Summary: This study aimed to predict the response of acromegaly patients to somatostatin receptor ligands (SRL) using mathematical modeling and clinical information. Accuracy of patient stratification was significantly improved when the cohort was fragmented based on relevant clinical characteristics. The proposed stratification method based on tumor characteristics, sex, age, and multiple biomarkers achieved accuracies ranging from 71% to 95%. The use of data mining offers a promising approach for personalized medicine in acromegaly.

Summary: Assessing the behavior of pancreatic neuroendocrine tumors (PNETs) remains a daily challenge for physicians. The definition of aggressiveness is crucial in determining the appropriate management strategy, ranging from observation to surgery. EUS has emerged as a valuable tool in diagnosing PNETs and has experienced rapid advancements in recent years, particularly in the assessment of aggressiveness using contrast-enhanced techniques. This review highlights the latest experiences and opportunities in evaluating PNET behavior with contact-enhanced EUS and contrast-enhanced harmonic EUS, with a focus on future applications and possibilities.

The gold standard test for assessing the HPA axis is the insulin tolerance test, but the ACTH stimulation test is safer and well-tolerated, although its diagnostic accuracy varies.

Summary: Skeletal fragility and vertebral fractures are common complications of growth hormone hypersecretion in acromegalic subjects. The coexistence of spine arthropathy further negatively impacts the quality of life for these patients. Managing these complications can be challenging even after controlling the disease biochemically. This article provides an analysis of the pathophysiological links between vertebral fractures and spine arthropathy in active and controlled acromegaly, as well as the diagnostic and therapeutic aspects of managing acromegalic osteo-arthropathy.

Summary: This study evaluated the hematological alterations in benign adrenocortical tumors with different degrees of cortisol excess. It found that the degree of cortisol excess was correlated with an increase in leukocytes, neutrophils, and monocytes. Additionally, the neutrophil-to-lymphocyte ratio, lymphocyte-to-monocyte ratio, and systemic immune-inflammation index were altered in patients with mild autonomous cortisol secretion (MACS) and adrenal Cushing's syndrome.
