Increasing the accuracy of colorectal cancer screening

Silvia Pecere, Cristina Ciuffini, Michele Francesco Chiappetta, Lucio Petruzziello, Luigi Giovanni Papparella, Cristiano Spada, Antonio Gasbarrini, Federico Barbaro

Summary: Colorectal cancer is a significant health issue, responsible for nearly 10% of cancer-related deaths. Screening is crucial for early detection of preneoplastic lesions or early-stage cancer. This review summarizes existing screening tools, their pros and cons, and highlights the potential game-changing technologies currently being investigated.

Evaluation of tolerability and major factors affecting the adherence to probiotic therapy in patients with irritable bowel syndrome: a prospective, observational, real-life study

Lucrezia Laterza, Marco Napoli, Valentina Petito, Franco Scaldaferri, Eleonora Gaetani, Antonio Gasbarrini

Summary: This study evaluated the adherence to probiotic therapy in real-life and identified factors that influenced adherence. It found that 60% of patients took all the prescribed probiotic therapy and perceived benefits in more than 60% of cases. The main factors affecting adherence were the price of the product, mild adverse events, and poor appreciation of flavor.

Summary: This retrospective study evaluated the feasibility and safety of endoscopic ultrasound-guided radiofrequency ablation (EUS-RFA) for unresectable pancreatic insulinomas. The study found that EUS-RFA was technically successful and achieved clinical remission in all patients, with no major complications observed. Complete radiologic response and symptom remission were assessed in all patients during follow-up.

Role of Akkermansia in Human Diseases: From Causation to Therapeutic Properties

Antonio Pellegrino, Gaetano Coppola, Francesco Santopaolo, Antonio Gasbarrini, Francesca Romana Ponziani

Summary: The gut microbiota plays a critical role in host metabolism and immune response regulation, and its impairment is associated with various gastrointestinal and extraintestinal diseases. A. muciniphila is proven to be important in maintaining intestinal barrier integrity, modulating the host immune response, and improving metabolic pathways, making it a key factor in the pathogenesis of several human diseases. Further research is needed to explore its mechanisms of action and properties in order to develop a more integrated and personalized therapeutic approach based on the knowledge of gut microbiota.

Effect of COVID-19 Vaccination on the In-Hospital Prognosis of Patients Admitted during Delta and Omicron Waves in Italy

Rossella Cianci, Laura Franza, Giulia Pignataro, Maria Grazia Massaro, Pierluigi Rio, Antonio Tota, Francesca Ocarino, Marta Sacco Fernandez, Francesco Franceschi, Antonio Gasbarrini, Giovanni Gambassi, Marcello Candelli

Summary: The impact of SARS-CoV-2 vaccination on the prognosis of infected patients was evaluated in this study. Results showed that vaccination with at least two doses reduced the need for ICU admission, and even one shot of the vaccine significantly reduced 30-day mortality. All-cause mortality related to SARS-CoV-2 infection has declined from the first wave to subsequent waves.

Correlation between a New Point-Shear Wave Elastography Device (X+pSWE) with Liver Histology and 2D-SWE (SSI) for Liver Stiffness Quantification in Chronic Liver Disease

Matteo Garcovich, Mattia Paratore, Laura Riccardi, Maria Assunta Zocco, Maria Elena Ainora, Geltrude Mingrone, Antonio Gasbarrini, Maurizio Pompili

Summary: This study aimed to investigate the feasibility, correlation, and accuracy of a new point shear-wave elastography device (X+pSWE) in patients with chronic liver disease. The results showed a good correlation between X+pSWE and 2D-SWE with SSI, with X+pSWE values being slightly lower. X+pSWE had high AUROC values for staging fibrosis, and correctly identified most patients according to histologic classification.

Gut Microbiota and Infectious Complications in Advanced Chronic Liver Disease: Focus on Spontaneous Bacterial Peritonitis

Valeria Maccauro, Carlo Airola, Francesco Santopaolo, Antonio Gasbarrini, Francesca Romana Ponziani, Maurizio Pompili

Summary: Liver cirrhosis is a chronic disease that often leads to complications such as variceal bleeding, hepatic encephalopathy, ascites, and jaundice, with increased mortality. Infections, particularly spontaneous bacterial peritonitis (SBP), are common in cirrhotic patients due to impaired immunity and altered intestinal microbiota. SBP is primarily caused by Gram-negative bacteria translocating through the defective intestinal barrier. The first-line treatment is antibiotic therapy, but it may worsen dysbiosis. Future therapies aim to selectively modulate the gut microbiota or reduce intestinal permeability.

Summary: This study aimed to develop and validate predictive mortality models using data from hospitalized COVID-19 patients. Increased FIB-4 index values were found to be associated with poor clinical outcomes. The study utilized artificial intelligence and big data processing to retrieve data and developed a multivariable logistic regression model. Among 4936 patients, 15.4% died.

ANCOC Score to Predict Mortality in Different SARS-CoV-2 Variants and Vaccination Status

Marcello Candelli, Marta Sacco Fernandez, Giulia Pignataro, Giuseppe Merra, Gianluca Tullo, Alessandra Bronzino, Andrea Piccioni, Veronica Ojetti, Antonio Gasbarrini, Francesco Franceschi

Summary: Our study aimed to validate the ANCOC score and assess its ability to predict 60-day mortality risk regardless of vaccination status and viral variant. The results suggest that the ANCOC score is able to accurately predict 60-day mortality risk independent of viral variant and vaccination status.

The Relationship between Post-COVID Syndrome and the Burden of Comorbidities Assessed Using the Charlson Comorbidity Index

Lorenzo Falsetti, Vincenzo Zaccone, Luca Santoro, Silvia Santini, Emanuele Guerrieri, Luca Giuliani, Giovanna Viticchi, Serena Cataldi, Antonio Gasbarrini, Francesco Landi, Angelo Santoliquido, Gianluca Moroncini

Summary: Patients with post-COVID-19 syndrome exhibit greater clinical complexity and a higher burden of comorbidities, as indicated by a higher CCI. Additionally, a higher CCI is associated with an increased risk of post-COVID-19 syndrome, with the presence of 1 or 4 or more CCI points associated with a 3-fold and 6-fold increased risk, respectively.

Non-hypervascular hepatobiliary phase hypointense lesions detected in patients with hepatocellular carcinoma: a post hoc analysis of SORAMIC trial to identify risk factors for progression

Fabritius, Christian Loewe, Otto van Delden, Vincent Vandecaveye, Bernhard Gebauer, Thomas Berg, Christian Sengel, Irene Bargellini, Roberto Iezzi, Alberto Benito, Maciej Pech, Antonio Gasbarrini, Bruno Sangro, Peter Malfertheiner, Jens Ricke, Max Seidensticker

Summary: This study aims to identify clinical and imaging parameters associated with the progression of non-hypervascular hepatobiliary phase hypointense lesions during follow-up after treatment for hepatocellular carcinoma. The results suggest that T2 hyperintensity, diffusion restriction, cirrhosis, and higher ECOG-PS are independent risk factors for the progression of these lesions.
