Significance of microbleeds in patients with transient ischaemic attack

Background and purpose: The aim of this study was to determine the prognostic significance of microbleeds in TIA-patients. In patients with a transient ischaemic attack (TIA), the prognostic value of microbleeds is unknown. Methods: In 176 consecutive TIA patients, the number, size, and location of microbleeds with or without acute ischaemic lesions were assessed. We compared microbleed- positive and microbleed- negative patients with regard to the end-point stroke within 3 months. Results: Four of the seven patients with subsequent stroke had microbleeds. Microbleed-positive patients had a higher risk for stroke [ odds ratios (OR) 8.91, 95% CI 1.87-42.51, P < 0.01] than those without microbleeds. Microbleed-positive patients with accompanying acute ischaemic lesions had a higher stroke risk than those with neither an acute ischaemia nor a microbleed (OR 6.20, 95% CI 1.10-35.12; P = 0.04). Conclusion: Microbleeds alone or in combination with acute ischaemic lesions may increase the risk for subsequent ischaemic stroke after TIA within 3 months.

Summary: The purpose of this review is to discuss the potential clinical implementation of promising blood biomarkers in order to achieve a more individualized treatment allocation and lower recurrence rates of ischemic stroke. Recent findings suggest that blood-based biomarkers have the potential to improve the identification of underlying causes as well as risk stratification of patients, leading to a refinement of decision algorithms for treatment allocation in secondary ischemic stroke prevention.

Summary: This study aimed to confirm the cardiac specificity of cTnT in patients with various skeletal muscle disorders (SMDs). The results showed that elevations in cTnT concentrations in patients with active chronic SMDs are common and not attributable to cardiac disease, while this phenomenon was not observed for cTnI. The re-expression of cTnT in skeletal muscle may partially explain this.

Summary: Patients with atrial fibrillation who suffer a stroke despite being on nonvitamin K antagonist oral anticoagulant therapy face a high risk of recurrent ischemic stroke and bleeding. Further research is needed to improve secondary prevention by investigating the mechanisms of recurrent ischemic stroke and bleeding.

Summary: Knowledge about different etiologies of non-traumatic intracerebral hemorrhage (ICH) and their outcomes is scarce. Hypertension is the leading etiology of ICH, but other etiologies are also common. Around one-third of patients achieve functional independence at 3 months, but the mortality rate is high. The risk of ischemic stroke within 3 months of ICH is higher than the risk of recurrent hemorrhage, except for patients with cerebral amyloid angiopathy (CAA).

Summary: Both once-daily and twice-daily direct oral anticoagulants show lower risk for recurrent ischemic stroke, major bleeding, and all-cause death compared to vitamin K antagonists in atrial fibrillation patients with recent stroke. There is no clear evidence that one dosing regimen of direct oral anticoagulants is more advantageous than the other.

Summary: In this study, the type of intracerebral hemorrhage (ICH) did not show significant differences in terms of hematoma expansion (HE) or favorable outcome. Tranexamic acid was found to significantly reduce the risk of HE, regardless of the type of ICH, but was not associated with a favorable outcome. Patients with lobar CAA-ICH did not independently have an increased risk of HE, suggesting different dynamics compared to other types of ICH.

Summary: This study aimed to determine the prognostic value of plaque surface echogenicity alone or combined with degree of stenosis for ischemic stroke or transitory ischemic attack (TIA). The results showed that a high proportion of low echogenicity on the surface of the plaque was strongly predictive of stroke or TIA occurrence, regardless of the degree of stenosis. These findings can help clinicians assess the risk and take preventive measures for adverse events in patients with carotid stenosis.

Summary: This study examined the safety and efficacy outcomes of mechanical thrombectomy (MT) in patients with anterior circulation large vessel occlusions (LVO) who fulfilled or did not fulfill the DEFUSE-3 and DAWN trial inclusion criteria. The results showed that there were no significant differences in outcomes between patients who fulfilled the criteria and those who did not, suggesting the need for randomized trials with broader inclusion criteria for MT.

Summary: This study evaluated the efficacy and safety of mechanical thrombectomy (MT) in adult stroke patients with anterior circulation large vessel occlusion presenting in the late time window not fulfilling the DEFUSE-3 and DAWN inclusion criteria. The study found that MT was associated with higher odds of favorable functional outcomes without an increased risk of symptomatic intracranial hemorrhage. These findings support the use of more permissive inclusion criteria for MT in the late time window.

Summary: This study retrospectively included data from 2715 patients with acute stroke and found that 5.9% (161/2715) of patients had pulmonary lesions on CTA. Approximately one-third of these lesions (58/161) were suspicious for malignancy. Further investigation confirmed primary or secondary pulmonary malignancy in two-thirds of patients (12/16) and they received oncologic therapy.
