Summary: This study aims to assess the prevalence of falls in community-dwelling frail older people aged 70 years and older, and identifies screening strategies based on comprehensive geriatric assessments to detect increased risk of falling and recurrent falling. The study found that among the 229 participants, 121 (54.9%) had fallen in the previous 12 months, and 20 of these (16.5%) were recurrent fallers (>= 2 falls). A score of 20 points or more on the Falls Efficacy Scale International is predictive of falling, with a sensitivity of 51.7% and a specificity of 73.9%. Polypharmacy, Short Physical Performance Battery score of 8 points or less, and Falls Efficacy Scale International score of 20 points or more can be used to screen for recurrent falling.

Summary: This study aims to determine the association between lifestyle factors (smoking, alcohol use, nutrition, physical activity) and multidimensional frailty. The results showed that higher alcohol consumption, physical activity, healthy nutrition, and less smoking were associated with lower levels of frailty. Physical activity had the greatest impact on frailty.

Summary: This study examined the association between the onset of physical frailty before, after, or in concert with cognitive impairment and fall incidence in older adults. The results showed that older adults experiencing both cognitive impairment and frailty had the greatest risk of repeated falls. Fall risk screening should take into account the order and timing of onset of physical frailty and cognitive impairment.

Summary: The study aimed to evaluate whether the addition of other biological and social variables to the frailty phenotype (FP) could improve the prediction of declining functional ability in community-dwelling older people. A scoring system was derived and validated using data from two studies, including FP components, age, perceived health status, and socio-economic disadvantage markers. The results showed that this scoring system was able to predict the declining functional ability in older adults.

Summary: This study aimed to assess the characteristics and validities of the Clinical Frailty Scale (CFS) in community-dwelling older people in Korea using the original classification tree (oCFS) and a culturally modified tree (mCFS). The results showed that the correlation coefficients with frailty index were higher in mCFS than in oCFS, and the classification coefficient of mCFS was significantly higher than that of oCFS in determining people with frailty. Therefore, the culturally modified CFS classification tree is considered valid in detecting the vulnerable population in the community-dwelling older people in Korea.

Summary: This study aimed to identify predictive attributes for accidental falls in community-dwelling older people in Hong Kong using decision tree analysis. A cross-sectional study was conducted over 6 months, with 1151 participants recruited from a primary healthcare setting. The results showed that decision tree analysis created decision models for fall screening in older people and paved the way for utility-based decision-making using supervised machine learning in fall risk detection.

Summary: The systematic review identified a variation in frailty prevalence among individuals aged 50 or older with HIV. The meta-analysis results showed that the overall prevalence of frailty and prefrailty in this population was 10.9% and 47.2% respectively. Frailty is a clinically recognized state associated with disability, hospitalization, and mortality. Effective strategies for frailty screening and intervention are needed for this vulnerable population.
