States of serum leptin and insulin in children with epilepsy: Risk predictors of weight gain

Background and objectives: Weight gain is an adverse metabolic effect in some children with epilepsy. The studies done to detect the effect of antiepileptic drugs and weight homeostatic hormones, insulin and leptin, were limited and controversial. Materials and methods: we evaluated the serum leptin and insulin as predictors of weight gain in children receiving long-term treatment with valproate (VPA), carbamazepine (CBZ), lamotrigine (LTG). This study included 90 patients (treated: 70; untreated: 20). Serum lipid profile, insulin and leptin were measured. Results: BMI, serum leptin and insulin were significantly elevated in VPA compared with controls, untreated patients and those treated with CBZ, LTG and combined therapy with LTG. Girls on VPA had higher BMI and leptin levels than boys. With VPA, serum insulin was correlated with BMI (r = 0.625, p < 0.01), leptin (r = 0.823, p < 0.001), treatment duration (r = 0.775, p < 0.01) and VPA dose (r = 0.975, p < 0.0001). Serum leptin was correlated with age (r = 0.980, p < 0.0001), BMI (r = 0.704, p < 0.01), serum insulin (r = 0.823, p < 0.001), LDL-c (r = 0.630, p < 0.01), HDL-c (r = -0.880, p < 0.001), treatment duration (r = 0.770, p < 0.01) and VPA dose (r = 0.970, p < 0.001). BMI is correlated with serum insulin, leptin, LDL-c (r = 0.835, p < 0.001) and HDL-c (r = -0.955, p < 0.0001). Conclusion: Hyperinsulinemia and hyperleptinemia are common with VPA and marked among epileptic children who gained weight suggesting states of insulin and leptin resistances. These alterations were not demonstrated with CBZ or LTG. The relationship between VPA, leptin and weight seems to be gender specific. Serum leptin may serve as a sensitive parameter for weight gain and reduction with intervention programs during follow-up of girls with epilepsy. (C) 2008 European Paediatric Neurology Society. Summary: This study identified families with neurological diseases caused by HPDL variants, showing various phenotypes ranging from juvenile-onset pure hereditary spastic paraplegia to infantile-onset spastic tetraplegia. Experimental evidence suggested a role of HPDL in the nervous system and supported its link to neurological disease.

Restless leg syndrome in patients with chronic kidney disease: a hospital-based study from Upper Egypt

Summary: This study aimed to determine the prevalence of restless leg syndrome (RLS) due to chronic kidney disease (CKD) and its predictors. A cross-sectional study was conducted on 520 patients, and RLS diagnosis was done according to International RLS Study Group criteria. The prevalence of RLS with CKD was found to be 22.31%, with higher rates in end-stage kidney disease (ESKD) patients. Factors such as ESKD, inadequate dialysis, hyperparathyroidism, and peripheral neuropathy were found to be independently associated with RLS.

Idiopathic steno-occlusive disease with bilateral internal carotid artery occlusion: A Case Report

Summary: This study presents the case of a 4-year-old girl with acute stroke and recurrent seizures, who was diagnosed with Moyamoya disease. The patient exhibited bilateral fronto-parietal ischemic infarctions and occlusion of the internal carotid arteries. During the 3-year follow-up, the patient's condition improved and stabilization was observed. Revascularization surgery was postponed.

Tinnitus could be the earliest and dominant somatic symptom induced by life stressors and psychological vulnerabilities. Therefore, multidisciplinary consultation (psychologists, psychiatrists, and neurologists) is important to acknowledge among the audiologists and otolaryngologists who primarily consult patients.

Summary: Research shows that loss of the Gnb5 gene in mice results in abnormal heart function, characterized by increased baseline heart rate and significant bradycardia. Knocking out the Gnb5 gene affects gene expression related to cardiac muscle contraction, leading to an increase in arrhythmia frequency. The study also expanded the cohort of IDDCA syndrome patients, indicating the important role of Gnb5 signaling in autonomic control of the heart.

Respiratory function in LAMA2-related muscular dystrophy and SELENON-related congenital myopathy, a 1.5-year natural history study

Summary: The majority of LAMA2-MD and all SELENON-RM patients had respiratory impairment. SELENON-RM patients showed lower respiratory function which was progressive, more prevalent mechanical ventilation, and more severe diaphragm atrophy and dysfunction than LAMA2-MD patients. Spirometry (FVC%, dVC) and respiratory muscle strength tests (SNIP) are useful in clinical care and as outcome measure in clinical trials.

An online survey among general pediatricians on melatonin use in children with chronic insomnia

Summary: This study aimed to describe the use of melatonin by Italian pediatricians in healthy children with chronic insomnia. The results showed that a high percentage of pediatricians prescribed melatonin, especially in children aged 1-2 years. The most common dosage was 1 mg/day and it was usually recommended to be taken 30 minutes before bedtime. Melatonin was often combined with sleep hygiene and was found to be effective in reducing difficulties falling asleep.

Yield of exome sequencing in patients with developmental and epileptic encephalopathies and inconclusive targeted gene panel

Summary: In this study, whole exome sequencing (WES) was performed to identify causal variants for developmental and epileptic encephalopathies (DEEs) in patients whose genetic diagnosis was not determined by gene panel testing. The results showed that WES can successfully identify disease-causing variants, even after inconclusive gene panel testing. Detailed clinical evaluations and phenotype-genotype correlation studies were conducted to better understand the rare subtypes of DEEs.
