Arterial stiffness, metabolic syndrome and inflammation amongst Asian ischaemic stroke patients

Background and purpose: Arterial stiffness and metabolic syndrome (MetS) are risk factors for ischaemic stroke. We studied the association of arterial stiffness, measured by carotid-femoral pulse wave velocity (PWV) and MetS amongst ischaemic stroke patients. We also investigated the role of inflammation measured by serum erythrocyte sedimentation rate (ESR) in the metabolic syndrome-arterial stiffness relationship. Methods: Amongst the 229 prospectively recruited acute ischaemic stroke patients, we measured carotid-femoral PWV using applanation tonometry and the inflammatory marker serum ESR. Results: Carotid-femoral PWV was significantly higher amongst patients with MetS (P = 0.002), increased waist circumference (P = 0.010), raised blood pressure (P < 0.001) and abnormal glycemia (P = 0.002); and increased with the number of MetS components (P = 0.002). In a sub-group of 199 patients, carotid-femoral PWV was significantly correlated with serum ESR (P < 0.001). In multivariate regression analysis including serum ESR and MetS as variables, carotid-femoral PWV was independently associated with higher ESR (P = 0.002) but not with MetS (P = 0.139). Conclusions: Arterial stiffness is significantly associated with MetS amongst ischaemic stroke patients, and inflammation appears to be involved in this relationship.

Retinal parameters, cortical cerebral microinfarcts, and their interaction with cognitive impairment

Summary: This study found that retinal vascular and neuronal changes are associated with cortical CMIs, which affect cognition. Larger venular caliber, increased venular fractal dimension and tortuosity, and thinner GC-IPL are associated with CMIs. Among individuals in the highest tertile of retinal parameters, there is an interaction between venular tortuosity and GC-IPL with CMIs and cognitive impairment.

Summary: Perivascular space (PVS) burden is a magnetic resonance imaging marker of cerebral small vessel disease, and genome-wide association studies have identified 24 significant PVS risk loci. These loci are associated with white matter PVS in young adults and are enriched in genes related to leukodystrophy and fetal brain endothelial cells. Additionally, Mendelian randomization analysis supports the link between high blood pressure and PVS, as well as the association between PVS and stroke.

Ischemic Stroke and Savings in Time to Achieve Functional Recovery: Experience from NeuroAiD

Summary: Despite advances in revascularisation interventions for acute ischemic stroke, many patients continue to experience disability. A study analyzed data from a randomized, double-blind, placebo-controlled trial of a neuro-repair treatment (NeuroAiD/MLC601) and found that patients receiving a 3-month course of MLC601 had a significantly shorter time to functional recovery compared to those receiving placebo.

Summary: Cerebral small vessel disease (SVD) is a common age-related condition that can cause stroke, cognitive decline, neurobehavioral symptoms, and functional impairment. STRIVE-1 classified and standardized the various visible features of SVD on structural MRI, but new information and imaging techniques have emerged since then. Quantitative imaging biomarkers and machine learning methods offer a more comprehensive understanding of the impact of SVD on the brain and can serve as intermediary outcomes in clinical trials and routine practice.

Summary: This study found that age-related protein damage can be mitigated by anti-isoDGR immunotherapy. The accumulation of isoDGR proteins is associated with increased expression of inflammatory cytokines and can be induced by injection of isoDGR-modified plasma proteins or synthetic peptides. However, treatment with anti-isoDGR monoclonal antibodies can significantly reduce the levels of isoDGR proteins in tissues, decrease the concentration of inflammatory cytokines in the plasma, improve cognitive/coordination metrics, and extend the lifespan of mice.
