Summary: Triptolide, the active component of Triptergium wilfordii Hook. F, shows potential for drug development in the treatment of cancers and autoimmune diseases. However, its clinical application is limited due to liver toxicity. Recent studies have identified various factors involved in triptolide-induced hepatotoxicity, including metabolic imbalance, oxidative stress, inflammation, autophagy, apoptosis, regulation of cytochrome P450 enzymes, gut microbiota, and immune cells. This review provides a comprehensive understanding of the pharmacological applications and hepatotoxicity mechanism of triptolide.

Summary: The study found that 1 beta-hydroxy-deoxycholic acid (1 beta-OH-DCA) to deoxycholic acid (DCA) urinary metabolic ratio (UMR) correlated significantly with oral MDZ clearance and C-max at baseline, and reflected modulation of CYP3A after rifampicin intake. However, a non-significant decrease in 1 beta-OH-DCA/DCA was observed during the inhibition session, possibly due to the short duration of CYP3A inhibitors intake in the clinical trial. Further studies with longer periods of CYP3A inhibition and larger sample sizes are needed to confirm the suitability of 1 beta-OH-DCA/DCA as a CYP3A biomarker.
