We describe the aetiology of community-acquired pneumonia (CAP) in HIV-infected patients, risk factors for bacterial or Pneumocystis jirovecii CAP and prognostic factors of 30-day mortality. This was a prospective observational study of 331 consecutive adult CAP cases in HIV-infected patients (January 2007 to July 2012). 128 (39%) patients had CD4(+) cell counts <200 per mm(3) and 99 (43%) ha HIV RNA levels <200 copies per mL on antiretroviral therapy. Streptococcus pneumoniae was the most frequent microorganism in the group with CD4(+) cell counts >= 200 per mm(3); P. jirovecii was the most frequent microorganism in the group with CD4(+) cell counts <200 per mm(3) and in patients with HIV RNA >= 200 copies per mL. Predictors of bacterial CAP were: time with symptoms <= 5 days (OR 2.6, 95% CI 1.5-4.4), C-reactive protein level mg center dot dL(-1) (OR 4.3, 95% CI 2.3-8.2) and hepatitis C virus co-infection (OR 2.3, 95% CI 1.4-3.9). White blood cell count <= 4 x 10(12) per L (OR 3.7, 95% CI 1.2-11.5), lactate dehydrogenase (LDH) level >= 598 U center dot L-1 (OR 12.9, 95% CI 4.2-39.7) and multilobar infiltration (OR 5.8, 95% CI 1.9-19.5) were predictors of P. jirovecii. Overall 30-day mortality was 7%. Appropriate antibiotic treatment (OR 0.1, 95% CI 0.03-0.4), LDH >= 598 U center dot L-1 (OR 6.2, 95% CI 1.8-21.8) and mechanical ventilation (OR 22.0, 95% CI 6.2-78.6) were the variables independently associated with 30-day mortality. The described predictors may help clinicians to distinguish between bacterial and P. jirovecii pneumonia in patients with suspected or confirmed HIV infection.

Population-based assessment of SARS-CoV-2 infection among people living with HIV and the general population of Catalonia (March-December, 2020)

Daniel K. Nomah, Yesika Diaz, Rosa M. Vivanco-Hidalgo, Jordi Casabona, Jose M. Miro, Juliana Reyes-Uruena

Summary: This study compared the impact of SARS-CoV-2 on people living with HIV (PLWH) and the general HIV-negative population in Catalonia, Spain. The findings showed that PLWH had lower testing rates but higher test positivity for SARS-CoV-2 compared to the general population. There were no significant differences in hospitalization and ICU admission rates, but PLWH had a lower mortality rate.

Summary: This study reports a case of a highly controlled HIV infection individual who maintained undetectable viral loads for 15 years after stopping antiretroviral therapy. The researchers found that the high activity of NK cells and CD8 T cells were associated with the control of viral replication.

Catia Cilloniz, Logan Ward, Mads Lause Mogensen, Juan M. Pericas, Raul Mendez, Albert Gabarrus, Miquel Ferrer, Carolina Garcia-Vidal, Rosario Menendez, Antoni Torres

Summary: This study applies a CPN model to predict mortality in CAP patients and compares it with other scoring systems. The results show that the SeF-ML model has higher predictive accuracy, suggesting its potential for improving mortality prediction in CAP patients.

Comparison of the design and methodology of Phase 3 clinical trials of bictegravir/emtricitabine/tenofovir alafenamide (BIC/FTC/TAF) and dolutegravir-based dual therapy (DTG) in HIV: a systematic review of the literature

Santiago Grau, Jose Maria Miro, Julian Olalla, Juan C. Alcala, Antonio Castro, Dario Rubio-Rodriguez, Carlos Rubio-Terres

Summary: This study aims to determine whether the quality of evidence from clinical trials of dolutegravir is methodologically comparable to that of clinical trials conducted with bictegravir/emtricitabine/tenofovir alafenamide. The quality of clinical trials was assessed using the Jadad scale, and the results showed that there were methodological differences between the trials, making the results not comparable.

Treatment with integrase inhibitors alters SARS-CoV-2 neutralization levels measured with HIV-based pseudotypes in people living with HIV

Erick De la Torre-tarazona, Alba Gonzalez-Robles, Almudena Cascajero, Paloma Jimenez, Jose Maria Miro, Sonsoles Sanchez-Palomino, Jose Alcami, Maria Jose Buzon, Javier Garcia-Perez, Hospital Clinic HIV Investigators

Summary: This study compares two methods using HIV and VSV-based pseudoviruses to detect neutralizing antibodies (NAbs) against SARS-CoV-2 in people living with HIV (PLWH). The HIV-pseudoparticles method shows higher neutralization titers, but also detects NAbs in COVID-19-naive PLWH. Treatment with integrase inhibitors can lead to inaccurate results when using the HIV-based pseudoviruses method, but this interference can be eliminated through IgG purification.

Summary: This study examined the characteristics of people living with HIV (PLWH) who sought consultation in a tertiary hospital in Barcelona, Spain, from 1982 to 2020, with a focus on sex-related differences. The findings revealed that women with HIV had higher rates of late diagnoses compared to men, and women aged 50 and above represented a significant proportion of those currently being followed up. These results emphasize the importance of sex stratification in HIV prevention and control interventions.
