Cumulative Blood Pressure in Early Adulthood and Cardiac Dysfunction in Middle Age The CARDIA Study

BACKGROUND Cumulative blood pressure (BP) exposure may adversely influence myocardial function, predisposing individuals to heart failure later in life. OBJECTIVES This study sought to investigate how cumulative exposure to higher BP influences left ventricular (LV) function during young to middle adulthood. METHODS The CARDIA (Coronary Artery Risk Development in Young Adults) study prospectively enrolled 5,115 healthy African Americans and whites in 1985 and 1986 (baseline). At the year 25 examination, LV function was measured by 2-dimensional echocardiography; cardiac deformation was assessed in detail by speckle-tracking echocardiography. We used cumulative exposure of BP through baseline and up to the year 25 examination (millimeters of mercury x year) to represent long-term exposure to BP levels. Linear regression and logistic regression were used to quantify the association of BP measured repeatedly through early adulthood (18 to 30 years of age) up to middle age (43 to 55 years). RESULTS Among 2,479 participants, cumulative BP measures were not related to LV ejection fraction; however, high cumulative exposure to systolic blood pressure (SBP) and diastolic blood pressure (DBP) were associated with lower longitudinal strain rate (both p < 0.001). For diastolic function, higher cumulative exposures to SBP and DBP were associated with low early diastolic longitudinal peak strain rate. Of note, higher DBP (per SD increment) had a stronger association with diastolic dysfunction compared with SBP. CONCLUSIONS Higher cumulative exposure to BP over 25 years from young adulthood to middle age is associated with incipient LV systolic and diastolic dysfunction in middle age. Heart Failure-Smart Life: a randomized controlled trial of a mobile app for self-management in patients with heart failure

Genetic testing should be considered for those with a high suspicion of underlying genetic aortopathy. The implementation of these strategies can standardize the diagnostic work-up and follow-up of patients with suspected HTAD and their relatives.

Canagliflozin independently reduced plasma volume from conventional diuretics in patients with type 2 diabetes and chronic heart failure: a subanalysis of the CANDLE trial

Summary: SGLT2 inhibitors have been found to reduce the risk of heart failure progression and mortality rates. This study shows that SGLT2 inhibitors can chronically reduce estimated plasma volume and improve heart failure prognosis in patients with type 2 diabetes and chronic heart failure, especially in those with preserved ejection fraction. The subanalysis of the CANDLE trial indicates that canagliflozin significantly reduces estimated plasma volume without worsening renal function and lowers serum uric acid without causing major electrolyte abnormalities.

Summary: Coronary artery calcium (CAC) scanning has limited value for risk stratification in symptomatic patients. This study aimed to identify and validate predictors of significant coronary obstruction (SCO) in symptomatic patients without CAC. Summary: We compared the diagnostic performance, costs, and association with major adverse cardiovascular events (MACE) of clinical coronary computed tomography angiography (CCTA) interpretation and a semiautomated artificial intelligence and machine learning approach (AI-QCT) for atherosclerosis imaging. The use of AI-QCT significantly reduced the rates and costs of invasive coronary angiography (ICA) in stable patients referred for nonemergent ICA, with no change in 1-year MACE.

Summary: This study investigates the aortic-related risks associated with pregnancy in women with bicuspid aortic valve (BAV) and evaluates changes in aortic diameter during pregnancy. The results show that pregnant women with BAV have a low rate of cardiac complications and aortic complications are rare. However, there is a small but significant increase in aortic diameter during pregnancy.

Relationship Between Canagliflozin, Sodium Glucose Cotransporter 2 Inhibitor, and Hematopoietic Effects in Patients With Diabetes and Mild Heart Failure: Results From the CANDLE Trial

Summary: There were limited clinical studies on the relationship between SGLT2 inhibitors and hematopoiesis in patients with diabetes and heart failure. In the CANDLE trial, 226 patients were studied, and it was found that canagliflozin was associated with increased hematocrit and hemoglobin levels compared to glimepiride. In conclusion, canagliflozin improved hematologic parameters in patients with diabetes and heart failure.

Summary: This study aimed to assess the disease progression and evolution of echocardiographic variables in patients with severe low-flow low-gradient AS. The results showed that the progression of mean gradient in low-gradient groups (LFLG and NFLG) was greater compared to the high-gradient group (HG), while the reduction of AVA was slower in LFLG group compared to NFLG. Most patients initially classified as LFLG AS changed over time to other severe forms of AS and received aortic valve replacement with high-gradient AS.

Summary: This study conducted a large-scale analysis and identified eleven new risk loci for coronary artery calcification (CAC), a measure of subclinical atherosclerosis. These new loci are related to bone mineralization, phosphate catabolism, and hormone metabolic pathways. The findings contribute to a better understanding of the genetic architecture of CAC and provide potential targets for therapeutic interventions.
