Effects of high-dose octreotide LAR on glucose metabolism in patients with acromegaly inadequately controlled by conventional somatostatin analog therapy

Objective: In this study, the effect of high-dose octreotide LAR on glucose metabolism in patients with acromegaly was investigated. Design: A post-hoc analysis of a clinical trial enrolling 26 patients with acromegaly not controlled by standard maximal somatostatin analog (SSAs) dose and randomized to receive high-dose (60 mg/28 days) or high-frequency (30 mg/21 days) octreotide i.m. injection (octreotide LAR) for 6 months. Methods: Glucose metabolic status was defined as worsened when a progression from normoglycemia to impaired fasting glucose (IFG) or from IFG to diabetes occurred or when an increase of HbAlc by at least 0.5% was demonstrated. An improvement of glucose metabolism was defined in the presence of a regression from IFG to normoglycemia and/or when HbAlc decreased by at least 0.5%. Results: Glucose metabolic status remained unchanged in a majority of patients (16/26 patients, 65.3%), worsened in six patients, and improved in four patients. Pre-existing metabolic status did not predict worsening of glucose metabolism, which, conversely, was significantly related to persistent biochemical activity of the disease. In fact, patients with worsened glucose metabolism exhibited a less frequent decrease in serum GH and IGF1 levels, compared with patients with improved or unchanged glucose metabolism (2/6 vs 18/20; P=0.01). Conclusion: An increase in octreotide LAR dose or frequency did not impact on glucose metabolism in most patients. Worsening of glucose metabolic status occurred in close relation with persistently uncontrolled acromegaly.

Summary: Hypogonadism is common in male patients with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Low testosterone (T) levels are associated with unfavorable outcomes of COVID-19, and further prospective studies are needed to evaluate the long-term outcomes of hypogonadism related to COVID-19 and the clinical impact of T replacement during and after acute illness.

The results showed that FRAX and BMD may be useful for predicting vertebral fractures in patients undergoing HDTs.

The results show that 40% of PES patients have hypopituitarism, but hormonal deterioration is relatively low (3%). This suggests that patients need to be carefully evaluated at diagnosis, even if PES is incidentally discovered.

Summary: This study investigated the relationship between serum testosterone levels and outcomes of COVID-19 in a large cohort of female patients. The results showed that high testosterone levels were associated with higher inflammatory markers and worse course of COVID-19, although this association disappeared after correcting for inflammatory parameters.

Summary: This study aimed to assess the differences in oxidative stress markers between omnivorous and semi-vegetarian dietary regimes. The findings indicate that semi-vegetarians have higher levels of antioxidant parameters (GPx and TRxR) and lower levels of AGE and AOPP markers compared to omnivorous individuals, particularly in females, different age groups, those with normal weight, and those not affected by Hashimoto's thyroiditis.

Looking at the spine in controlled Acromegaly

Summary: Skeletal fragility and vertebral fractures are common complications of growth hormone hypersecretion in acromegalic subjects. The coexistence of spine arthropathy further negatively impacts the quality of life for these patients. Managing these complications can be challenging even after controlling the disease biochemically. This article provides an analysis of the pathophysiological links between vertebral fractures and spine arthropathy in active and controlled acromegaly, as well as the diagnostic and therapeutic aspects of managing acromegalic osteo-arthropathy.
