Summary: This case series reports 9 surgical patients with CDI who did not experience diarrhea prior to diagnosis. While all patients tested positive for C. difficile toxin, some presented with minimal bowel movements or constipation instead. These findings highlight the importance of considering CDI as a potential diagnosis in patients with atypical gastrointestinal symptoms even in the absence of diarrhea.

Summary: This study investigated the prevalence and subtype characteristics of Blastocystis in patients with suspicion of C. difficile infection in Singapore. The study found a Blastocystis positivity rate of 10.1%, with the predominant subtype being ST7. Co-infection of Blastocystis and C. difficile was observed in five patients, with subtypes ST7, ST1, and ST3 identified.

Summary: The risk factors of Clostridium difficile-associated diarrhea (CDAD) vary among hospitalized patients with different lengths of hospital stay. Malignancy, prior cephalosporin, and proton pump inhibitor therapy are independent risk factors for CDAD. Intervention strategies for preventing CDAD may need to be tailored based on the duration of hospital stay.

Summary: Severe, complicated Clostridium difficile colitis is rare in pediatric patients and little is known about its medical and surgical management in this population. A case report of a 15-year-old female patient with severe, complicated C difficile colitis successfully treated with diverting loop ileostomy and colonic lavage is presented. This approach, instead of subtotal colectomy and end ileostomy, should be considered as an alternative for pediatric patients with severe, complicated C difficile colitis.

Summary: This study aimed to identify predictors of hospital-onset Clostridioides difficile infection (CDI) in pediatric patients with antibiotic-associated diarrhea (AAD) and develop a predictive scoring system for at-risk patients. The scoring system, based on identified predictors including abdominal pain, prolonged hospitalization, prolonged antibiotic use, and specific antibiotics received, can be easily used by clinicians to identify AAD patients likely to have CDI.

Summary: Clostridioides difficile (C. difficile) infection is a global threat to many healthcare settings. Excessive use of antimicrobials, lack of optimal antibiotic policies, and suboptimal infection control practices have fueled the development of this health issue. Prudent use of antimicrobials and simple infection control measures can significantly reduce infection rates.

Summary: A case-control study compared hospitalized patients with and without Clostridioides difficile infection (CDI) to determine the association between healthcare exposures prior to hospitalization and risk for hospital-onset CDI, with results showing that patients with CDI had more frequent prior healthcare exposures. The likelihood of CDI during hospitalization was greater with healthcare visits, antibiotic use, and family exposures, but the association decreased with time between exposure and hospitalization. These findings were consistent across different CDI case definitions.

Summary: This review provides an overview of Clostridium difficile infection (CDI) in southeast Asia, including its prevalence, detection methods, antimicrobial susceptibility profiles, and the potential significance of a One Health approach. CDI is a common cause of hospital-acquired gastroenteritis worldwide, with higher rates observed in high-income countries. Lack of knowledge and awareness about CDI, along with antimicrobial abuse and inadequate education on appropriate usage, contribute to its evolution. Underreporting of CDI rates and misuse of antimicrobial agents pose a significant threat in regions like Indochina, while C. difficile RT 017 strain is endemic in southeast Asia.

Summary: This study analyzed data extracted from the Dhaka Hospital of International Centre for Diarrhoeal Disease Research in Bangladesh to investigate the prevalence and outcome of anemia among children under five years of age hospitalized for pneumonia/severe pneumonia. The study found that anemia was inversely related to increasing age and was independently associated with respiratory failure and fatal outcome during hospitalization. Interventional studies on the early management of anemia may be necessary to reduce morbidity and mortality in these children.

Summary: Two phase 1/phase 2 studies assessed the investigational bivalent Clostridioides difficile vaccine in healthy adults 50-85 years of age, with both formulations demonstrating robust immunogenicity. However, both studies were stopped early due to grade 3 injection site redness postdose 2 of the day regimen. Instead, an aluminum hydroxide-containing formulation of the vaccine candidate that was safe and immunogenic in phase 1 and 2 studies advanced to phase 3 studies.

Summary: This study analyzed the incidence of diabetes among 2898 HCV infected patients treated in Poland and evaluated its relationship with HCV genotype and fibrosis progression. The results indicate that HCV infection seems to be a risk factor for diabetes in individuals with more advanced liver fibrosis, older age, and male gender, with no differences regarding genotype, including genotype 3.

Summary: This study analyzed the differences in severity and clinical characteristics of COVID-19 in infants hospitalized in Poland from 2020 to 2021. It found that more infants were hospitalized in 2021, with an increase in comorbidities. Fever and respiratory symptoms were prevalent, and severe conditions were more common among infants with comorbidities. The study also noted a higher number of hospitalizations during the period of dominance of variants of concern, especially the delta variant.

Summary: The study compared the efficacy of 8-week and 12-week treatment regimens in non-cirrhotic hepatitis C patients infected with genotype 1. The results showed high effectiveness of ledipasvir/sofosbuvir regardless of treatment duration. Additionally, the study found that ribavirin was not necessary in this population and shortening the treatment duration significantly improved the safety profile of ledipasvir/sofosbuvir.
