Background: Although several strategies to predict the development of diabetes have been developed the question is whether better scores can be developed without sacrificing simplicity. Methods: Data on 3242 participants of Tehran Lipid and Glucose Study aged epsilon 20 years, without diabetes at the baseline that completed a similar to 6-year follow-up were used to develop prediction models by running a series of logistic regression model. A simple score system was then developed based on the most important variables selected with forward stepwise approach. Results: During follow-up, 231 individuals developed diabetes. The area under the receiver operating characteristic curve for the score system based on the model including systolic blood pressure (SBP), family history of diabetes, waist-to-height ratio (WHtR), triglyceride-to-high-density lipoprotein cholesterol ratio (TG/HDL-C) epsilon 3.5 and fasting plasma glucose (FPG) levels epsilon 5mmol l(-1) was 0.83 (95% CI 0.80-0.86); the model discriminated subjects with substantial risk for diabetes, appreciably better than 2-h post-challenge plasma glucose (2h-PCPG) alone (0.78; 95% CI 0.75-0.82) (P < 0.001). Scoring epsilon 25 yielded a positive likelihood ratio of 3.27. FPG levels even in the presence of 2h -PCPG predicted incident diabetes. Conclusion: We presented a simple model based on SBP, family history of diabetes, WHtR, TG/HDL-C and FPG; concluding that this approach is superior to relying exclusively on the 2h-PCPG for identifying individuals at high risk for developing diabetes in a Middle Eastern adult population.

Effects of isolated maternal hypothyroxinemia on adverse pregnancy outcomes

Sima Nazarpour, Fahimeh Ramezani Tehrani, Maryam Rahmati, Mina Amiri, Fereidoun Azizi

Summary: The study found that women with maternal hypothyroidism identified in early pregnancy are more likely to have adverse pregnancy outcomes, including preterm premature rupture of membranes (PPROM) and low birth weight (LBW), compared to women with euthyroid status. Additionally, neonatal head circumference and weight were lower in the hypothyroid group than in the euthyroid group.

Summary: This study evaluated the familial aggregation, heritability, and inheritance mode of type 2 diabetes (T2D) in the Iranian population. The findings showed that T2D has a higher heritability with a polygenic mode in the Iranian population compared to the global average. It was also found that T2D is equally transmitted into siblings, with parental affect being the leading risk factor.

Summary: The study aimed to assess the association between dietary choline and betaine intake and cardiovascular disease (CVD). The results showed that overall intake of choline and betaine was not associated with CVD risk, while higher intake of free choline was associated with a lower risk of CVD.

Summary: This study aimed to investigate the association of the Empirical Dietary Index for Hyperinsulinemia (EDIH) and Empirical Lifestyle Index for Hyperinsulinemia (ELIH) with the risk of obesity phenotypes among Iranian adults. The findings revealed that higher ELIH scores were associated with greater risks for MUO and MHO incidence, while higher EDIH scores were related to an increased risk of MUO but not MHO. This suggests that a high insulinemic potential of diet and lifestyle may lead to an increase in the simultaneous occurrence of obesity with metabolic disorders in Iranian adults.

Sex differences in the association between diabetes and hypertension and the risk of stroke: cohort of the Tehran Lipid and Glucose Study

Summary: This study aimed to evaluate the sex differences in the association between blood pressure and glucose intolerance status and the risk of stroke among Iranian adults. The results showed that the association between increased blood glucose levels and stroke risk was stronger in men than women. Each 10mmHg increase in diastolic blood pressure was also found to have a stronger association with stroke risk in men compared to women. Additionally, hypertension and type 2 diabetes were associated with a higher risk of stroke in men than women.

Summary: Hypothyroidism is a common condition of insufficient thyroid hormone production or metabolism, which can be life-threatening if untreated. It has serious adverse health effects on multiple organ systems, with the cardiovascular system being the most studied target. The main treatment approach is the use of levothyroxine, but there are still persistent symptoms that cannot be relieved.
