bacteria translocating through the defective intestinal barrier. The first-line treatment is antibiotic therapy, but it may worsen dysbiosis. Future therapies aim to selectively modulate the gut microbiota or reduce intestinal permeability.

Summary: Patients with post-COVID-19 syndrome exhibit greater clinical complexity and a higher burden of comorbidities, as indicated by a higher CCI. Additionally, a higher CCI is associated with an increased risk of post-COVID-19 syndrome, with the presence of 1 or 4 or more CCI points associated with a 3-fold and 6-fold increased risk, respectively.
