Summary: The study examined FALD in the young adult population post-Fontan surgery and identified non-invasive markers associated with bridging fibrosis. Bridging fibrosis was observed in 35% of patients within the first decade post-Fontan and was linked to worse survival. Changes in platelet counts, even years before biopsy, and APRI/FIB-4 scores had modest discriminatory power in identifying patients with advanced fibrosis. Steatosis may represent an additional risk factor for disease progression in obese patients. Further prospective studies are needed to develop strategies for screening FALD in adolescents.

Summary: In this systematic review, abnormalities in multiple organ systems for patients with a longstanding Fontan circulation were examined. The study found that these patients may experience abnormalities in the nervous system, pituitary, kidneys, and musculoskeletal system. These abnormalities may be related to Fontan physiology, cyanosis, iatrogenic factors, or lifestyle. Health care providers should be aware of the potential for hypothyroidism, visual or hearing deficits, and sleep disordered breathing in Fontan patients.

Summary: A meta-analysis was conducted to evaluate the impact of transcatheter closure of Fontan fenestration on clinical outcomes. The results showed that closure of the fenestration led to improved resting and exercise oxygen saturation, as well as a modest improvement in exercise duration. However, it was associated with slightly higher cavo-pulmonary mean pressures.

Summary: In late Fontan failure in adults, ventricular dysfunction is a common feature while increases in pulmonary vascular resistance are rarely observed. Over time, ejection fraction decreases, mean pulmonary artery pressure and systemic ventricular end-diastolic pressure increase, but indexed pulmonary vascular resistance does not significantly change.

Summary: This study assessed the incidence of ventricular arrhythmias (VAs) in Fontan patients and their implications on transplant-free survival. The results showed that 11% of patients were diagnosed with VA, and VA was highly associated with the risk of transplant or death. Older age at Fontan and a history of right ventricular to pulmonary artery conduit were risk factors for VA. A diagnosis of VA in Fontan patients should lead to increased clinical surveillance.

A peak oxygen consumption less than 50% of predicted is an independent predictor of Fontan-related morbidity and mortality. The recovery of oxygen consumption after exercise (VO2RD) has not been extensively studied in youth with Fontan physiology.

Summary: Findings of the study showed that parents of children with chronic diseases experienced higher levels of anxiety, with COVID-19-specific stressors and parental psychological experiences being associated with increased anxiety, depression, and sleep problems. Moreover, parental burn-out was identified as a mediator for the impact of COVID-19-specific stressors on the well-being outcomes. This highlights the importance of interventions targeting parental burn-out in this vulnerable group.

Summary: This study evaluated serum CHI3L1, NGAL, TIMP-2, IGFBP7, and NephroCheck(R) as predictors of acute kidney injury (AKI) after pediatric cardiac surgery. The results showed that these markers could reliably predict the occurrence of AKI within 48 hours and 12 hours, respectively, after adjusting for urine dilution.

Summary: This study found that smaller patients are at a higher risk for tricuspid regurgitation (TR) recurrence when the absolute tricuspid annulus (TA) diameter is used as the primary criterion, as they tend to have a proportionally larger TA at the time of repair. An individualized approach guided by patient's body size might be more appropriate to indicate functional tricuspid regurgitation (FTR) correction and adjust for the annuloplasty sizing method.

Summary: We report a rare case of persistent left ventricular failure in a patient with anomalous left coronary artery from the pulmonary artery, who could not be weaned off the ventilator and inotropes after successful coronary reimplantation. Myocardial recovery was enhanced by pulmonary artery banding.
