Summary: This study explored the different involvement patterns of the knee synovio-entheseal complex (SEC) on MRI in patients with spondyloarthritis (SPA), rheumatoid arthritis (RA), and osteoarthritis (OA). The results showed that SEC involvement patterns differed between the three groups, which can be used for differential diagnosis of different diseases and should be comprehensively evaluated in clinical practice.

Summary: Diagnostic differentiation between rheumatoid arthritis (RA) and psoriatic arthritis (PsA) remains challenging due to overlapping phenotypical presentations. High-resolution 3-T MRI imaging features, particularly extracapsular inflammatory changes, show potential for assisting in the clinical differentiation of inflammatory hand conditions. A prediction model based on strong associations between imaging features and demographic characteristics demonstrated excellent differentiation capability with an area under the curve of 98.1%.

Summary: The study aimed to assess the predictive value of tenderness in non-swollen joints for subsequent radiographic progression in rheumatoid arthritis (RA) and psoriatic arthritis (PsA), taking into consideration other potential risk factors. Clinical and sonographic examination of hand joints were performed on patients with RA and PsA. The results showed that tenderness in non-swollen joints was associated with radiographic progression in RA, while the impact was weaker in PsA. Other risk factors such as sonographic signs for synovitis and baseline radiographic damage also had an effect on subsequent structural damage in both diseases.

Summary: This study identified the association between traditional cardiovascular risk factors, diseases related factors, body composition, and adipokines with high cardiovascular risk in patients with psoriatic arthritis and non-psoriatic spondyloarthritis. The presence of sarcopenia and any traditional cardiovascular comorbidity were associated with high cardiovascular risk. Non-psoriatic spondyloarthritis patients had a lower chance of having high cardiovascular risk compared to psoriatic arthritis patients. Adipokine levels correlated with body composition but not with high cardiovascular risk. Sarcopenia was found to be an important determinant of high cardiovascular risk, possibly due to ectopic adipose tissue deposition in skeletal muscles. Physical therapy targeting sarcopenia, optimal treatment of traditional cardiovascular risk factors, and adequate disease control may help prevent atherosclerosis in spondyloarthritis.

Summary: The hand and wrist are frequently involved in rheumatic diseases, and this study aimed to identify the distinguishing radiographic characteristics of psoriatic arthritis (PsA), seropositive rheumatoid arthritis (RA), and seronegative RA, especially in the early stages. The analysis of radiographic hand findings on 180 seropositive RA patients, 154 PsA patients, and 36 seronegative RA patients revealed that symmetric distribution of lesions was more suggestive of seropositive or seronegative RA, while lesions affecting PIP joints, wrist, or styloid process of the radius were more common in patients with seropositive RA. PsA was more likely to be suggested by DIP joints' involvement and proliferative bone changes. Furthermore, there were differences in the types of bone lesions and their distribution between seropositive RA, seronegative RA, and PsA, with ulnar styloid involvement more commonly seen in seronegative RA patients.

Summary: This study aimed to evaluate the ability of low-dose dual-energy computed tomography (ld-DECT) virtual non-calcium (VNCa) images to detect bone marrow pathologies in patients with axial spondyloarthritis (axSpA). Sixty-eight patients underwent both ld-DECT and MRI for assessment. The accuracy of ld-DECT in detecting osteitis and fatty bone marrow deposition was evaluated, and correlations with MRI were calculated. The results showed that ld-DECT had limited sensitivity and specificity in detecting these pathologies, and higher radiation might be needed for more accurate analysis.

Summary: Limited evidence on long-term COVID-19 vaccine safety in patients with idiopathic inflammatory myopathies (IIMs) contributes to vaccine hesitancy. This study analyzed delayed-onset vaccine adverse events (AEs) in patients with IIMs, other systemic autoimmune and inflammatory disorders (SAIDs), and healthy controls (HCs). The findings showed that vaccination is safe in individuals with IIMs, with AEs and hospitalization rates comparable to SAIDs, and mostly limited to those with autoimmune multimorbidity and active disease. These observations can inform guidelines for identifying high-risk patients in the post-vaccination period.
