The intratumoral CXCR3 chemokine system is predictive of chemotherapy response in human bladder cancer

Tino Vollmer, Stephan Schlickeiser, Leila Amini, Sarah Schulenberg, Desiree J. Wendering, Viqar Banday, Anke Jurisch, Rebecca Noster, Desiree Kunkel, Nicola R. Brindle, Ioannis Savidis, Levent Akyuz, Jochen Hecht, Ulrik Stervbo, Toralf Roch, Nina Babel, Petra Reinke, Ola Winqvist, Amir Sherif, Hans-Dieter Volk, Michael Schmueck-Henneresse

Summary: The CXCR3-CXCL11 chemokine system plays a crucial role in predicting chemotherapy responsiveness in MIBC by stimulating CD8(+) T cells. The presence of CXCR3alt and CXCL11 can differentiate responder and nonresponder patients before treatment, and may provide immunotherapeutic options for activating intratumoral stem-like T cells in solid tumors.

Proteomic Profiling of Tissue Exosomes Indicates Continuous Release of Malignant Exosomes in Urinary Bladder Cancer Patients, Even with Pathologically Undetectable Tumour

Maria Eldh, Michael Mints, Stefanie Hiltbrunner, Sam Ladjevardi, Farhood Alamdari, Markus Johansson, Tomasz Jakubczyk, Rosanne E. Veerman, Ola Winqvist, Amir Sherif, Susanne Gabrielsson

Summary: The study investigated the release of exosomes from urinary bladder tissue post-TUR-B in UBC patients to find cancer-promoting exosomes originating from both tumour-proximal and tumour-distant sites, highlighting the need for early radical cystectomy. This suggests that the entire bladder tissue may release exosomes contributing to metastasis despite the absence of detectable tumour, supporting the role of exosomes as potential biomarkers and mediators of metastatic spread. Additionally, the enrichment of cancer-related pathways in tissue-derived exosomes from both sites strengthens the hypothesis and emphasizes the necessity for early removal of the bladder in UBC patients.

Control computerized tomography in neoadjuvant chemotherapy for muscle invasive urinary bladder cancer, has no value for treatment decisions and low correlation with nodal status

S. Tenninge, H. Mogos, E. Eriksson, H. Netterling, S. Pelander, M. Johansson, F. Alamdari, Y. Huge, F. Aljabery, J. Svensson, J. Styrke, A. Sherif

Summary: Background Control computerized tomography (cCT) is routinely used in many cystectomy centres before the final treatment cycle in patients with muscle-invasive urinary bladder cancer (MIBC) undergoing neoadjuvant chemotherapy (NAC). This study aimed to evaluate the frequency of changed treatment strategies following cCT and investigate discrepancies between cCT results and final nodal staging. The results showed a low percentage of treatment strategy changes after cCT and that cCT cannot accurately predict pN-status in MIBC patients undergoing NAC.

Summary: Patients diagnosed with non-muscle-invasive bladder cancer at age <50 have a decreased risk of bladder cancer-specific death compared to those aged 50-70, with no significant difference in lower urinary tract infection diagnoses between age groups. These findings suggest potential age-related differences in diagnostic procedures, clinical decision-making, and tumor biology.

Standardized care pathways for patients with suspected urinary bladder cancer: the Swedish experience

Suleiman Abuhasanein, Staffan Jahnson, Firas Aljabery, Truls Gardmark, Tomas Jerlstrom, Fredrik Liedberg, Amir Sherif, Viveka Strock, Henrik Kjolhede

Summary: This study compared the differences in diagnosis and treatment time, tumor characteristics, and management in patients with primary urinary bladder cancer before and after the implementation of a standardized care pathway (SCP) in Sweden. The results showed a decrease in the median time to transurethral resection of bladder tumor (TURBT) after SCP implementation. The proportion of patients discussed at a multidisciplinary team conference (MDTC) increased, but did not reach the recommended levels. Measures should be taken to increase adherence to SCP recommendations and guidelines.

Central Venous Access and the Risk for Thromboembolic Events in Patients Undergoing Neoadjuvant Chemotherapy and Radical Cystectomy for Muscle-Invasive Bladder Cancer

Harriet Rydell, Ylva Huge, Victoria Eriksson, Markus Johansson, Farhood Alamdari, Johan Svensson, Firas Aljabery, Amir Sherif

Summary: This study aims to compare the differences in thromboembolic events (TEE) between peripherally inserted central catheter (PICC) and totally implanted port (PORT) as central venous access during neoadjuvant chemotherapy (NAC) in patients with urothelial muscle-invasive bladder cancer (MIBC). The results show a higher risk of TEE in PICC patients compared to PORT patients, suggesting a preference for the usage of PORTs in MIBC-NAC patients.

Summary: This study investigated the overall and disease-specific risk of death in patients with non-muscle invasive bladder cancer (NMIBC). The results showed a slightly increased risk of death from non-bladder cancer causes in NMIBC patients, which may be explained by confounding factors. Repeated transurethral procedures during follow-up did not increase the risk of death from non-bladder cancer causes.

Summary: This study aimed to assess the incidence and severity of adverse events (AEs) during neoadjuvant chemotherapy (NAC) for muscle invasive bladder cancer (MIBC), and to investigate the possible effects of AEs on downstaging outcomes. The results showed that NAC in MIBC poses a significant risk for AEs with clinical implications, affecting multiple organ systems. It was also found that patients who prematurely terminated chemotherapy had higher AE grades and decreased downstaging, and that anemia and acute kidney injury were individually associated with decreased downstaging.

Immune-Activated B Cells Are Dominant in Prostate Cancer

Aws Saudi, Viqar Banday, A. Ali Zirakzadeh, Martin Selinger, Jon Forsberg, Martin Holmbom, Johan Henriksson, Mauritz Walden, Farhood Alamdari, Firas Aljabery, Ola Winqvist, Amir Sherif

Summary: Contrary to common belief, our study shows that prostate cancer has tumor-associated B-cell responses. We found mature and activated B cells, including effector plasmablasts, in tumor-draining lymph nodes, indicating a B-cell-specific antitumor immune response. This highlights the importance of targeting B cells in prostate cancer immunotherapy.

A Validation Study of cT-Categories in the Swedish National Urinary Bladder Cancer Register-Norrland University Hospital

Erik Wiberg, Andres Vega, Victoria Eriksson, Viqar Banday, Johan Svensson, Elisabeth Eriksson, Staffan Jahnson, Amir Sherif

Summary: This study aimed to validate the registered clinical tumour categories (cT-categories) in the Swedish National Register for Urinary Bladder Cancer (SNRUBC) for Norrland University Hospital, Sweden, from 2009 to 2020. The study found significant incorrect registration of cT-categories, with a major part of the misclassifications related to tumour-associated hydronephrosis (TAH) and tumour in bladder diverticulum (TIBD). The inclusion of these variables in SNRUBC is recommended to improve correct cT-classification.
