Cost-effectiveness of highly purified omega-3 polyunsaturated fatty acid ethyl esters in the treatment of chronic heart failure: results of Markov modelling in a UK setting

Aims A recent randomized placebo-controlled clinical trial has reported reductions in mortality and hospitalizations in patients with chronic heart failure (CHF) who were prescribed highly purified omega-3 polyunsaturated fatty acid ethyl esters (n-3 PUFA). This study aimed at evaluating the cost and benefits associated with their use in the treatment of CHF in a UK setting. Methods and results Results from a recent clinical trial were used to develop a Markov model to project clinical outcomes while capturing relevant costs and patient quality of life. The model captured outcomes over a lifetime horizon from a UK National Health Service perspective, with direct costs accounted in 2009 GBP ((sic)) and discounted at 3.5% together with clinical benefits. Results are presented in terms of life expectancy, quality-adjusted life expectancy, direct costs, and incremental cost-effectiveness ratios. In addition to standard therapy, n-3 PUFA vs. placebo increased lifetime direct costs by (sic) 993 (approximate to E1150), with additional quality-adjusted life expectancy of 0.079 quality-adjusted life years (QALYs), and mean lifetime costs of (sic)12 636 (approximate to(sic)E14 600) per QALY gained. Probabilistic sensitivity analyses suggested a 60% likelihood of n-3 PUFA being regarded as cost-effective versus placebo at a willingness-to-pay threshold of (sic)30 000 (approximate to E34 600) per QALY gained. Conclusions By currently accepted standards of value for money in the UK; the addition of n-3 PUFA to optimal medical therapy for patients with heart failure is likely to be cost-effective.

Summary: In this study, we compared changes in inferior vena cava diameter (IVCD) with venous pressure during ultrafiltration in patients with cardiac dysfunction. The results showed that IVCD tracked changes in volume status and hemoconcentration better than venous pressure, suggesting that IVCD is a more accurate indicator for monitoring the management of heart failure patients.

Risk prediction of mortality for patients with heart failure in England: observational study in primary care

Summary: This study aimed to explore risk prediction models for heart failure based on general practitioners' electronic health records and compare the data from two different cohorts. The results showed that the survival rates were similar in different years, but the later population was older, frailer, and had more comorbidities. The study also found consistent predictors including age, sex, blood pressure, body mass index, GP visits before diagnosis, and some comorbidities.

Summary: As part of the REWOLUTION HF programme, two international surveys were conducted to assess the educational needs of healthcare professionals (HCPs) and patients' perspectives on the care of heart failure (HF). The surveys revealed that most HCPs prioritize rapid initiation of guideline-mandated medications and discuss treatment goals with patients. There was good agreement between HCPs and patients on key treatment goals, although HCPs placed greater emphasis on reducing hospitalizations. Patients expressed a desire for more information about HF and its consequences, prognosis, and treatments.

Benefits and challenges of telemedicine for heart failure consultations: a qualitative study

Summary: Prior to the Covid-19 pandemic, heart failure (HF) disease management programmes were predominantly in-person, but the pandemic necessitated a shift to teleconsultations. Telemedicine consultations were found to be more convenient for patients, saved them time, and were generally acceptable to clinicians. However, it also brought changes to workflows, consultation dynamics, and clinical assessment methods.

Their use can lead to improvements in healthcare quality and informed clinical decisions. This article provides an overview of PROs for cardiovascular populations, including disease-specific and domain-specific measures, as well as international standards and guidelines for their selection. Collaboration and evaluation of PROs are needed to guide decision-making in regulatory contexts and to monitor care quality and resource allocation. New sources, such as wearable devices and electronic health records, offer opportunities for collecting PROs.

Serum brain natriuretic peptide levels, left ventricular ejection fraction, and myocardial inflammation were identified as independent predictors of outcomes.

The cost-effectiveness of NT-proBNP for assessment of suspected acute heart failure in the emergency department

Summary: When relying on clinical assessment alone, an estimated 22% of acute heart failure (AHF) patients are missed. Therefore, clinical guidelines recommend the use of NT-proBNP for AHF diagnosis. However, there has been poor uptake of NT-proBNP testing due to concerns over low specificity. This study proposes the use of age-specific thresholds to improve the specificity of NT-proBNP testing.
