Summary: The effectiveness and safety of GH replacement treatment in adult patients with LCH and GHD were analyzed. The results showed that after 1 year of GH treatment, metabolic variables and quality of life improved in the patients, with no new safety signals.

Summary: The pattern of insulin sensitivity in patients with GH deficiency (GHD) is contradictory, and this study assessed the insulin sensitivity and body composition in adults with hypopituitarism. It was found that patients with childhood-onset GHD showed higher insulin sensitivity compared to controls, while patients with adulthood-onset GHD had similar insulin sensitivity as the control group.

Summary: This study investigated the current practices of AGHD management throughout Europe, revealing that despite guideline recommendations, GH replacement for AGHD is still unavailable or not fully reimbursed in all European countries. Knowledge among non-endocrine professionals and healthcare administrators regarding AGHD is low, highlighting the need for improvement in postgraduate AGHD curriculum training to optimize care for adults with GHD.

Summary: The cardiovascular risk profile in patients with craniopharyngioma treated with growth hormone replacement therapy is worse compared to patients with NFPA, with higher BMI and lower HDL observed in craniopharyngioma patients at baseline. During follow-up, BMI and diastolic blood pressure worsened in the craniopharyngioma group, while lipid profile improved similarly in both groups over time. No significant differences were found in the occurrence of diabetes mellitus, cerebrovascular accidents, cardiovascular disease, or overall mortality between the two groups.

Summary: In order to better understand the causes of hypophosphatemia in children, a study was conducted on 29,279 phosphate tests from 21,398 patients over an 18 year period. It was found that endocrinopathies, tumors, and vitamin D deficiency were commonly associated with mild hypophosphatemia, but the cause was unclear in many cases. Among patients with endocrinopathies, those with diabetes mellitus had lower mean serum phosphate levels compared to other conditions. It was also observed that hypophosphatemia is often neglected and inadequately evaluated by pediatricians, highlighting the need for more education and awareness about this condition.

Summary: A retrospective study evaluated testicular growth and pubertal onset in deficient children treated with recombinant human growth hormone (rhGH), suggesting a potential role of GH in testicular growth and pubertal onset. Further studies are needed to confirm these findings.

Summary: Growth hormone and insulin-like growth factor-1 have effects on different organs and metabolic processes. Adult growth hormone deficiency (AGHD) is associated with nonalcoholic fatty liver disease (NAFLD), which is often underdiagnosed or untreated. NAFLD usually progresses rapidly after AGHD diagnosis and may require liver transplantation. Increased awareness of the association between AGHD and NAFLD is needed to facilitate early diagnosis.

Summary: There is variability in the characteristics, surgical approach, complications, and outcomes of children with craniopharyngioma in Italy. Postsurgery complications are not rare and almost all patients develop pituitary deficiency.

Summary: PWS is a complex disorder caused by the failure of paternal allele expression on chromosome 15. This study found that long-term GH treatment can improve body fat distribution and body composition in PWS patients. However, the increase in glucose values during GH therapy should be considered, and continuous surveillance of glucose metabolism is mandatory during long-term GH therapy, especially in obese patients.

Summary: This study proposes a quantitative estimation of GHD probability prior to stimulation tests based on IGF-I values and the presence/absence of other pituitary deficits. The risk class stratification provides a simple tool for a Bayesian interpretation of stimulation test results, potentially reducing the risk of wrong GHD diagnosis.

Summary: This study aimed to investigate the effects of physiological dose growth hormone replacement therapy on bone mineral density, body composition, and quality of life in adults with growth hormone deficiency. After 18 months of treatment, lumbar spine bone density increased, while total hip bone density remained stable; the treatment also significantly increased lean body mass, decreased body fat ratio and visceral adipose tissue, and improved quality of life.

Summary: Vitamin-responsive neurogenetic diseases are rare conditions that are potentially underdiagnosed in adults and may show a dramatic response to treatment if detected early. This review outlines the key features of adult-onset forms and provides simple key messages to aid in the timely diagnosis and treatment of these conditions.

Summary: This study found that growth hormone (GH) status plays an important role in regulating body composition and insulin sensitivity. Excess GH reduces total fat mass and intrahepatic lipid content, leading to insulin resistance. Therefore, GH-induced insulin resistance is unrelated to hepatic lipid accumulation.

Summary: This study aimed to assess the prevalence and characteristics of misdiagnosed adult-onset type 1 diabetes mellitus in Thai people by random plasma C-peptide testing. It found that approximately one-tenth of adult T1DM patients were misdiagnosed, and random plasma C-peptide testing was superior to the measurement of pancreatic autoantibodies. The study highlights the need to increase accuracy in the diagnosis of T1DM patients by reassessing endogenous insulin production with the measurement of random plasma C-peptide levels.
