Marco Merlo, Giulia Gagno, Anna Baritussio, Barbara Bauce, Elena Biagini, Marco Canepa, Alberto Cipriani, Silvia Castelletti, Santo Dellegrottaglie, Andrea Igoren Guaricci, Massimo Imazio, Giuseppe Limongelli, Maria Beatrice Musumeci, Vanda Parisi, Silvia Pica, Gianluca Pontone, Giancarlo Todiere, Camilla Torlasco, Cristina Basso, Gianfranco Sinagra, Pasquale Perrone Filardi, Ciro Indolfi, Camillo Autore, Andrea Barison

Summary: Cardiac magnetic resonance (CMR) is an essential tool for evaluating cardiomyopathy patients, providing precise cardiac data and non-invasive assessment of myocardial tissue for early diagnosis, individualized treatment, and prognostic risk assessment. New experimental CMR techniques are being developed to further expand our knowledge in the field of cardiomyopathies.

Summary: Heart failure is a major global cause of morbidity and mortality, with significant impact on quality of life and healthcare costs. Assessment of myocardial sympathetic innervation function plays a crucial role in prognosis assessment of heart failure patients. This review summarizes the latest clinical applications of iodine-123 metaiodobenzylguanidine (I-123-MIBG) imaging in heart failure patients and related comorbidities.

Summary: This study aimed to investigate whether a reduction in daily sedentary behavior improves insulin sensitivity in adults with metabolic syndrome. The results showed that reducing sedentary behavior by 40 minutes per day, while increasing moderate-to-vigorous physical activity by 20 minutes per day, resulted in slightly decreased fasting insulin levels, but had no effects on insulin sensitivity or body adiposity.

Summary: This study investigated the relationship between non-obstructive left main (LM) disease, diabetes mellitus (DM), and all-cause mortality (ACM). The results showed that while DM was associated with increased ACM, the presence of non-obstructive LM disease was not an independent risk factor for ACM.

Associations of sedentary time, physical activity, and fitness with muscle glucose uptake in adults with metabolic syndrome

Summary: The objective of this study was to investigate the associations between sedentary time, physical activity, and cardiorespiratory fitness with skeletal muscle glucose uptake. Sedentary time, physical activity, steps, and VO(2)max were found to be associated with muscle glucose uptake, but these associations became non-significant when adjusted for body fat-%. Therefore, body composition is a more important determinant of muscle glucose uptake in this population.

Summary: Myocardial bridging (MB) is a segment of coronary arteries that is usually free from atherosclerosis, while the adjacent proximal segment is prone to it due to local endothelial shear stress (ESS). This study used coronary computed tomography angiography and computational fluid dynamics to analyze the ESS in coronary arteries with MBs and investigate the association between atherosclerosis and hemorheological characteristics. The results showed that there was no significant difference in ESS between the proximal 5 mm to MBs and the region within MBs. No significant ESS variances were observed around the MB entrance in arteries with plaques, regardless of the morphological features of the bridged segment. MBs have homogeneous, atheroprotective ESS, which may explain the absence of atherosclerosis within bridged segments. The interplay between ESS and atherosclerosis is potentially similar in arteries with MB compared to those without bridges.

Summary: This explorative study aimed to investigate the role of microvasculature in critical limb-threatening ischaemia (CLTI) pathology. High-resolution imaging of CLTI patients and muscle samples from amputated limbs were used to describe microvascular pathology. The findings suggest that CLTI muscles have altered blood flow dynamics and tissue damage due to hypoxia-driven capillary enlargement.

Summary: This study evaluated the incremental value of positron emission tomography myocardial perfusion imaging (PET MPI) over coronary computed tomography angiography (CCTA) in predicting short- and long-term outcomes. The results showed that PET MPI data improved the prediction of adverse events beyond CCTA imaging for the first 4 years, but had no significant incremental prognostic value after that.
