Survival in pulmonary hypertension in Spain: insights from the Spanish registry

A pulmonary hypertension (PH) registry (Spanish Registry of Pulmonary Arterial Hypertension) was undertaken to analyse prevalence, incidence and survival of pulmonary arterial hypertension (PAH) and chronic thromboembolic pulmonary hypertension (CTEPH) in Spain, and to assess the applicability of recent survival prediction equations. Voluntary reporting of previously diagnosed and incident PAH or CTEPH cases (July 2007 June 2008) was performed. Demographic, functional and haemodynamic variables were evaluated. 866 patients with PAH and 162 with CTEPH were included. PAN associated with toxic oil syndrome and pulmonary veno-occlusive disease were reported for the first time in a PAH registry. Estimated prevalences were as follows: PAH, 16 and CTEPH, 3.2 cases per million adult inhabitants (MAI). Estimated incidences were as follows: PAH, 3.7 and CTEPH, 0.9 cases per MAI per yr. 1-, 3- and 5-yr survival was 87%, 75% and 65%, respectively, with no differences between PAH and CTEPH. Male sex, right atrial pressure and cardiac index were independent predictors of death. Matching between observed survival and that predicted by different equations was closer when the characteristics of the cohorts were similar. Epidemiology and survival of PAH patients in the Spanish registry are similar to recent registries. Characteristics of the population from which survival prediction equations are derived influence their applicability to a different cohort. CTEPH is much less prevalent than PAN, although has a similar survival rate.

Sotatercept for the treatment of pulmonary arterial hypertension: PULSAR open-label extension

Summary: In participants with pulmonary arterial hypertension, 24 weeks of sotatercept resulted in a significant reduction in pulmonary vascular resistance. This study reports the longer-term safety and efficacy of sotatercept in the PULSAR open-label extension, showing that sotatercept has good safety and efficacy in patients with pulmonary arterial hypertension.

Summary: This study aimed to evaluate the efficacy of the selective beta 3 adrenoreceptor agonist mirabegron in patients with combined pre- and post-capillary pulmonary hypertension (CpcPH). The results showed that mirabegron treatment did not reduce pulmonary vascular resistance in CpcPH patients, but significantly improved right ventricular ejection fraction.

Summary: This study retrospectively collected data from 14 patients with persistent pulmonary hypertension (PH) after pulmonary endarterectomy (PEA) who were treated with balloon pulmonary angioplasty (BPA). The results showed that BPA effectively reduced pulmonary arterial pressure and pulmonary vascular resistance, and improved the clinical status of patients. Severe BPA-related complications were rare and no perioperative deaths were observed.

Summary: Pulmonary arterial hypertension (PAH) is a severe condition with a high mortality rate. Despite advances in diagnostic and therapeutic strategies, current treatments mainly focus on pulmonary vasodilation and do not address the pathological changes in the pulmonary vasculature. This review discusses the molecular mechanisms involved in PAH, the development of novel therapeutic compounds, and their potential role in future PAH treatment algorithms.

Mesenchymal stem cells (MSC), known for their remarkable repair capacity, show potential for COPD treatment. This study compares the behavior of AD-MSC from COPD and non-COPD subjects and their therapeutic effects in a mouse model. The results demonstrate that AD-MSC from both groups have similar therapeutic potential, supporting their use in treating COPD.

Coronary Artery Disease and Prognosis of Heart Failure with Reduced Ejection Fraction

Summary: The aim of this study was to determine the prognostic impact of coronary artery disease (CAD) on heart failure with reduced ejection fraction (HFrEF) mortality and readmissions. A total of 583 patients with left ventricular ejection fraction <40% were included in the study, among which 266 had CAD and 137 had idiopathic dilated cardiomyopathy (DCM). The study found that one-year mortality and readmissions were similar between patients with CAD and idiopathic DCM, but patients with idiopathic DCM were more likely to receive a heart transplant.

Summary: COVID-19 survivors often experience various sequelae, including physical impairment, which can impact their quality of life and ability to return to work. Assessing physical capacity through field or laboratory tests has proven useful for evaluating post-COVID-19 patients and those with other chronic respiratory, metabolic, cardiac, or neurological diseases. When traditional tests are not feasible, assessing physical function can serve as a good alternative, particularly for evaluating intervention effects. Choosing the appropriate test should be based on the individual characteristics of each subject.
