The purpose of the present study was to determine the effect of low level laser therapy (LLLT) treatment before knee extensor eccentric exercise on indirect markers of muscle damage. Thirty-six healthy men were randomized in LLLT group (n = 18) and placebo group (n = 18). After LLLT or placebo treatment, subjects performed 75 maximal knee extensors eccentric contractions (five sets of 15 repetitions; velocity = 60A degrees A seg(-1); range of motion = 60A degrees). Muscle soreness (visual analogue scale-VAS), lactate dehydrogenase (LDH) and creatine kinase (CK) levels were measured prior to exercise, and 24 and 48 h after exercise. Muscle function (maximal voluntary contraction-MVC) was measured before exercise, immediately after, and 24 and 48 h post-exercise. Groups had no difference on kineanthropometric characteristics and on eccentric exercise performance. They also presented similar baseline values of VAS (0.00 mm for LLLT and placebo groups), LDH (LLLT = 186 IU/l; placebo = 183 IU/l), CK (LLLT = 145 IU/l; placebo = 155 IU/l) and MVC (LLLT = 293 Nm; placebo = 284 Nm). VAS data did not show group by time interaction (P = 0.066). In the other outcomes, LLLT group presented (1) smaller increase on LDH values 48 h post-exercise (LLLT = 366 IU/l; placebo = 484 IU/l; P = 0.017); (2) smaller increase on CK values 24 h (LLLT = 272 IU/l; placebo = 498 IU/l; P = 0.020) and 48 h (LLLT = 436 IU/l; placebo = 1328 IU/l; P < 0.001) post-exercise; (3) smaller decrease on MVC immediately after exercise (LLLT = 189 Nm; placebo = 154 Nm; P = 0.011), and 24 h (LLLT = 249 Nm; placebo = 205 Nm; P = 0.004) and 48 h (LLLT = 267 Nm; placebo = 216 Nm; P = 0.001) post-exercise compared with the placebo group. In conclusion, LLLT treatment before eccentric exercise was effective in terms of attenuating the increase of muscle proteins in the blood serum and the decrease in muscle force.

Effects of Kilohertz Frequency, Burst Duty Cycle, and Burst Duration on Evoked Torque, Perceived Discomfort and Muscle Fatigue A Systematic Review

Karenina Arrais Guida Modesto, Julia Aguillar Ivo Bastos, Marco Aurelio Vaz, Joao Luiz Quagliotti Durigan

Summary: This study investigated the effects of Kilohertz-frequency alternating current on muscle performance. Results showed that carrier frequencies up to 1 kHz elicited higher torque, while carrier frequencies between 2.5 and 5 kHz resulted in lower perceived discomfort. Shorter burst duty cycles (10%-50%) induced higher evoked torque and lower perceived discomfort. The study concludes that Kilohertz-frequency alternating current with carrier frequencies between 1 and 2.5 kHz and burst duty cycles less than 50% can generate greater evoked torque, while frequencies between 2.5 and 5 kHz along with burst duty cycles less than 50% can reduce perceived discomfort.

Summary: This study conducted a systematic review and meta-analysis of 20 studies and found that although foam rolling can increase the knee extensor concentric torque, it does not have a significant impact on myofascial tissue stiffness and muscle strength.

Distal overactivation of gastrocnemius medialis in persistent plantarflexion weakness following Achilles tendon repair

Carlos De la Fuente, Eduardo Martinez-Valdes, Emmanuel S. da Rocha, Jeam M. Geremia, Marco A. Vaz, Felipe P. Carpes

Summary: Structural alterations of the triceps surae and Achilles tendon following a repair surgery can lead to plantarflexion weakness and influence the activation strategies of the Gastrocnemius Medialis (GM) muscle. This study aimed to investigate the spatial activation patterns of GM in patients with persistent plantar flexion weakness one-year post-surgery. The results showed increased distal overactivation in GM in patients with persistent weakness, indicating altered activation patterns in this muscle group following Achilles tendon repair.

Pilates Method and/or Photobiomodulation Therapy Combined to Static Magnetic Field in Women with Stress Urinary Incontinence: A Randomized, Double-Blind, Placebo-Controlled Clinical Trial

Thiago De Marchi, Joao Vitor Ferlito, Ariane Cristina Turra, Sheila Flamia, Franciele de Bispo Magro, Maribel Luiza Pavelecini Donida, Michele Dilkin, Djessica da Silva, Vanessa Massia Ribas, Ernesto Cesar Pinto Leal Junior

Summary: This study aimed to investigate the effectiveness of the Pilates method for stress urinary incontinence (SUI) and the effects of photobiomodulation therapy associated with static magnetic field (PBMT/sMF) on pelvic floor muscles (PFM) in women with SUI. A three-arm, parallel randomized, double-blinded, placebo-controlled trial was conducted, and the results showed that the Pilates method, PBMT/sMF, and the combination of the two therapies were effective in improving the signs and symptoms of women with SUI.

Quadriceps Muscle Morphology Is an Important Determinant of Maximal Isometric and Crank Torques of Cyclists

Fabio Juner Lanferdini, Fernando Diefenthaeler, Andressa Germann Avila, Antonio Renato Pereira Moro, Stephan van der Zwaard, Marco Aurelio Vaz

Summary: The aim of this study was to investigate if quadriceps morphology, vastus lateralis muscle architecture, and muscle quality can account for differences in knee extensor maximal voluntary isometric contraction, crank torque, and time-to-exhaustion in trained cyclists. The results revealed that quadriceps muscle volume determined the corresponding maximal voluntary isometric contraction and crank torque, but did not explain the differences in time-to-exhaustion at maximal power output. Additionally, there were no significant differences in quadriceps morphological variables between the left and right legs of competitive cyclists.

Eccentric, but not concentric muscle contraction induce inflammation and impairs fibrinolysis in healthy young men

Summary: Different types of muscle contraction have different effects on the musculature and inflammatory responses. Acute increases in inflammation markers can increase the risk of thrombus formation and cardiovascular events. This study investigates the effects of concentric and eccentric exercise on hemostasis markers and CRP levels.

Summary: This study presents two suggestions for further research on body composition assessment, specifically focusing on the effects of intermittent fasting and time-restricted feeding on the gut microbiota in obese individuals. The findings indicate that both interventions can lead to changes in the Firmicutes to Bacteroidetes ratio and increase the abundance of protective strains, such as Lactobacillus spp. and Akkermansia, which may contribute to metabolic benefits against body mass gain.

Kilohertz Frequency Alternating Current Induces Less Evoked Torque and Less Neuromuscular Efficiency Than Pulsed Current in Healthy People: A Randomized Crossover Trial

Isabel de Almeida Paz, Francesca Chaida Sonda, Matias Frohlich, Joao Luiz Quagliotti Durigan, Marco Aurelio Vaz

Summary: This study compared the effects of pulsed current and kilohertz frequency alternating current on healthy individuals. The results showed that pulsed current generated higher torque, had higher neuromuscular efficiency, and similar discomfort levels. Therefore, the study suggests that pulsed current is the best choice for clinicians to use in NMES-based protocols.

Summary: This systematic review examined the effects of photobiomodulation therapy (PBMT) on pain intensity and disability in people with plantar fasciitis. PBMT was found to improve pain intensity in the short-term and was superior to extracorporeal shock wave therapy for pain relief. However, the evidence was inconclusive regarding PBMT's effect on disability in the long-term.
