Pregnancy outcomes in pulmonary arterial hypertension in the modern management era

Previous studies have reported mortality rates of up to 56% associated with pregnancy in pulmonary arterial hypertension (PAH) but the management of this disease has changed considerably in recent years. We compiled a multinational, prospective registry to examine the contemporary outcome of pregnancies in patients with PAH. During a 3-yr period, the 13 participating centres reported 26 pregnancies. Three (12%) females died and one (4%) developed right heart failure requiring urgent heart-lung transplantation. There were eight abortions; two spontaneous and six induced. 16 (62%) pregnancies were successful, i.e. the females delivered healthy babies without complications. These females had well controlled PAH (pulmonary vascular resistance (PVR) 500 +/- 352 dyn.s.cm(-5)); eight of them were long-term responders to calcium channel blockers. In contrast, the females who died or required transplantation had poorly controlled PAH (PVR 1,667 +/- 209 dyn.s.cm(-5)). Pregnancy remains associated with a substantial mortality rate in PAH. However, our results indicate that the outcome of pregnancy in PAH has improved, at least when PAH is well controlled, and particularly in long-term responders to calcium channel blockers. These data must be confirmed by larger series before the general recommendation to avoid pregnancy in all patients with PAH is reconsidered.

Summary: This study analyzed the response to PAH therapy in IPAH patients and compared the differences between patients without comorbidities, those with 1-2 comorbidities, and those with 3-4 comorbidities. The results showed that patients with comorbidities had smaller improvements in functional class, 6-minute walking distance, BNP/NT-pro-BNP, and mortality risk compared to patients without comorbidities. However, the 4-strata risk tool predicted the outcome in IPAH patients regardless of the presence of comorbidities.

Summary: Patients with chronic obstructive pulmonary disease (COPD) may experience acute episodes of worsening dyspnea, accompanied by increased cough, sputum, and/or sputum purulence. These exacerbations impact health status, accelerate lung function decline, and increase the risk of hospitalization. It is important to recognize and address other morbidities that can mimic or aggravate COPD exacerbations to improve outcomes and reduce readmissions.

Clinical, procedural and lead outcomes associated with different pacing techniques: a network meta-analysis

Summary: This study compared different pacing techniques in terms of clinical, procedural, and lead outcomes. Left bundle branch pacing (LBBP) significantly improved left ventricular ejection fraction (LVEF), narrowed QRS duration, and reduced hospitalization for heart failure (HFH) compared to other pacing techniques. His-bundle pacing (HBP) had higher pacing thresholds and lower R-wave amplitudes over time compared to LBBP. Biventricular pacing (BVP) and HBP were associated with a higher risk of lead issues compared to LBBP.

Multiparametric evaluation of right ventricular function in pulmonary arterial hypertension associated with congenital heart disease

Summary: This study aimed to explore the accuracy of different echocardiographic parameters in assessing right ventricular function in patients with pretricuspid shunt and pulmonary arterial hypertension (PAH). The results showed that right ventricular global longitudinal strain, right atrial area, and right ventricular end-systolic remodeling index were strongly correlated with right ventricular ejection fraction. These parameters are important markers for evaluating right ventricular dysfunction in patients with pretricuspid shunt and PAH.

Early selexipag initiation and long-term outcomes: insights from randomised controlled trials in pulmonary arterial hypertension

Summary: Further understanding of when to initiate therapies in pulmonary arterial hypertension (PAH) is important to improve long-term outcomes. Post hoc analyses of GRIPHON (NCT01106014) and exploratory analyses of TRITON (NCT02558231) suggested benefit of early selexipag initiation on long-term outcomes, despite no additional benefit versus initial double combination on haemodynamic and functional parameters in TRITON. These post hoc, pooled analyses build on previous insights, further supporting selexipag use within 6 months of diagnosis, including as part of triple therapy, to delay disease progression.

Summary: Precapillary pulmonary hypertension (PH) is a rare and often overlooked complication of myeloproliferative neoplasms (MPNs). This study describes the characteristics and outcomes of MPN-associated PH. Patients with MPN often present with severe hemodynamic impairment and impaired clinical conditions. Chronic thromboembolic PH and group 5 PH are common diagnoses. Thromboendarterectomy can improve survival rates.

Summary: Activin A and FSTL3 serum levels are associated with transplant-free survival in PAH. They can serve as prognostic biomarkers for PAH, and their diagnostic values have been confirmed in an independent validation cohort. The activin signaling system in PAH involves phosphorylated Smad2/3 nuclear accumulation and differential immunoreactivities of various proteins in vascular endothelial and smooth muscle layers.

Pharmacological management of connective tissue disease-associated pulmonary arterial hypertension

However, the study suggests that the combination therapy of endothelin receptor antagonists (ERAs) and phosphodiesterase type 5 inhibitors (PDE5is) may improve the long-term survival of patients with SSc-PAH.

Summary: Pulmonary veno-occlusive disease (PVOD), characterized by substantial involvement of small pulmonary veins and capillaries, is an uncommon etiology of pulmonary arterial hypertension (PAH). Recent advancements include the discovery of biallelic mutations in the EIF2AK4 gene causing heritable PVOD. Differentiating PVOD from PAH is vital due to the worse prognosis and poor response of PVOD to PAH-approved drugs.

Exploratory study on the endogenous ouabain in idiopathic pulmonary arterial hypertension patients

Summary: This study found that patients with idiopathic pulmonary arterial hypertension had higher plasma concentrations of endogenous ouabain (EO) than control subjects. The concentrations were negatively correlated with pulmonary arterial pressure and vascular resistance, suggesting a potential role of EO in the adaptation of right ventricular afterload.
