Background. Several types of drugs have been recommended for the secondary prevention of myocardial infarction (MI). However, these conventional strategies have several limitations, such as low adherence, high cost, and side effects during long time use. Novel approaches to this problem are still needed. This trial aimed to test the effectiveness and safety of Qi-Shen-Yi-Qi Dripping Pills (QSYQ), a multi-ingredient Chinese patent medicine, for the secondary prevention of MI. Methods and Findings. A total of 3505 eligible patients were randomly assigned to QSYQ group (1746 patients) or aspirin group (1759). Patients took their treatments for 12 months. The final follow-up visit took place 6 months after the end of the trial drugs. The 12-month and 18-month estimated incidences of the primary outcome were 2.98% and 3.67%, respectively, in the QSYQ group. The figures were 2.96% and 3.81% in the aspirin group. No significant difference was identified between the groups. Conclusions. This trial did not show significant difference of primary and secondary outcomes between aspirin and QSYQ in patients who have had an MI. Though inconclusive, the result suggests that QSYQ has similar effects to aspirin in the secondary prevention of MI.

Dynamic variations of bioactive compounds driven by enzymes in Psoralea corylifolia L. from growth to storage and processing

Danni Wang, Jiaming Guo, Xin Chai, Jing Yang, Yuefei Wang, Xiumei Gao

Summary: This study focused on the dynamic variations of compounds in Psoralea corylifolia during its growth, storage, and treatment by different temperatures. It successfully established a method for the simultaneous determination of nine compounds and identified the characteristics of compounds' variation in different parts of the plant. The results demonstrated that compounds' transformation arising from the growth, processing, and storage of Psoralea corylifolia are critical factors to ensure the quality of Fructus Psoraleae.

Summary: Addressing chemical complexity is crucial in various research fields. The integration of multidimensional chromatography and mass spectrometry enables comprehensive characterization of herbal metabolites. In this study, a three-dimensional liquid chromatography system combined with quadrupole time-of-flight mass spectrometry was used to fully characterize the metabolome of two Astragalus species. By utilizing an in-house library and reference standards, a total of 513 compounds from the Astragalus species were identified. This approach allows for in-depth characterization of herbal metabolites.

Rg3 regulates myocardial pyruvate metabolism via P300-mediated dihydrolipoamide dehydrogenase 2-hydroxyisobutyrylation in TAC-induced cardiac hypertrophy

Jingyu Ni, Hao Zhang, Xiaodan Wang, Zhihao Liu, Tong Nie, Lan Li, Jing Su, Yan Zhu, Chuanrui Ma, Yuting Huang, Jingyuan Mao, Xiumei Gao, Guanwei Fan

Summary: This study reveals that 2-hydroxyisobutyrylation is an essential protein translational modification that regulates the activity of the pyruvate dehydrogenase complex (PDHc) and has an impact on cardiac hypertrophy. Ginsenoside Rg3 can reduce 2-hydroxyisobutyrylation levels and restore PDH activity by inhibiting the activity of P300, suggesting a potential therapeutic effect for reversing myocardial hypertrophy.

SUMO1 regulates post-infarct cardiac repair based on cellular heterogeneity

Zhihao Liu, Xiaozhi Liu, Li Liu, Ying Wang, Jie Zheng, Lan Li, Sheng Li, Han Zhang, Jingyu Ni, Chuanrui Ma, Xiumei Gao, Xiyun Bian, Guanwei Fan

Summary: SUMOylation is a dynamic post-translational modification that maintains cardiac function and protects against hypertrophic response in the heart. The role of SUMO1 in myocardial infarction (MI) and the molecular details of heart cell responses to SUMO1 deficiency were investigated in this study. SUMO1 knockout worsened systolic dysfunction and infarct size after myocardial injury, and its loss promoted proliferation of endothelial cell subsets with angiogenic potential.

Chlorogenic acid reduces inflammation by inhibiting the elevated expression of KAT2A to ameliorate lipopolysaccharide-induced acute lung injury

Bin Lv, Jinhe Guo, Yaodong Du, Yang Chen, Xin Zhao, Bin Yu, Jiarui Liu, Tianyi Cui, Haoping Mao, Xiaoying Wang, Xiumei Gao

Summary: Respiratory diseases, such as acute lung injury (ALI), have become a global health problem. The excessive activation and recruitment of immunocytes in the lungs, as well as the release of cytokines, are considered the primary causes of ALI. However, the cellular mechanisms involved in ALI remain unknown. Therefore, new therapeutic strategies need to be developed to control inflammation and prevent the worsening of ALI.

Remodeling of the gut microbiome by Lactobacillus johnsonii alleviates the development of acute myocardial infarction

Xinqin Zhong, Yucui Zhao, Lu Huang, Jiarui Liu, Kaiyue Wang, Xiumei Gao, Xin Zhao, Xiaoying Wang

Summary: In this study, it was demonstrated that L. johnsonii protects cardiac function, delays cardiac pathology, and improves gut barrier integrity by remodeling the gut microbiome. The beneficial effects of L. johnsonii on cardiac function post-AMI were abrogated by microbiome dysbiosis caused by antibiotics. The enrichment of L. johnsonii reshapes the gut microbiome by increasing the abundance of Muribaculaceae, Lactobacillus, and decreasing Romboutsia, Clostridia UCG-014, which are correlated with cardiac traits and serum metabolic biomarkers.

M2 Macrophage-Derived sEV Regulate Pro-Inflammatory CCR2+ Macrophage Subpopulations to Favor Post-AMI Cardiac Repair

Summary: Tissue-resident cardiac macrophage subsets play important roles in cardiac tissue inflammation and repair after acute myocardial infarction (AMI). CC chemokine receptor 2 (CCR2)-expressing macrophages recruit circulating monocytes to infarcted myocardium and promote inflammation. However, small extracellular vesicles (sEV) derived from CCR2 macrophages, which resemble M2-polarized macrophages, have anti-inflammatory effects and contribute to cardiac repair.

Three-dimensional characteristic chromatogram by online comprehensive two-dimensional liquid chromatography: Application to the identification and differentiation of ginseng from herbal medicines to various Chinese patent medicines

Xiaoyan Xu, Meiting Jiang, Xiaohang Li, Yu Wang, Meiyu Liu, Hongda Wang, Yueguang Mi, Boxue Chen, Xiumei Gao, Wenzhi Yang

Summary: We propose the concept of a three-dimensional characteristic chromatogram (3D CC) and use it to authenticate various ginseng products through online comprehensive two-dimensional liquid chromatography (LC x LC) technology. By analyzing the stereo peaks and shade depth in the 3D CC, it is feasible to identify and distinguish different ginseng species and Chinese patent medicines. This provides a practical solution for the accurate identification of easily confused traditional Chinese medicines (TCMs).

Summary: This study aimed to reveal the hepatotoxicity distinction of coumarin-related compounds from glycosides to aglycones in Fructus Psoraleae (FP) and elucidate their potential mechanism. The results showed that P, IP, FA, and IFA were identified as hepatotoxic compounds, while their glycosides were almost non-hepatotoxic. The conversion from glycosides to aglycones played an essential role in FP-induced hepatotoxicity.
